Application of High-Flow Nasal Oxygenation during Endotracheal Intubation in Patients with Difficult Mask Ventilation

Anticipated difficult mask ventilation and intubation remains a daily challenge and a major cause of hypoxemia during anesthesia. Preoxygenation with a high-flow nasal oxygenation (HFNO) during induction of general anesthesia has recently become an appealing option to anesthesiologists. We describe the case of a 57-year-old male who underwent emergency flap salvage with angioplasty because of mandible area flap congestion. Mask ventilation was hardly implemented because mandible was irregular due to post-operative flap. We applied HFNO during induction. Although 5-min apnea time was needed for intubation, peripheral oxygen saturation was maintained at 100%. After surgery patient was discharged without respiratory complication. HFNO can secure a safe apnea time in difficult airway situations.

각 도시마다 스마트 도시구축을 목표로 운영하는 스마트 클러스트는 각각의 특화된 사업목적과 전략을 가지고 스마트 플랫폼을 운영하고 있는데, 각 거점 간의 소통이나 ‘스마트 도시구축’이라는 대전제를 위한 협업방안은 부재한 상황이다. 따라서 본 연구를 통해 현재 사업진행 중이거나 곧 추진을 앞둔 ICT 클러스트 간 연계 및 상호 협력적 운영, 통합적 성과관리를 위한 코디네이션 방안 중 하나로 거점 간 협력을 위한 시각화 플랫폼을 제안하고자 한다.

가속화되는 초고령화를 대비한 다양한 솔루션이 필요한 시점으로 고령자의 경우 안전사고 발생 시 일반 성인데 비해 빠른 대처가 어려워 생명을 잃게 되거나 노쇠현상으로 떨어진 체력, 인지능력 저하로 안전사고 발생률이 매우 높고, 회복과 치료에 긴 시간이 필요하다. 따라서 본 연구는 고령 1인 가구의 안전예방 및 안전관리를 위한 스마트 플랫폼 설계안을 제안한다.

최근 들어 전 세계를 위협하는 메르스, 사스, 코로나-19와 같은 감영병으로 인하여 대규모의 인명피해는 물론 감염병 예방과 대응 및 관리를 위한 막대한 사회적 비용을 지불하고 있다. 본 연구에서는 폭발적 확산으로 생명을 위협하는 감염병 특성을 고려하여 필요한 정보를 효율적으로 통합관리할 수 있고 즉시적인 업데이트와 가독성 높은 정보전달이 가능한 정보 시각화 플랫폼 설계 구성안을 제시한다. 이는 지역 거점을 중심으로 한 감염병 예방 자가관리, 검역관리, 질병관리, 인적관리, 물품관리, 환자 관리 등 6개 부분의 데이터를 통합적으로 관리 · 운영하며, 시민 중심의 감염병 대응체계 및 정확하고 신속한 검역 · 방역 업무 지원 · 연계를 목표로 한다.

Since 2000, there have been many changes in the residential types, such as an intelligent apartment which integrates both resting space and production space with comfortable, pleasant and safe environments based on internet systems. As the result of the rapid decrease in the birth rate and the prolonged lifespan around the world, it is expected that we will enter into an aging society by 2026. With such the expected change in the living patterns, many issues concerning the increased number of the elderly people have emerged as major social problems such as changes of family formation, its function and values, responsibility of caring for the aged, health and medical care, habitation policies and so on, and it is required to reconsider the social awareness on the living qualities of each old person. However, we have not yet satisfied such requirements with appropriate policies for the improvement of the living environment for them. It is considered, therefore, that we need to develop a new tool that, according to our goals, can give us a broad understanding on several considerations in designing the future homes for the elder people, making it possible to check the process changed by residents' needs in real time and apply it immediately to the space. As we have seen, it is required that spatial design process and design methodology should be considered in designing the homes for the aged for the pre-occupancy design evaluation.

