        수정체유화술에서 관류액에 의한 각막의 형태변화

        Purpose: To determine the effects of BSS Plus? and Optiplus? irrigating solution on the corneal endothelium in patients undergoing phacoemulsification with posterior chamber lens implantation. Methods: The study comprised 42 eyes irrigated with BSS Plus? and 37 eyes irrigated with Optiplus? solution. Cell density of the corneal endothelial cell, coefficient variation of the cell area, hexagonality, and central corneal thickness were measured. Results: There was no significant difference between the two solutions in their effect on corneal endothelial cell density, coefficient variation of the cell area, hexagonality, and central corneal thickness postoperatively after one day, one month, and two months (p>0.05). Conclusions: There were no differences in the effects of BSS Plus? and Optiplus? on the measured parameters of the corneal endothelium over the first two postoperative months.

        지대주의 반복적인 착탈에 따른 임플랜트 고정체의 external hexagon과 지대주 internal hexagon의 변화에 관한 연구

        Statement of problem: Repeated delivery and removal of abutment cause some changes such as wear, scratch or defect of hexagonal structure. It may increase the value of rotational freedom(RF) between hexagonal structures. Purpose: The purpose of this study was to evaluate surface changes and rotational freedom between the external hexagon of the implant fixture and internal hexagon of abutment after repeated delivery and removal under SEM and toolmaker's microscope. Materials and methods: Implant systems used for this study were 3i and Avana. Seven pail's of implant fixture, abutment and abutment screws for each system were selected and all fixtures were perpendicularly mounted in liquid unsaturated polyesther with dental surveyor. Each one was embedded beneath the platform of fixture. Surfaces of hexagonal structure before repeated closing and opening of abutment were observed using SEM and rotational freedom was measured by using toolmaker's microscope. Each abutment was secured to the implant future by each abutment screw with recommended torque value using a digital torque controller and was repeatedly delivered and removed by 20 times respectively. After experiment, evaluation for the change of hexagonal structures and measurement of rotational freedom were performed. Result : The results were as follows; 1. Wear of contact area between implant fixture and abutment was considerable in both 3i and Avana system. Scratches and defects were frequently observed at the line-angle of hexagonal structures of implant fixture and abutment. 2. In the SEM view of the external hexagon of implant fixture, the point-angle areas at the corner edge of hexagon were severely worn out in both systems. It was more notable in the case of 3i systems than in that of Avana systems. 3. In the SEM view of the internal hexagon of abutment, Gingi-Hue abutment of 3i systems showed severe wear in micro-stop contacts that were machined into the corners to prevent rotation and cemented abutment of Avana systems showed wear in both surface area adjacent to the corner mating with external hexagon of implant fixture. 4 The mean values of rotational freedom between the external hexagon of the implant fixture and internal hexagon of abutment were 0.48$\pm$0.04$^{\circ}$ in pre-tested 3i systems and 1.18$\pm$0.25$^{\circ}$ after test, and 1.80$\pm$0.04$^{\circ}$ in pre-tested Avana systems and 2.61$\pm$0.16$^{\circ}$ after test. 5. Changes of rotational freedom after test shouted statistical)y a significant increase in both 3i and Avana systems(P<0.05, paired t-test). 6. Statistically, there was no significant difference between amount of increase in the rotational freedom of 3i systems and amount of increase in that of Avana ones(P>0.05, unpaired t-test). Conclusion: Conclusively, it was considered that repeated delivery and remove of abutment by 20 times would not have influence on screw joint stability. However, it caused statistically the significant change of rotational freedom in tested systems. Therefore, it is suggested that repeated delivery and remove of abutment should be minimal as possible as it could be and be done carefully Additionally, it is suggested that the means or treatment to prevent the wear of mating components should be devised.

        Docking 술식을 이용한 야구선수의 내측측부인대 재건술에서 상완골 터널 시작위치에 따른 임상결과 비교

        The purpose of this study was to evaluate the humeral tunnel characters and clinical relevance according to entry point of the humeral tunnel in the baseball players. It was hypothesized that the medial collateral ligament (MCL) reconstruction with nonanatomical starting location of the humeral tunnel (inferior edge of the medial epicondyle: group NA) provided less favorable radiological and clinical outcomes compared to that with anatomical starting location (original footprint of the MCL: group A). The retrospective case review yielded 19 consecutive athletes who underwent isolated MCL reconstruction using the docking technique. Three dimensional-computed tomography scan was performed at 3 months, and the iso-surfacing by marching cubes algorithm were applied to evaluate the length and angle of humeral tunnel. Three outcome measures were used in this study: the visual analog scale for pain, range of motion and the Conway scale. The angle of the humeral tunnel was measured 12.2<sup>o</sup> (range, 7.9<sup>o</sup>-25.2<sup>o</sup>) in the group NA and 15.5o (range, 9.8<sup>o</sup>-30.4<sup>o</sup>) in the group A (p<0.05). The mean length of humeral tunnel is measured 16.3 mm (range, 11.7-20.1 mm) in the group NA and 15.2 mm (range, 10.3-19.1 mm) in the group A (p<0.05). MCL reconstruction brought substantial improvement in pain and function. However, between-group comparison revealed no statistical differences in all outcome measurements. The MCL reconstruction using the docking technique provided favorable clinical outcomes in baseball players. Although the humeral tunnel angle and length were different depending on the humeral entry points, clinical differences between the two entry points were not found.

