        Comparison of the Efficacy of Two Microsphere Embolic Agents for Transcatheter Arterial Chemoembolization in Hepatocellular Carcinoma Patients

        Purpose Transarterial chemoembolization (TACE) delivers cytotoxic drugs intra-arterially and induces ischemic necrosis by arterial embolization. Embolization is achieved using a variety of agents that differ widely in particle size and range, deformation, and in vivo arterial distribution. The clinical significance of these differences has not been thoroughly characterized. The present study is to compare the efficacy of Embosphere and Embozene microspheres in TACE therapy for hepatocellular carcinoma. Materials and Methods This retrospective study includes 108 hepatocellular carcinoma (HCC) patients who received TACE/doxorubicin with Embozene (70 patients) or Embosphere (38 patients) at a single medical center. Patient outcomes, including liver function, tumor size, tumor response, and complications after treatment, were analyzed. The change in total target lesion size and tumor response was evaluated according to embolization agent and clinical characteristics. Results The postoperative glutamate oxaloacetate transaminase (mean, 194.5 vs. 147.5; p=0.032) and bilirubin (mean, 1.11 mg/dL vs. 0.73 mg/dL; p=0.016) were higher among patients treated with Embozene, the decrease in the number (55.86±25.55% vs. 41.81±38.51%, p=0.027) and size (56.37±25.91 mm vs. 43.44±37.89 mm, p=0.001) of liver tumors relative to baseline was greater in these patients than in those treated with Embosphere. These greater antitumor effects were achieved using lower doses of doxorubicin than for treatment with Embozene. Minor complications were more common among patients treated with Embosphere than with Embozene. Conclusion These results suggest that Embozene is more efficacious than Embosphere for HCC treatment using TACE/doxorubicin.

        Sensor-Based Path Planning for Planar Two-identical-Link Robots by Generalized Voronoi Graph

        일반화된 보로노이 그래프(GVG)는 자율 주행 로봇을 위한 일종의 로드맵으로서. GVG는 선서에다 받은 정보만 사용하여 작업 공간거리의 계산에 따라 정의를 한다. 로봇은 장애물까지의 최대 거리를 검출할 수 있기 때문에 포인트 뷰에서 GVG의 최적은 정출 몇 장애물 회피이다. 로봇의 경우에는, GVG는 가장 안전적인 길이라고 할 수 있다. 따라서 높이 링크 로봇의 GVG가장거리에 대한 연구가 매우 필요하다. 기존 연구에서 점(point) 로봇을 위한 GVG(point-GVG)와 로드 로봇을 위한 GVG(rod-GVG)가 발표되었다. 이 논문은 더 고차원의 로봇인 두 개의 동일 링크가 관절로 연결된 (tow-identical-link; L2) 로봇을 위한 GVG(L2-GVG)에 대한 연구이다. L2-GVG는 미지의 평면 작업공간에서 움직이는 L2로봇의 짜임새 공간R<sup>2</sup>×T<sup>2</sup>상에서 로드맵을 생성하되, 이전 연구와 마찬가지로 지역적 센서 정보만을 이용해 로봇이 스스로 주행하면서 맵을 만들어 낸다. 이 논문에서는 이전 point-GVG와 rod-GVG에서는 나타나지 않는, 관절이 존재하여 생기는 복잡한 특성에 대해서 분석한다. 이는 다관절 로봇으로의 확장에 중요한 초석이 될 것이다. The generalized Voronoi graph (GVG) is a topological map of a constrained environment. This is defined in terms of workspace distance measurements using only sensor-provided information, with a robot having a maximum distance from obstacles, and is the optimum for exploration and obstacle avoidance. This is the safest path for the robot, and is very significant when studying the GVG edges of highly articulated robots. In previous work, the point-GVG edge and Rod-GVG were built with point robot and rod robot using sensor-based control. An attempt was made to use a higher degree of freedom robot to build GVG edges. This paper presents GVG-based a new local roadmap for the two-link robot in the constrained two-dimensional environment. This new local roadmap is called the two-identical-link generalized Voronoi graph (L2-GVG). This is used to explore an unknown planar workspace and build a local roadmap in an unknown configuration space R<sup>2</sup> × T<sup>2</sup> for a planar two-identical-link robot. The two-identical-link GVG also can be constructed using only sensor-provided information. These results show the more complex properties of two-link-GVG, which are very different from point-GVG and rod-GVG. Furthermore, this approach draws on the experience of other highly articulated robots.

