        Optimization of Laccase Production from Bacillus sp. PK4 through Statistical Design of Experiments

        Statistical design of experiments was employed to optimize the media composition for the production of laccase from Bacillus sp. PK4. In order to find the key ingredients for the best yield of enzyme production from the selected eleven variables viz yeast extract, glucose, zinc sulphate, copper sulphate, potassium chloride, magnesium sulphate, calcium chloride, ferrous sulphate, sodium chloride, potassium dihydrogen phosphate (KH<sub>2</sub>PO<sub>4</sub>) and dipotassium hydrogen phosphate (K<sub>2</sub>HPO<sub>4</sub>), Plackett-Burman design was applied. The MgSO<sub>4</sub>, FeSO<sub>4</sub>, and CuSO<sub>4</sub> showed positive estimate, and their concentration optimized further. The steepest ascent method and Box-Behnken method revealed that 1.5 mM MgSO<sub>4</sub>, 0.33 g/l FeSO<sub>4</sub> and 1.41 mM CuSO<sub>4</sub> were optimal for the laccase production by Bacillus sp. PK4. This optimization strategy leads to enhancement of laccase production from 2.13 U/ml to 40.79 U/ml. Agro-wastes residues replace the carbon source glucose in the optimized media namely sugarcane bagasse, wheat bran, rice husk, and groundnut shell, among these groundnut shells (117 U/ml) was found to enhance the laccase production significantly. The laccase produced by Bacillus sp. PK4 was found to have the potential to degrade persistent organic pollutant benzo[a]pyrene.

        Effect of fourth hourly oropharyngeal suctioning on ventilator-associated events in patients requiring mechanical ventilation in intensive care units of a tertiary care center in South India: a randomized controlled trial

        Background: Mechanical ventilation (MV) is a necessary life-saving measure for critically ill patients. Ventilator-associated events (VAEs) are potentially avoidable complications associated with MV that can double the rate of death. Oral care and oropharyngeal suctioning, although neglected procedures, play a vital role in the prevention of VAE.Methods: A randomized controlled trial was conducted in the intensive care units to compare the effect of fourth hourly oropharyngeal suctioning with the standard oral care protocol on VAE among patients on MV. One hundred twenty mechanically ventilated patients who were freshly intubated and expected to be on ventilator support for the next 72 hours were randomly allocated to the control or intervention groups. The intervention was fourth hourly oropharyngeal suctioning along with the standard oral care procedure. The control group received standard oral care (i.e., thrice a day) and on-demand oral suctioning. On the 3rd and 7th days following the intervention, endotracheal aspirates were sent to rule out ventilator-associated pneumonia.Results: Both groups were homogenous at baseline with respect to their clinical characteristics. The intervention group had fewer VAEs (56.7%) than the control group (78.3%) which was significant at P<0.01. A significant reduction in the status of “positive culture” on ET aspirate also been observed following the 3rd day of the intervention (P<0.001).Conclusions: One of the most basic preventive strategies is providing oral care. Oropharyngeal suctioning is also an important component of oral care that prevents microaspiration. Hence, fourth-hourly oropharyngeal suctioning with standard oral care significantly reduces the incidence of VAE.

        Cervical polyp: evaluating the need of routine surgical intervention and its correlation with cervical smear cytology and endometrial pathology: a retrospective study

        ObjectiveTo determine the frequency of premalignant and malignant lesions in cervical polyps in order to examine whethercervical polyps need to be removed routinely and also to appraise its association with cervical smear cytology andendometrial pathologies. MethodsWe retrospectively re-examined the hospital records of 299 cases over a period of 5 years. All patients weresegregated into perimenopausal and postmenopausal groups according to their menopausal status. The groupswere compared in terms of histological results of cervical polyp biopsy, endometrial pathologies, and cervical smearcytology. Pearson’s χ2 test and Fisher’s exact test were used for statistical analysis, and a P-value of <0.05 was acceptedas statistically significant. ResultsIn the histopathological reports of cervical polyps, premalignant lesions were found in 2% of cases, and only 0.3%of malignant lesion was observed in menopausal women. Subsequently, no malignancy was noted in cervical smearcytology for both groups. Premalignant and malignant lesions in endometrial histopathology findings were 1.33%and 0.66%, respectively. There was a statistically significant association between the menopausal status of the patientand cervical smear cytology and endometrial pathology, but the histological findings were not statistically significantin relation to the symptomatic status of the patients. ConclusionAs per the results, we strongly suggest the removal of all cervical polyps with subsequent histological review. Theevaluation of cervical smear cytology prior to polypectomy can provide information about its malignancy potential. We believe that along with cervical polypectomy,endometrial sampling should be recommended,especially for postmenopausal women.
