Background: This study evaluated clinical characteristics and outcomes in very elderly (=80 years of age) critically ill patients admitted to a medical intensive care unit (ICU) in a regional single tertiary hospital. Method: We retrospectively evaluated prospectively collected data in the medical ICU of Gyeongsang University Hospital in Korea for the period of December 2011 to May 2014. Patients were divided into =80 and <80 years of age and clinical characteristics and outcomes were compared among these patients. Results: A total of 468 patients were evaluated and 102 patients (21.7%) were =80 years of age. Overall mortality was 38.5% in the ICU and 44.7% in the hospital. There was no significant difference in ICU and in-hospital mortalities between those =80 years and those <80 years (34.9% vs. 39.5% for ICU mortality, 40.6% vs. 45.9% for in-hospital mortality). In addition, early (<48 h) ICU mortality was not significantly different between patients =80 years and patients <80 years (39.4% vs. 53.9%). Lengths of ICU and hospital stays were significantly longer in patients <80 years compared to patients =80 years (10.57 ± 19.96 days vs. 8.19 ± 8.78 days for ICU stay; 27.95 ± 39.62 vs. 18.17 ± 15.44 days for hospital stay). The extubation failure rate was not significantly higher in the =80 years group compared to the <80 years group (28.4% vs. 24.5%). The rate of withholding intensive care was significantly higher in patients =80 years compared to patients <80 years (22.9% vs. 11.8%). Conclusion: Clinical outcomes were not significantly different for very elderly critically ill patients compared to those of their younger counterparts in the medical ICU in this study.

Mouse hepatitis virus (MHV) is one of the major troublesome infectious diseases in laboratory mice. ELISA techniques generally have been shown to be more sensitive than other diagnostic methods for detection of MHV infection. Here, we developed an ELISA test method by using MHV type-2 strain and it gave reliable test results about detection of MHV infection in mice with high accuracy and low costs.
Isolation and Identification of Secondary Metabolites from the Ovary of Nelumbo nucifera

연꽃의 자방으로부터 80% MeOH로 추출하고, 얻어진 추출물을 n-hexane, ethyl acetate, n-butanol 및 H₂O으로 용매 분획하였다. 이 중 n-hexane 분획물에 대해 silica gel과 octadecyl silica gel column chromatography 및 Prep-HPLC system을 반복 수행하여 5 종의 물질을 분리 하였다. 각 화합물의 화학구조는 NMR, GC/MS및 ESI/MS 등의 분광학적 스펙트럼을 측정하고, 해석하여 1-eicosanol (1), cycloartenol (2), trans-squalene (3), pentadecanoic acid (4) 및 β-sitosterol (5)으로 동정하였다. 이 화합물들은 연꽃 자방추출물에서 처음으로 분리하고 동정하였으며 앞으로 이 화합물들에 대한 다양한 생리적 및 약리적 활성을 검토함으로써 건강기능성 식품 또는 의약품의 소재로서의 충분한 가치가 있다고 여겨진다. The ovary parts of Nelumbo nucifera were extracted in 80% methanol (MeOH), and the concentrated extract was then partitioned using n-hexane, ethyl acetate (EtOAc), n-butanol (n-BuOH), and H₂O, successively. Using an octadecyl silica gel (ODS) column, silica gel (SiO2) column chromatography, and a HPLC purification system, five compounds were isolated from the n-hexane fraction obtained from the extract of N. nucifera ovary. The chemical structures of the metabolites were determined using several spectroscopic methods, including NMR and GC/MS and MS of 1-eicosanol (1), cycloartenol (2), trans-squalene (3), pentadecanoic acid (4), and β-sitosterol (5). This study is a first attempt to isolate and identify secondary metabolites from the ovary of N. nucifera. The results indicated that the extract of N. nucifera ovary has biological effects, such as antibacterial and -tumor activity. Therefore, it could decrease the risk of HIV transmission through breastfeeding.

Purpose: Since pharmacologic agents are the mainstay of epilepsy treatment, drug compliance is one of the most important factors in seizure control. Once-daily levetiracetam (LEV) has been proven to have the same efficacy as that of an immediate-release (IR) formulation. A reduced number of doses may improve drug compliance and patient satisfaction. The aim of this study was to assess drug compliance and patient satisfaction when changing from IR to an extended-release (ER) formulation. Methods: Adolescent patients diagnosed with epilepsy who were taking LEV from 2018 to 2020 were included in this study. Compliance charts were reviewed retrospectively. We compared the frequency of seizure occurrence with the frequency of skipping doses and adverse effects before and after changing formulations. Changes in subjective compliance and satisfaction were also investigated. Results: Among 585 patients taking LEV, 44 were included in this study. The average age of the included patients was 16.4±2.0 years. There was no significant change in the average seizure frequency (P=0.491) after switching formulations. Objective compliance based on chart records significantly improved after switching formulations (P=0.021). Additionally, 26 of 44 patients mentioned how they felt about switching formulations, of whom 25 (96.2%) were satisfied with the ER formulation. Thirteen of 24 patients (54.2%) reported better compliance. Conclusion: Our study shows that the efficacy of LEV ER was similar to that of the IR formulation. The reduced number of medication doses improved patient satisfaction and medication compliance. LEV ER may be preferable in adolescent epilepsy patients.