네르발의 『동방 여행』에 나타난 오리엔탈리즘 연구

19세기, 동양은 더 이상 꿈속에서나 나올법한 경험을 바탕으로 묘사하고, 제시해야 하는 현실의 탐험 지역이 아니었다. 사람들은 19세기의 동양여행기나 동양에 대한 묘사 속에서, 앞서 만들어진 문화적 타자에 관한 담론을 발견하고 있었다. 동양이라는 타자에 대한 묘사는 이처럼 제국주의자들에 의한 차별적 관점에서 타자를 바라보는 이데올로기적 구조에 의해 불명확해졌다. 실제로, 동양 여행기들은 제국주의자들이 행하는 담론에 따라 인상, 성격, 풍습, 인종, 신앙같은, 매우 기이한 동양에 대한 묘사와 언급으로 단순한 관심 혹은 해석학적 관심을 차례로 나타내며, 다소 공들여 만들어진 총체의 작용소들로서 기능하는 것들에 대해 이야기 한다. 오리엔탈리즘의 저자 에드워드 사이드는 서양 제국주의라는 커다란 흐름에 편승하며 낙인찍는 용어들로 동양에 대해 이야기하는 19세기 모든 작가들을 비난한다. 그는 특히 네르발과 그의 작품을 여기에 인용한다. 모두가 네르발을 광인으로 취급할 때, 네르발은 사람들에게 자신의 정신건강을 증명하고자 동양으로 떠났다. 네르발은 지식인이자 주변인이었고 또한 휴머니스트였다. 그는 동양을 동양 그 자체로, 그의 체험을 통해 보고 싶어 했고, 또 그렇게 동양을 직접 체험했다. 그는 타자인 동양을 편견으로 대하는 사람들을 비판하였다. 동양은 모든 신앙의 기원이었고, 그에게 문학적 영감을 주었던 시인의 출신지였다. 작가이자 여행자였던 네르발은, 판단하기 이전에 타인의 신념, 신앙 나아가 미신까지도 이해하고, 받아들일 수 있는 정신적 태도를 배우게 된다. 그 메시지는 네르발보다 앞서 동양을 알았던 샤토브리앙이나 라마르틴의 그것과는 꽤 동떨어진 것이었다. 그의 동양 체류 동안, 그는 현실과 꿈을 혼동하기도 했으나, 그는 곧 사람들에게서 동양의 현실을 깨닫게 된다. 그의 작품 속에서 네르발은 동양의 삶, 풍습, 다양한 종교 그리고 특히 동양의 관용이라는 교훈에 대해 묘사한다. 네르발은 제국주의 오리엔탈리스트가 아니라 동양애호가였다. 우리는 에드워드 사이드가 생각하는 오리엔탈리즘을 완전히 반박하는 그의 동양관을 증명할 수 있다. 네르발에게 동양으로 가는 길은, 자신의 출생지의 문화적 정체성을 찾기 위함뿐만 아니라, 자기 자신의 정체성을 밝히기 위함이기도 했다. 그리고 동양 국가들의 상생이라는 관점으로 쓴 그의 동양 여행기 속에서 작가의 평화주의적인 정신은 분명하게 드러난다. 네르발은 동양인들과 더불어 동양에서, 진정한 동양적인 삶을 체험하고 싶었다. 그리고 그는 동양인의 생각을 이해하려 했고, 긍정적으로 그것을 받아들이려 노력했다. 우리시대는 세계의 평화를 위해 관용과 이해심이 필요하다. 에드워드 사이드와 네르발 모두 그것을 이해했고 실천하길 원했다. 그들의 세계시민주의적인 노력은 우리를 되돌아보게 한다. 네르발은 견자였다. 실제로 그는 미래를 예견하며 그의 시대에 이미 충분히 경험하였다. 그가 펼쳐 보일 수 있던 바로 그러한 관점 안에서, 우리는 네르발이라는 작가의 위대함과 현대성을 엿볼 수 있다. On retrouve dans les recits de voyages en Orient et plus largement dans la representation de l``Orient au XIXe siecle un discours sur l``Autre culturel qui est preconstruit. La representation de l``Autre oriental va ainsi etre obliteree par le mecanisme ideologique de l``Autre dans une perspective de discrimination par les imperialistes. En effet, les recits de voyages en Orient parlent, selon le registre de discours qu``ils pratiquent, d``impressions, de caracteres, de mœurs, de races, de croyances tous termes qui fonctionnent comme des operateurs de synthese plus ou moins elabores, manifestant tour a tour une preoccupation hermeneutique ou le simple souci de l``evocation, de la description de cet Orient si singulier. L``auteur de l``Orientalisme, E. W. Said a reproche a tous les auteurs du XIXe qui ont parle de l``Orient avec des termes stigmatisant qui participaient sur la grande vague imperialiste de l``Occident. Il y cite tout particulierement Nerval et son œuvre. Quand tout le monde prenait Nerval pour un fou, celui-ci est parti vers l``Orient pour prouver sa sante mentale au public. Nerval est un intellectuel, un marginal et aussi un humaniste. Il a critique les gens qui considerent avec preoccupation l``Autre, l``Orient. Il a vu et voulu decouvrir l``Orient par lui-meme, en faire sa propre experience. L``Orient est senti comme le lieu d``origine de toutes les croyances et du poete qu``il est s``en est inspire pour ecrire ses œuvres. Le voyageur-ecrivain a appris a comprendre avant de juger, a se mettre dans les dispositions d``esprit permettant d``accepter la foi des autres, les croyances, voire les superstitions. Le message est assez eloigne de celui de Chateaubriand ou de Lamatine qui ont precede le poete dans ces lointaines contrees. En passant sa vie en Orient, il a confondu son reve avec la realite. Cependant il realise bientot la realite de l``Orient qui voit en personnes. Dans son œuvre, Gerard de Nerval decrit la vie, les mœurs orientales, les religions diverses et surtout la lecon de tolerance de l``Orient. Nerval n``etait pas un orientaliste imperialiste mais un orientophile. On peut approuver ses ideologies orientalistes tout en niant son orientalisme, comme le pense E. W. Said. La route pour l``Orient, vers l``origine pour Nerval, en quete d``une identite culturelle de sa patrie occidentale, c``est egalement pour retablir son identite personnelle. Et l``esprit pacifiste de l``auteur intervient certainement dans son interpretation faite sous la vue d``une cohabitation globale des pays orientaux. Nerval a voulu experimenter la vraie vie orientale en Orient avec des orientaux. Et il a essaye de comprendre leur pensee et de l``accepter positivement. Notre epoque a besoin de tolerance et de comprehension afin d``etablir la paix dans le monde. E. W. Said et Nerval les ont compris et voulaient agir. Leurs efforts cosmopolites nous font reflechir. Nerval est un visionnaire. En effet, il a vecu pleinement dans son epoque tout en devinant le futur. C``est dans les points de vue qu``il a su developper que l``on peut apprehender la grandeur et la modernite de cet ecrivain.

최근 딥러닝의 발전은 인공지능기술의 인지, 학습, 추론, 행동 등과 같은 영역에 혁신적 진화를 거듭하고 있으며 시각지능은 이미지 속 사물의 종류 인식을 뛰어넘어 영상이나 이미지 속의 상황을 이해하기 시작하였다. 따라서 본 연구에서는 메타버스 안에서 공간정보 기반의 이동하는 객체가 시각지능을 통해 인지할 수 있는 상황 및 범위와 객체 간 관계성을 알아보고자 한다.
