N/A In febrile state, which is one of the stressful conditions, glucose tolerance is impaired. To evaluate the role of intracellular processing of insulin in the pathogenesis of glucose intolerance in febrile state, we measured insulin internalization and degradation during febrile state (n=4) and after recovery of febrile state in patients with febrile illness. Healthy persons (n=14) were used as a control group, Febrile patients and the control group had no evidence and history of DM. Febrile state is defined as body temperature above 38.3℃. Febrile patients and controls had similar basal insulin and C-peptide levels. The results were as follows: 1) In febrile state, insulin internalization and degradation were significantly decreased as compaired with the control group. 2) After recovery of febrile state, insulin internalization and degradation were normalized. The above data suggested that as a mechanism of glucose intolerance in febrile state, decreased intracellular processing of insulin is suggested.

N/A Familial hypercholesterolemia, a common autosomal dominant disorder affecting approximately 1 in every 500 persons, is caused by a mutaton in the gene for the LDL receptor. A 69-year-old woman was admitted to the hospital because of chest tightness and easy fatigue. Cutaneous xanthoma was presented on the right elbow and both ankles, Serum levels of total cholesterol and triglyceride were 668 mg9p and 986 mg9 respectively. Lipoprotein electrophoresis revealed type IV pattern. Her first daughter also showed hypercholesterolemia. After 6 months of glucose control, the serum triglyceride level was markedly decreased from 980 mg9. to 180㎎% Repeated lipoprotein electrophoresis revealed specific elevation of LDL-cholesterol fraction (Type lla pattern). LDL receptor binding assay in cultured fibroblasts showed markedly decreased activity of the LDL receptor.
고 Prolactin 혈증 환자의 당불내인성 ( Glucose Intolerance ) 의 기전에 관한 연구

N/A Prolactin may have a diabetogenic action. But underlying mechanisms how prolactin induce changes in glucose homeostasis are unknown. To ascertain the potential contribution of altered insulin receptor to this phenomenon, we studied insulin binding to erythrocyte and internalization of insulin in 5 patients of hyperprolactinemia and 8 healthy, non-pregnant female control. The results were as follows: 1) Glucose tolerance deteriorates in hyperprolactinemic patients compaired with nofmal control. 2) Plasma insulin and c-peptide concentration after oral GTT of hyperprolactinemic patients were not different that of normal control. 3) Insulin binding to erythrocyte was lower in hyperprolactinemic patients than normal control. 4) Internalization of insulin was lower in hyperprolactinemic patients than normal cotnrol. Above data suggested that hyperprolactinemia is accompanied with significant insulin resistance which is partly due to reduced number of insulin receptor without changes of binding affinity and reduced internalization of insulin.
