        Total robotic right hepatectomy for multifocal hepatocellular carcinoma using vessel sealer

        Rapid adoption of a robotic approach as a minimally invasive surgery tool has enabled surgeons to perform more complex hepatobiliary surgeries than conventional laparoscopic surgery. Although various types of liver resections have been performed robotically, parenchymal transection is challenging as commonly used instruments (Cavitron Ultrasonic Surgical Aspirator [CUSA] and Harmonic) lack articulation. Further, CUSA also requires a patient-side assistant surgeon with hepatobiliary laparoscopic skills. We present a case report of total robotic right hepatectomy for multifocal hepatocellular carcinoma in a 70-year-old male using ‘Vessel Sealer’ for parenchymal transection. Total operative time was 520 minutes with a blood loss of ~400 mL. There was no technical difficulty or instrument failure encountered during surgery. The patient was discharged on postoperative day five without any significant complications such as bile leak. Thus, Vessel Sealer, a fully articulating instrument intended to seal vessels and tissues up to 7 mm, can be a promising tool for parenchymal transection in a robotic surgery.

        Efficient plant regeneration from immature embryo cultures of Jatropha curcas, a biodiesel plant

        Jatropha curcas L. (Physic nut) is a commercially important non-edible oil seed crop known for its use as an alternate source of biodiesel. In order to investigate the morphogenic potential of immature embryo, explants from four developmental stages were cultured on medium supplemented with combinations of auxins and cytokinins. It was found that the size of embryo is critical for the establishment of callus. Immature embryos (1.1-1.5 cm) obtained from the fruits 6 weeks after pollination showed a good response of morphogenic callus induction (85.7%) and subsequent plant regeneration (70%) with the maximum number of plantlets (4.7/explant) on Murashige and Skoog's (MS) medium supplemented with IBA (0.5 $mg\;l^{-1}$) and BA (1.0 $mg\;l^{-1}$). The above medium when supplemented with growth adjuvants such as 100 $mg\;l^{-1}$ casein hydrolysate + 200 $mg\;l^{-1}$ L-glutamine + 8.0 $mg\;l^{-1}$ $CuSO_4$ resulted in an even higher frequency of callus induction (100%). Plant regeneration (90%) with the maximum number of plantlets (10/explant) was achieved on MS medium supplemented with 500 $mg\;l^{-1}$ polyvinyl pyrrolidone + 30 $mg\;l^{-1}$ citric acid + 1 $mg\;l^{-1}$ BA + 0.5 $mg\;l^{-1}$ Kn + 0.25 $mg\;l^{-1}$ IBA. It was observed that plantlet regeneration could occur either through organogenesis of morphogenic callus or via multiplication of pre-existing meristem in immature embryos. The age of immature embryos and addition of a combination of growth adjuvants to the culture medium appear to be critical for obtaining high regeneration rates. Well-developed shoots rooted on half-halfstrength MS medium supplemented with 0.5 $mg\;l^{-1}$ IBA and 342 $mg\;l^{-1}$ trehalose. The rooted plants after acclimatization were successfully transferred to the field in different agro-climatic zones in India. This protocol has been successfully evaluated on five elite lines of J. curcas.

      • Influence of Ni doping on PtNi nanoparticles: Synthesis, electronic/atomic structure and photocatalyst investigations

        Carbon-supported Pt and PtNi nanoparticles (NPs) were synthesized using a borohydride reduction method. Structural properties were studied by synchrotron X-ray diffraction (XRD) and the size/shape of the NPs was determined by transmission electron microscope (TEM). X-ray absorption spectroscopy with its two amendments; X-ray absorption near edge structure (XANES) and extended X-ray absorption fine structure (EXAFS), has been employed to investigate the local electronic/atomic structure surrounding the Pt and Ni atoms. XANES results, at Pt L<SUB>3</SUB>-edge and Ni K-edge, have shown fractional oxidation of Pt and Ni atoms. The Pt3Ni1NPs have exhibited a lower bond distance of Pt-Ni shell and higher coordination number of Pt-Ni shells, indicating the alloy formation between Pt and Ni. We further have demonstrated that the Pt and PtNi NPs can serve as effective photocatalysts towards the degradation of water pollutant dye (methyl orange (MO)). By considering the interband charge-transfer of Pt (5d →6sp), a tentative mechanism is proposed to understand the photocatalytic degradation of MO dye molecules by Pt/PtNi NPs under the light irradiation.

      • Electronic structure and dielectric properties of ZrO<sub>2</sub>-CeO<sub>2</sub> mixed oxides

        <P><B>Abstract</B></P> <P>ZrO<SUB>2</SUB>–CeO<SUB>2</SUB> mixed ceramics were studied for their local electronic structure and dielectric properties. X-ray diffraction (XRD), scanning electron microscope (SEM), X-ray absorption fine structure (XAFS) and frequency dependence dielectric constant measurements were performed on the samples. Rietveld refinements and quantitative phase analysis of ZrO<SUB>2</SUB>-CeO<SUB>2</SUB> ceramics showed an increased concentration of tetragonal ZrO<SUB>2</SUB> and cubic Zr<SUB>0.5</SUB>Ce<SUB>0.5</SUB>O<SUB>2</SUB> phases at the expense of parent monoclinic ZrO<SUB>2</SUB>. The morphology of the ZrO<SUB>2</SUB>-CeO<SUB>2</SUB> ceramics remained invariant for all of the fractional percentages of CeO<SUB>2</SUB> loading. O K-edge and Ce M<SUB>5,4</SUB>-edge NEXAFS showed the oxygen vacancies and Ce<SUP>3+</SUP>-Ce<SUP>4+</SUP> redox pair formation in ZrO<SUB>2</SUB>CeO<SUB>2</SUB> ceramics, which is further corroborated by Ce L<SUB>3</SUB>-edge XAFS.         Microwave-assisted synthesis of magnesium oxide nanoflakes via green chemistry approach using Ficus Racemosa leaf extract: characterization and antibacterial activity

