Signifi cance: Chronic constipation is an important component of clinical gastroenterology practice worldwide. Based on the defi nition, chronic constipation can affect from 2% to 27% of the population. It is physically and mentally troublesome for many patients, and can signifi cantly interfere with their daily living and well-being. Although only a proportion of patients with constipation seek medical care, most of them use prescribed or over-the-counter medication to improve their condition, one of which is Psyllium fi ber. We report a rare case of complete small bowel obstruction secondary to pharmacobezoar in an elderly patient with chronic intake of psyllium fi ber. Clinical presentation: Case is a 78 year old female, Cervical cancer survivor, who came in due to epigastric pain, with initial consideration of small bowel obstruction secondary to tumor recurrence, later diagnosed to have pharmacobezoar (Psyllium fi ber) intraoperatively. Conclusion: Psyllium fi ber drink is one of the few causes of pharmacobezoars and is a rare documented cause of small bowel obstruction. Pharmacobezoars form under a wide variety of circumstances and in a wide variety of patients. They are diffi cult to diagnose reliably. In patients with suspected gastrointestinal tract emptying problems, whether esophageal, gastric, small bowel, or colonic, the astute clinician should consider pharmacobezoar in the differential diagnosis. Rules for suspecting, diagnosing, and properly managing a pharmacobezoar are highly dependent on the pharmaceutical agent or agents involved. Becoming familiar with the limited data available on pharmacobezoars and maintaining a high index of suspicion may be the best way to improve diagnostic sensitivity and accuracy.
