Cementless Hemiarthroplasty for Intracapsular Femoral Neck Fractures in the Octa- and Nonagenarians

Background: Current evidence supports the use of cemented hemiarthroplasty for treatment of intracapsular femoral neck fractures since it is associated with a lower risk of implant-related complications. However, many medical centers employ the cementless technique for the frail elderly population because it is faster and has lower cardiovascular risks and perioperative mortality. This observational study reports the outcomes of cementless bipolar hemiarthroplasty for intracapsular femoral neck fractures in patients aged 80 years and older. Methods: A total of 424 patients (female, 77.1%) with a mean age of 86.9 years were operated for intracapsular femoral neck fractures between January 2009 and December 2017. Of those, 66.7% had an American Society of Anaesthesiologists (ASA) score of 3 or more. All operations were performed with the posterolateral surgical approach and all patients received a cementless stem. Intraoperative and perioperative values and in-hospital outcomes were evaluated, and clinical and radiographical follow-up was done at 40 days, 90 days, and when possible between 5 months and 12 months postoperatively. Multivariate analysis was performed to evaluate if there were factors affecting mortality. Results: The mean operative time was 50 minutes. There were no deaths intraoperatively. Intraoperative periprosthetic fractures occurred in 2.1% of the cases with 66.7% of them fixed through cerclage wires intraoperatively. The median length of hospitalization was 11 days (interquartile range, 8.75–15) and 2.4% of patients died while in hospital after surgery. Approximately 91.5% of patients presented with perioperative anemia. Only 1.9% of the complications were related to the implant, 62.5% of which were dislocations. More than 90% of patients were ambulatory either autonomously or with support at each follow-up assessment. Age, male sex, and higher ASA score were related to increased mortality. Conclusions: Despite some limitations, this observational study underlines that a cementless femoral stem of modern design can give good clinical outcomes, thus being an appropriate solution especially for the frail elderly.

DNA methyltransferases (DNMTs) are an enzyme family that catalyzes the transfer of a methyl group to DNA for a wide variety of biological functions. To determine whether the number and composition of the Dnmt gene family affects the 5-methylcytosine (5mc) ratio at the genome level, genome-wide 5mc ratios from three marine animals, the mangrove killifish (Kryptolebias marmoratus), an intertidal copepod (Tigriopus japonicus), and a monogonont rotifer (Brachionus koreanus), were analyzed in each organism after the cloning of Dnmt genes. Lineage- and teleost-specific gene evolution was observed in the vertebrate Dnmt3 gene family, while unique gene expansion was found in the T. japonicus Dnmt1 gene family. However, the rotifer did not have any apparent homologue of Dnmt1 or Dnmt3 in its genome. This gene information was highly supportive of genome-wide 5mc levels in the three marine animals. Therefore, the absence or presence of the Dnmt gene family could be an important evolutionary parameter of how this could affect genomewide epigenetic metabolism.

Ultra performance liquid chromatography–mass spectrometry (UPLC–MS) method has been developed for the simultaneous determination of pharmaceuticals in industrial waste water samples. The binary mobile phase was used a mixture of acetonitrile and 0.1% aqueous solution of formic acid at a flow rate of 0.4 ml min1. Identification and quantitation of the analyzed drug samples were carried out using mass spectrometer equipped with electrospray ionization source operated in negative ionization mode. The linear range of the proposed method was found to be in the range of 18–270 ng ml1 for all drugs with limit of detection in the range of 1.25 to 1.89 ng ml1.

Objectives This study aimed to evaluate in vitro the effects of the self-adhesive resin cements RelyX U200 (3M ESPE) and seT PP (SDI Limited) on murine macrophages and the interference of the photoactivation. Materials and Methods Cell viability assays, cell adherence, yeast phagocytosis of Saccharomyces boulardii and production of reactive oxygen species (ROS) were performed in the presence of capillaries containing the respective self-adhesive cement when photoactivated or not. Results After long periods of contact, both types of cements, when not photoactivated, are more cytotoxic for macrophages. The seT PP cement when only chemically activated seems to interfere more negatively in the process of phagocytosis of yeasts S. boulardii. Both types of cements interfere in the cell adhesion process, independent of photoactivation. None of the types of cements tested was able to induce the production of ROS. Conclusions Our results highlight the great importance of the photoactivation of self-adhesive resin cements in the dental clinic, since RelyX U200, when photoactivated, presented the best results within the evaluated parameters.

The pathway of electron transport to nitrogenase in the endophytic β-Proteobacterium Herbaspirillum seropedicae has not been characterized. We have generated mutants in two nif-associated genes encoding putative ferredoxins, fdxA and fdxN. The fdxA gene is part of the operon nifHDKENXorf1orf2fdxAnifQmodABC and is transcribed from the nifH promoter, as revealed by lacZ gene fusion. The fdxN gene is probably cotranscribed with the nifB gene. Mutational analysis suggests that the FdxA protein is essential for maximum nitrogenase activity, since the nitrogenase activity of the fdxA mutant strain was reduced to about 30% of that of the wild-type strain. In addition, the fdxA mutation had no effect on the nitrogenase switch-off in response to ammonium. Nitrogenase activity of a mutant strain lacking the fdxN gene was completely abolished. This phenotype was reverted by complementation with fdxN expressed under lacZ promoter control. The results suggest that the products of both the fdxA and fdxN genes are probably involved in electron transfer during nitrogen fixation.
Lens culinaris: A New Biocatalyst for Reducing Carbonyl and Nitro Groups

A series of aliphatic and aromatic aldehydes and ketones, as well as some nitrocompounds were reduced using whole plant cells from Lens culinaris seeds. In addition, we also investigated the possibility of enzymatic ester hydrolysis to explore the potential of these seeds. The reduced ketones products were obtained in yields of 8 ~82% and enantiomeric excess of 39 ~ 75%. Aldehydes were more reactive than ketones with high chemical yield (95→99%), whereas the aromatic nitrocompounds showed low (2%) to high (> 99%) conversion depending upon the nature and position of the aromatic ring substituents. Ester hydrolysis by the Lens culinaris was quite effective with the ester p-nitrophenyl acetate (> 99% conversion).
