        The Impact of Antibiotic-Loaded Bone Cement on Antibiotic Resistance in Periprosthetic Knee Infections

        Background: Antibiotic-loaded bone cement (ALBC) is commonly used in total knee arthroplasty (TKA), especially among high-risk patients. While previous studies have reported on the efficacy of ALBC in reducing the rate of periprosthetic joint infection (PJI), its impact on antibiotic resistance has not been determined. The purpose of this study was to investigate antibiotic resistance among organisms causing PJIs after TKA in which ALBC was utilized. Methods: A retrospective review from December 1998 through December 2017 identified 36 PJIs that met inclusion criteria. Patients with culture-negative infection and unknown cement type were excluded. Patient characteristics, infecting organism, and antibiotic susceptibilities were recorded. ABLC included an aminoglycoside in all cases. Results: There was no difference in the type of PJI between the 2 groups. Staphylococcus species was the most commonly isolated, with 9 of 16 cases (56.3%) using non-ALBC and 14 of 20 (65.0%) cases using ALBC. Of those infected with Staphylococcus , there was no significant difference in antibiotic susceptibilities between groups. Overall, there were only 3 cases where the infecting organism was aminoglycoside resistant (standard cement, 1; ALBC, 2). Conclusions: These results suggest that the use of ALBC does not increase the risk of antibiotic resistance or affect the pattern of infection, even as the use of ALBC continues to increase, particularly among high-risk patients.

        Quasi-Normal Relations - a New Class of Relations

        In this paper, concepts of quasi-normal and dually quasi-normal relations are introduced. Characterizations of these relations are obtained. In addition, particulary we show that the anti-order relation ${\nleqslant}$ ($={\leqslant}^C$) is a (dually) quasi-normal relation if and only if the partially ordered set (X, ${\leqslant}$) is an anti-chain.

        Bridging Solutions for a Heterogeneous WiMAX-WiFi Scenario

        Recently, the metropolitan area network (MAN) has attracted much attention in telecommunication research and has emerged as one of the most important research topics in the community. Several standards representing the first step for developing metropolitan networks have been published; IEEE 802.16 (WiMAX) has taken a relevant role in reaching the goal of realizing a full-service network all over a urban and suburban area. At the same time, the wireless local area networks (WLAN) have been widely used for in-home or short range communications, mainly basing on the IEEE 802.11 (WiFi) standard. A consequence is the increasing interest in interworking technology, that allows an interconnection between different standards by maintaining certain properties, mainly in terms of quality of service (QoS). One of the major issues is to design bridging devices capable of transparently interconnect different wireless technologies. In this paper, we propose two interconnection bridging solutions between WiMAX and WiFi links; the first is more based on the concept of maintaining a certain end-to-end QoS level independently from the wireless technologies used. The second method is more devoted to the reduction of the implementation complexity at the cost of no QoS assurance. The performance of the two methods are compared by resorting to computer simulations showing the advantages of each one technique.

      • Design for Robustness and Cost Effectiveness: The Case of an Optical Profilometer

        The paper presents the design of an optical profilometer, a device used for the reconstruction of the micro-geometry of mechanical parts in applications where high precision is needed. The design is based on Robust Design, a major' methodology for quality improvement of engineering systems. Several design solutions, namely different hardware and software setups of the device, are compared in order to select a configuration realising a desired trade-off between performance and cost. The peculiarity of the design strategy is the use of a computer model of the measurement process where the physical part of the process is simulated. This allows for an extensive exploration of the design space, thus opening the way to product innovation.

        Initial Experience Using the New pHLO 0.072-inch Large-Bore Catheter for Direct Aspiration Thrombectomy in Acute Ischemic Stroke

        Purpose: A direct aspiration, first pass technique (ADAPT) has been introduced as a rapid and safe thrombectomy strategy in patients with intracranial large vessel occlusion (LVO). The aim of the study is to determine the technical feasibility, safety, and functional outcome of ADAPT using the newly released large bore pHLO 0.072-inch aspiration catheter (AC; Phenox).Materials and Methods: We performed a retrospective analysis of data collected prospectively (October 2019–November 2021) from 2 comprehensive stroke centers. Accessibility of the thrombus, vascular recanalization, time to recanalization, and procedure-related complications were evaluated. National Institutes of Health stroke scale scores at presentation and discharge and the modified Rankin scale (mRS) score at 90 days post-procedure were recorded.Results: Twenty-five patients (14 female, 11 male) with occlusions of the anterior circulation were treated. In 84% of cases, ADAPT led to successful recanalization with a median procedure time of 28 minutes. In the remaining cases, successful recanalization required (to a total of 96%; modified thrombolysis in cerebral infarction score 2b/3) the use of stent retrievers. No AC-related complications were reported. Other complications included distal migration of the thrombus, requiring a stent-retriever, and symptomatic PH2 hemorrhage in 16% and 4%, respectively. After 3 months, 52% of the patients had mRS scores of 0–2 with an overall mortality rate of 20%.Conclusion: Results from our retrospective case series revealed that thrombectomy of LVOs with pHLO AC is safe and effective in cases of large-vessel ischemic stroke. Rates of complete or near-complete recanalization after the first pass with this method might be used as a new benchmark in future trials.