        Sensor-based Path Planning: The Two-Identical-Link Hierarchical Generalized Voronoi Graph

        We present a new roadmap based on a generalized Voronoi graph (GVG) for two-identical-link ( L2) robots to explore an unknown planar environment. It is called the L2 hierarchical generalized Voronoi graph (L2-HGVG). The L2 robot is formed by two equal length rods and a revolute joint between them. The configuration of the robot possesses four degree-of-freedom, two for position and two for orientation, and hence the roadmap is one-dimensional in an unknown configuration space R2 × T2. L2-HGVG is defined in terms of workspace distance measurements using only sensor information without prior knowledge of the environment, with the robot having the maximum distance from obstacles, and is therefore optimum in a point of view for exploration and obstacle avoidance.

      • Novel Quantum-Inspired Co-evolutionary Algorithm

        Co-evolutionary mechanism is now used into evolutionary algorithms and provides these algorithms the power to promote the convergence. In order to promote the performance of the traditional quantum-inspired evolutionary algorithm (QEA), we proposed a novel quantum-inspired co-evolutionary algorithm (NQCEA), in this paper. The quantum state population is firstly divided into multiple sub-populations, which complete the evolution processes independently. In each evolution cycle, every sub-population will produce an elitist individual, which is then selected to construct an elite library. Subsequently, these individuals in this elite library can help the poor sub-population to find the global optimal solution or near-optimal solution. In addition, a diversity indicator is defined for every sub-population and is used to measure the diversity of the corresponding sub-population. As for the sub-population with poor diversity, the mutation strategies are implemented in order to expand its diversity and improve its global search ability. Finally, the NQCEA is compared with the traditional QEA to test their performance. Experiments are performed on the global numerical optimization functions and the simulation results indicate that this new algorithm has the characteristics of good global search capability and more stable performance than the traditional QEA.

      • Clinical Evaluation of Human Papillomavirus Detection by careHPV<sup>TM</sup> Test on Physician-Samples and Self-Samples using The Indicating FTA Elute® Card

        Objective: To make the clinical evaluation of a solid-state human papillomavirus (HPV) sampling medium in combination with an economical HPV testing method ($careHPV^{TM}$) for cervical cancer screening. Methods: 396 women aged 25-65 years were enrolled for cervical cancer screening, and four samples were collected. Two samples were collected by woman themselves, among which one was stored in DCM preservative solution (called "liquid sample") and the other was applied on the Whatman Indicating FTA $Elute^{(R)}$ card (FTA card). Another two samples were collected by physician and stored in DCM preservative solution and FTA card, respectively. All the samples were detected by $careHPV^{TM}$ test. All the women were administered a colposcopy examination, and biopsies were taken for pathological confirmation if necessary. Results: FTA card demonstrated a comparable sensitivity of detecting high grade Cervical Intraepithelial Neoplasia (CIN) with the liquid sample carrier for self and physician-sampling, but showed a higher specificity than that of liquid sample carrier for self-sampling (FTA vs Liquid: 79.0% vs 71.6%, p=0.02). Generally, the FTA card had a comparable accuracy with that of Liquid-based medium by different sampling operators, with an area under the curve of 0.807 for physician &FTA, 0.781 for physician &Liquid, 0.728 for self & FTA, and 0.733 for self &Liquid (p>0.05). Conclusions: FTA card is a promising sample carrier for cervical cancer screening. With appropriate education programmes and further optimization of the experimental workflow, FTA card based self-collection in combination with centralized $careHPV^{TM}$ testing can help expand the coverage of cervical cancer screening in low-resource areas.

      • Human Papillomavirus Vaccine Awareness, Acceptability, and Decision-Making Factors among Chinese College Students