        내부시장 지향성이 서비스 종업원의 개인-조직 적합성과 직무열의, 적응판매행동에 미치는 영향

        본 연구는 내부마케팅의 중요성을 강조하기 위해 기업의 내부시장 지향성이 서비스 종업원의 심리적 상태와 직무행동에 어떠한 영향을 미치는지 확인하고자 하였다. 구체적으로 본 연구에서는 내부마케팅을 반영하는 개념으로 내부시장 지향성을 제시하였으며, 내부적 정보생성 및 내부 커뮤니케이션, 내부시장에 대한 반응성의 세 가지 하위요인으로 구분하였다. 또한 내부시장 지향성은 직무 관련된 서비스 종업원의 인지적 심리상태인 개인-조직 적합성과 정서적 심리상태인 직무열의를 통해 서비스 종업원의 직무행동 측면인 적응판매행동을 증가시킬 것으로 예측하였다. 이러한 연구의 목적을 달성하기 위해 선행연구를 바탕으로 연구가설과 연구모델을 설정하고 가설검증을 실시하였다. 데이터는 소매금융 산업에서 금융서비스를 제공하는 서비스 종업원을 대상으로 수집되었다. 가설분석 결과 내부시장 지향성 중 내부적 정보생성은 서비스 종업원의 개인-조직 적합성에 영향을 미치지 못하는 반면, 내부 커뮤니케이션 및 내부시장에 대한 반응성은 개인-조직 적합성에 긍정적으로 작용하는 것을 확인하였다. 뿐만 아니라 서비스 종업원의 개인-조직 적합성은 직무열의에 긍정적으로 작용하였으며, 직무열의는 서비스 종업원의 적응판매행동을 증가시키는 것으로 확인되었다. In order to emphasize the importance of internal marketing, this study attempted to confirm how internal market orientation affects the psychological state and job behavior of service employees. Specifically, in this study, internal market orientation was presented as a concept that reflects internal marketing, and it was divided into three sub-factors: internal information generation, internal communications and responsiveness to the internal market. In addition, internal market orientation was predicted to increase adaptive selling behavior through service employees’ job-related psychological processes such as person-organizational fit and work engagement. To achieve the purpose of this study, research hypotheses and research models were established based on previous studies, and hypothesis testing was conducted. Data were collected from service employees who provide financial services in the retail banking industry. As a result of the hypothesis analysis, it was confirmed that internal information generation did not affect person-organizational fit of service employees, whereas internal communications and responsiveness to the internal market had a positive effect on person-organizational fit. In addition, it was confirmed that service employees’ person-organizational fit had a positive effect on work engagement, and work engagement increased service employees’ adaptive selling behavior.

        역행성 견관절 전치환술

        목적: 정형외과 의사로 하여금 역행성 견관절 전치환술의 생역학적 기능과 그 작용을 더 잘 이해 할 수 있게 함과 동시에, 최선의 수술적 치료를 시행할 수 있도록 돕는 데 있다. 대상 및 방법: 역행성 견관절 전치환술의 특정한 수술적 기법 및 주의점에 대해 심도있게 설명하고자 하였고, 견갑골 패임이나 능동적 외회전의 복원과 같이 역행성 견관절 전치환술과 관련된 최근의 주된 관심사에 대해서도 살펴보고자 하였다. 결과 및 결론: 수술 후 합병증을 증가시키지 않으면서 최선의 기능적 결과를 얻기 위해서는 역행성 견관절 전치환술의 개념, 그리고 그 수술 기법과 주의점에 대한 철저한 이해가 필수적이다. Purpose: The purpose of the present article is to help orthopedic surgeons better understand the function and performance of reverse total shoulder arthroplasty, and also to help them perform the most proper surgical technique for reconstruction. Materials and methods: In this article, the specific technical aspects and pitfalls of reverse total shoulder arthroplasty were reviewed in depth. Additionally, the current issues relevant to the reverse total shoulder arthroplasty such as scapular notching and restoration of active external rotation were discussed. Results and conclusion: An understanding of the biomechanics of reverse total shoulder arthroplasty and the technical details and pitfalls of its implantation are critical in order to provide the best functional outcome without increasing the risk of complications.
