        Background: Autosomal dominant polycystic kidney disease (ADPKD) is an inherited disorder affecting 1 in 1,000 people in the general population. It belongs to common reasons of chronic kidney disease (CKD), and is responsible for 10% of cases of the end stage renal disease. Apart from renal manifestations, changes in other organs may be present, including arterial hypertension, intracranial aneurysms, liver cysts, and others. Pain is a common symptom in ADPKD, affi icting as much as 60% of patients, and is frequently connected to the kidney. However, in our practice, the most severe cases of chronic lumbalgia and abdominal pain in ADPKD patients were of extrarenal origin. Methods: Report of 2 ADPKD cases, in which the lumbalgia and abdominal pain were initially regarded as caused by polycystic kidneys. Results: In a 55-year-old female with stage 3 CKD due to ADPKD, severe backache, and abdominal pain, requiring opioid analgetics, spondyloarthrosis and osteoporosis with compression fracture of the sixth thoracic vertebra were fi nally diagnosed. In a 51-year-old male with ADPKD, stage 2 CKD and chronic lumbalgia, scoliosis and advanced spondyloarthrosis were diagnosed. Conclusions: It should not be forgotten that the lumbalgia and abdominal pain in ADPKD may be of extrarenal origin. Other than polycystic kidneys sources of pain should be always sought in ADPKD subjects complaining of pain.
