비인강 혈관 섬유종(Nasopharyngeal Angiofibroma)의 증례보고

Nasopharyngeal angiofibroma is an uncommon benign tumor of vascular fibrotic nature, but can be dealt with clinically malignancy because of expansive growth, locally invasive tendency, anatomical relationship, severe bleeding tendency in surgical procedures and tesistance to treatment. And it is generally considered to be a lesion which occurs exclusively on adolescent male. The authors had experienced a case of nasopharyngeal angiofibroma in 9-year-old boy with severe nasal obstruction. The tumor mass had occupied posterior wall of nasopharynx and extended to the nasal cavities. The tumor mass was red or purple-colored, round & multilobulated shape & its base was pedunculated. The tumor mass was easily removed by method of transpalatal approach under general ansthesia without severe bleeding. After treatment, nasal breathing & speech was completely corrected.

Tetanus is a rare, often fatal disease that is produced by liberation of an exotoxin by Clostridium tetani. It is found universally in street dust, soil, and the feces of many animals. Patients with mild disease have only trismus with or without minor and biief muscle spasms. Moderate disease is characterized by trismus, dysphagia, rigidity, and intermittent muscle spasms. With severe tetanus there are generalized convulsions. The most important therapeutic maneuvers are to insure airway patency and to provide mechanical ventilation to counter the tendency for respiratory arrest. Other therapeutic principles include antimicrobial durgs, active and passive immunization, wound debridement, and supportive care. A 61-year-old man with mouth opening limitation visited the emergency room. He had been injured on Lt. Zygoma area two weeks ago. We had diagnosed him as tetanus and treated successfully.

This is case presentation in three patients who have shown multiple cysts in jaws. We obtained the following results through the postoperative periodic check. 1. The multiple cysts were mainly appeared on the unerupted teeth, especially the 3rd molars. 2. Among the case reports, one case was multiple dentigerous cysts associated with maxillary left and right 3rd molar areas and mandibular left and right 3rd molar areas, the other case presented the primordial cyst on the right maxillary and left mandibular 3rd molar areas, the another case was multiple dentigerous cysts associated with impacted teeth on mandibular body areas. 3. The familiar tendency, peculiar feature of multiple cyst, was obtained only from one case report. 4. Though primary closure on the large-sized cysts, total case reports showed good healings.
農藥中毒後遺 筋强直으로 惹起된 顎顔面 變形 患者의 治驗例

A 32-year old man was admitted to the Department of Oral and Maxillofacial Surgery, Busan National University Hospital with complaints of unsightly appearance, inability of chewing and swallowing. The patient had attempted suicide by taking insecticide and persistent muscle contraction for over 2 years has brought collapse of lower arch, open bite deformity and disturbances of mastication and deglutition. After reducing the contraction of muscles by administration of muscle relaxants and tranquilizers, we carried out intraoral sagittal split osteotomy on both rami and suprahyoid myotomy. In this paper, we also reviewed the effects of insecticide poisoning to the nervous system and the dentofacial deformity caused by abnormal muscle forces.