        Background: College students are recommended as the target groups for catch-up human papillomavirus (HPV) vaccination. Systematical exploration of awareness, acceptability, and decision-making factors of HPV vaccination among Chinese college students has been limited. Materials and Methods: A multi-center survey was conducted in mainland China between November 2011 and May 2012. College students aged 18-22 years were stratified by their grade, gender, and major for sampling. Socio-demographic and HPV-related information such as knowledge, perceptions, acceptability, and attitudes were collected through a questionnaire. Results: A total of 3,497 undergraduates completed the questionnaire, among which 1,686 were males. The acceptability of the HPV vaccine was high (70.8%). Undergraduates from high-level universities, at lower grade, or with greater prior knowledge of HPV vaccines showed higher acceptability of HPV vaccination ($p_{trend}$ <0.001). Additionally, undergraduates with vaccination experience outside the National Expanded Program on Immunization (OR=1.29; 95%CI: 1.10-1.51) or fear of HPV-related diseases (OR=2.79; 95%CI: 2.28-3.41) were more willing to accept HPV vaccination. General knowledge of HPV vaccine was low among undergraduates, and safety was a major concern (71.05%). The majority of students wished to pay less than 300RMB for HPV vaccine and chose the Chinese Center for Disease Control and Prevention as the most appropriate venue for vaccination. Conclusions: Although most undergraduates demonstrate positive attitudes towards HPV vaccination, challenges pertaining to introduction exist in China. Corresponding proactive education and governmental subsidy to do so are urgently needed by this age-group population. Suggestions and potential strategies indicated may help shape the future HPV vaccination program in China.

        Missile two-loop acceleration autopilot design based on L<SUB>1</SUB> adaptive output feedback control

        This article documents the design of a novel two-loop acceleration autopilot based on L1 adaptive output feedback control for tail-controlled missiles. The inner loop is an adaptive angle-of-attack tracking loop and the outer loop is the traditional PI controller for error compensation. A systematic low-pass filter design procedure is provided for minimum phase system and is applied to the inner loop design while the parameters of the outer loop are obtained from the multi-objective optimization problem. The effectiveness of the proposed autopilot is verified through numerical simulations under various conditions.

      • Clinical Observation on Recombinant Human Endostatin Combined with Chemotherapy for Advanced Gastrointestinal Cancer

        Objective: To explore the clinical efficacy and toxic and side effects of recombinant human endostatin (rhendostatin/endostar) combined with chemotherapy in the treatment of advanced gastric cancer. Materials and Methods: A total of 70 patients with advanced gastrointestinal adenocarcioma confirmed by histopathology and/or cytological examination were divided into group A (37 patients) and group B (33 patients). Patients in group A were given intravenous drip of 15 mg endostar added into 500 mL normal saline, once every other day until the cessation of chemotherapy or patients' maximal tolerance to chemotherapy. Patients in group B received chemotherapy alone. Two groups selected the same chemotherapy regimens. FOLFIRI scheme: 90-min intravenous drip of $180mg/m^2$ irinotecan, intravenous drip of $200mg/m^2$ calcium folinate (CF) and $400mg/m^2$ 5-fluorouracil (5-Fu) on d1, and continuous intravenous pumping of 2 $400mg/m^2$ 5-Fu for 46 h. FOLFOX4 scheme: intravenous injection of $85mg/m^2$ oxaliplatin (L-OHP), $200mg/m^2$ calcium folinate (CF) and $400mg/m^2$ 5-FU on d1 for 2 h, and then continuous intravenous pumping of 2 $400mg/m^2$ 5-Fu for 46 h. XELOX scheme: oral administration of 1 $500mg/m^2$ xeloda (or tegafur 50~60 mg) in twice during d1~14 and intravenous drip of $135mg/m^2$ L-OHP on d1 for 2 h. The modified FOLFOX scheme: intravenous injection of $135mg/m^2$ L-OHP on d1 for 2 h, $200mg/m^2$ CF and 1.0 g tegafur during d1~5. Whereas, control Group B received chemotherapy regimens which were same as Group A, but no addition of endostar. Before chemotherapy, patients were given intravenous injection of 8 mg ondansetron, intramuscular injection of 10 mg metoclopramide and 20 mg diphenhydramine for prevention of vomiting, protection of liver and stomach as well as symptomatic supportive treatment. One cycle was 21 d, 4~6 cycles in total. The efficacy was evaluated every 2 cycles. Results: 32 patients in Group A could be evaluated, and the response rate (RR) and disease control rate (DCR) were 59.38% and 78.13%, respectively. 31 patients in Groups could be evaluated, and the RR and DCR were 32.26% and 54.84%, respectively. The differences between 2 groups were significant. The toxic effects include myelosuppression, gastrointestinal reaction, fatigue, cardiotoxicity and peripheral neurotoxicity. Conclusions: Preliminary observations show that endostar (once every other day) combined with chemotherapy is effective in the treatment of advanced gastrointestinal cancer, with low toxic effects, good tolerance, deserving further study.