        Global development of nanostructured bioceramics with enhanced functionalities has provided an insight into orthopedic, dental, and other biomedical applications. The present work reports a cost-effective and eco-friendly method for the green synthesis of magnesium oxide (MgO) nanoflakes (NFs) using Ficus Racemosa leaf extract. The NFs were subjected to different characterization techniques like X-ray diffraction, field emission scanning electron microscopy, energy-dispersive spectroscopy, Fourier-transform infrared spectroscopy, ultraviolet–visible, and photoluminescence spectroscopy to detect their structural, physical, chemical, and elemental characteristics. The powder X-ray diffraction of MgO NFs revealed the single phase with an average crystallite size of 24 nm. The Williamson–Hall plot also confirmed the crystallite size as 28 nm, nearly the same as obtained through Debye–Scherrer (24 nm) formula. Field emission scanning electron microscopy revealed the flake-like structure with an average thickness of 3 nm, and the average length was depicted as 280 nm. The ultraviolet–visible absorption spectra of MgO NFs showed a maximum wavelength of around 275 nm, and photoluminescence spectroscopy was used to observe the fluorescent properties. The antibacterial activity of MgO NFs against both S. aureus and E. coli bacteria demonstrated a significant reaction, making it a promising agent for several biomedical applications.

        Creating Datasets in Information-Poor Environments: Patterns of Collective Violence in Indonesia, 1990–2003

        Indonesia has witnessed explosive group violence in recent years, but unlike its plentiful economic statistics, the data on conflict are remarkably sketchy. Because the New Order (1966–1998) wanted to give the appearance of order and stability, it did not believe in publishing reports on group conflict, nor did it allow researchers and nongovernmental organizations to probe the patterns and causes of conflict. This article is based on the first multiyear dataset ever constructed on group violence in Indonesia. Following, and adapting for Indonesian conditions, methodologies developed and used elsewhere, we cover the years 1990–2003, split the data into various categories, and identify the national, regional, and local patterns of collective violence. Much that we find is surprising, given the existing theories and common perceptions about violence in Indonesia. Of the several conclusions we draw, the most important one is that group violence in Indonesia is highly locally concentrated. Fifteen districts and cities (kabupaten and kota), in which a mere 6.5 percent of the country's population lived in 2000, account for as much as 85.5 percent of all deaths in group violence. Large-scale group violence is not as widespread as is normally believed. If we can figure out why so many districts remained reasonably quiet, even as the violent systemic shifts—such as the decline of the New Order—deeply shook fifteen districts causing a large number of deaths, it will advance our understanding of the causes of collective violence in Indonesia.

        Electrical resistivity of alkali metal doped manganites LaxAyMnwO3(A = Na, K, Rb) : Role of electron-phonon, electron-electron and electron-magnon interactions

        The electrical resistivity behaviour of alkali metal (Na, K, Rb) substitutions at La site in LaxAyMnwO3(A = Na, K, Rb) manganites is studied caused by electronephonon, electroneelectron and electron emagnon scattering. Substitutions affect average mass and ionic radii of A-site and hence resulting lattice and optical phonon softening. Estimated resistivity compared with reported metallic resistivity,accordingly rdiff. = [rexp - {r0 + reeph (=rac + rop)}], infers electroneelectron and electronemagnon dependence over most of the temperature range. Electronephonon contribution indicates that alkali metal K doping provoked larger lattice distortion, while electroneelectron interaction is more dominating process for Na and Rb doped compound favouring motion of excess charge carrier. Semiconducting nature is discussed with variable range hopping and small polaron conduction model. The change in activation energies and the density of states at the Fermi-level is consistently explained by cationic disorder and Mn valence.

      • Management of Chronic Pancreatitis- Step Up?

        Aims: Endoscopic treatment is used in several units prior to surgical treatment for pain in chronic pancreatitis. There is limited information on patients who undergo surgical ‘salvage’ after endoscopic failure. We conducted a comparative study between patients who had undergone surgery after prior non-surgical intervention and upfront surgery. Methods: Patients who underwent surgical drainage in our institution over the last 6 years were reviewed, in two groups- Group A (n=29) - surgery with prior non-surgical interventions (ESWL and/or endoscopy); Group B (n=79) - upfront surgery. Pain scores and QOL scores were measured prospectively. Results: The two groups were comparable for baseline characteristics except that group A patients had more strictures/ stones in body and tail region (P<0.05). Short term morbidity was more in Group A vs Group B (65% vs 26%, P<0.01) - wound infection (45% vs 10%, P<0.01; Gastroparesis (10% vs nil, P<0.01). On long term followup complete pain relief was worse in Group A patients (37% vs 68%, P=0.05). Quality of life scores (WHOQOL- BREF) in social domain was significantly better in Group B. Improvement in exocrine and endocrine insufficiency was similar in both groups. Conclusions: Patients with chronic pancreatitis who undergo ‘salvage’ surgery after non-surgical interventions are at increased risk of postoperative morbidity, lower quality of life and poor pain control as compared to those who undergo upfront surgery. Patients with pancreatic body/tail strictures/ stones have poor outcomes with non-surgical interventions and may be considered for upfront surgery.
