      • Poster Session : PS 0146 ; Neurology : Memory Impairment in Young Diabetic Males: An Alarming Concern!

        Background: With diabetes rapidly affecting even the younger population makes it alarming for the physicians to understand its various complications. Although the effects of DM on peripheral nervous system are well established, the effects of Diabetes(DM) on CNS have been less clear. Even a modest effect on cognitive function will have signifi cant public health implications. Our study is one of the unique study from Indian subcontinent where we aimed to compare the memory profi le of 30-55 years old men with Type II DM with age matched controls and to fi nd whether any relationship exists between DM control and memory function in this age group. Methods: The study design was a cross sectional clinical study of Diabetic males of age group 30-55 years and age matched controls for memory testing. The study included men of 30-55 years diagnosed to have Type 2 DM.All patients with previous neurological disorders or hypothyroidism were excluded. All patients,after valid consent, were tested for memory testing using PGI Memory Scale. Test was objectively scored scores and ranged from 0-100.All patients with Diabetes were also tested for HbA1c. Results: Average memory score in case group was 80.76±8.75 and it was 87.68±7.39 in control group which was statistically signifi cant showing better memory scoring in control group. When long term diabetes control was compared it was seen that there is a trend of higher HbA1c having poor memory scoring. Conclusions: Our study showed presence of mild cognitive impairment in young males with diabetes which is an alarming revelation. This pathbreaking fi nding is an alarming indication to the clinicians and neurologist to look for this complication in young patients of diabetes. Also our study showed trend of higher HbA1c had poor memory scoring underscoring the importance of strict diabetes control especially in young diabetes.

        Background: Horizon towards a healthier tomorrow... from the healing touch of a doctor to the blessings from the patient to doctor…or doctor fees and medicolegal aspects…we as society often neglect the varied aspects of doctor patient relationship. We attempted to unravel the realms of this unsaid complex perplexing and yet ignored relationship. Methods: We attempted to discover the aspirations, motives, expectations and experiences on both sides. We interviewed 50 physicians, 20 students aspiring to becoming doctors and 50 patients who shared there viewpoints with us. We conducted a survey in this group to fi nd the most common reason to choose this profession and other relevant questions. Questionnaire to patients included questions which revealed their experiences and expectations from their doctor. We analyzed the survey using SSP4 software. Results: The interview and questionnaire on patient doctor relation revealed to us that 62%doctors had chosen this profession to serve the society, however only 45% found this profession satisfying and more than 50 % felt that they should be better paid for there services. On the other hand, patients think 40% doctor`s primary aim is to earn money and 35%believed that there doctor truly served the society. More than 90% of both patient and doctors believed that the relation of doctor and patient needs to be strengthened and both sides should make an effort for that. Conclusions: The fast pacing materialistic world seems to have eroded the era where this noble profession was always believed to have a golden healing touch. We appreciate the noble doctors who are relentlessly serving the society and hope both doctors and patients should understand each other`s point of view and move towards a beautiful and healthier tomorrow!

        Diagnostic Evaluation of Non-Interpretable Results Associated with rpoB Gene in Genotype MTBDRplus Ver 2.0

        Background: Line probe assay (LPA) is standard diagnostic tool to detect multidrug resistant tuberculosis. Noninterpretable (NI) results in LPA (complete missing or light wild-type 3 and 8 bands with no mutation band in rpoB gene region) poses a diagnostic challenge. Methods: Sputum samples obtained between October 2016 and July 2017 at the Intermediate Reference Laboratory, All India Institute of Medical Sciences Hospital, New Delhi, India were screened. Smear-positive and smear-negative culturepositive specimens were subjected to LPA Genotype MTBDRplus Ver 2.0. Smear-negative with culture-negative and culture contamination were excluded. LPA NI samples were subjected to phenotypic drug susceptibility testing (pDST) using MGIT-960 and sequencing. Results: A total of 1,614 sputum specimens were screened and 1,340 were included for the study (smear-positive [n=1,188] and smear-negative culture-positive [n=152]). LPA demonstrated 1,306 (97.5%) valid results with TUB (Mycobacterium tuberculosis) band, 24 (1.8%) NI, three (0.2%) valid results without TUB band, and seven (0.5%) invalid results. Among the NI results, 22 isolates (91.7%) were found to be rifampicin (RIF) resistant and two (8.3%) were RIF sensitive in the pDST. Sequencing revealed that rpoB mutations were noted in all 22 cases with RIF resistance, whereas the remaining two cases had wild-type strains. Of the 22 cases with rpoB mutations, the most frequent mutation was S531W (n=10, 45.5%), followed by S531F (n=6, 27.2%), L530P (n=2, 9.1%), A532V (n=2, 9.1%), and L533P (n=2, 9.1%). Conclusion: The present study showed that the results of the Genotype MTBDRplus assay were NI in a small proportion of isolates. pDST and rpoB sequencing were useful in elucidating the cause and clinical meaning of the NI results.
