        예방진료에 대한 일부 치과병원 의료진의 인식 및 요구도 파악을 위한 질적 연구

        Objectives: This study aims to understand the perceptions regarding preventive dental services among dental professionals. Methods: In-depth interviews were conducted with three groups of dental professionals―dentists, dental hygienists, and dental hygienists that specialized in preventive service―who were selected using a convenience sampling method. Results: First, there was limited awareness among dentists regarding the need for preventive care, because of the treatment-oriented philosophy and scarcity of preventive care success models. Second, dental hygienists commented that there was a lack of the person in charge and systematic process for preventive services. It would not help when they decided to change working place even though they had high anxiety working for preventive care. However, both these professionals agreed that preventive treatment is important as the core of care and that it was important to develop various improvement methods to promote preventive services. Conclusions: To activate preventive services in dental clinics, it is essential to improve the operating system and encourage dental professionals to realize the importance of preventive services.

        Among the various treatment methods for Halitosis, here I will discuss the chart-form diagnosis method. To do this, I will make specific observations on the types of halitosis that require treatment as well s the steps for each treatment. Then I will try to establish a treatment principle by applying an in-depth chart-form dignosis method. First of all, I will make observations on selected halitosis patients and discuss the guidelines on oral sanltataion. In order to do this, I will examine the fctors that could cause halitosis to a healthy person It will then be necessary for me to look into that person °Øs diet and lifestyle. Next, I will examine that person°Øs oral tension self-control method. I will then introduce halitosis treatments specific for persons wearing prosthesis such s dentures and for those who are underage. In addition, I will also tackle counseling techniques for patients. This will bean important factor in oral sanitation guidelines. Lastly, I will sum up the causes of halitosis as claimed by actual halitosis patients, the self-control method, and the wasys to alleviate symptoms of halitosis.

        Association between Obesity and Periodontitis in Korean

        Objective: Obesity is a major public health concern worldwide. Periodontitis is a major oral health problem and was reported to be associated with obesity. So, the present study aimed to examine the association between obesity and periodontitis in Korean. Methods: The study used data from 2010 to 2015 the Korea National Health and Nutrition Examination Survey. A dependent variable, periodontitis, was decided as community periodontal index 3. Obesity was defined as body mass index (BMI), and independent variables are socioeconomic variables (sex, age, residence, economic status, education levels, and occupation), health-related variables (drinking, smoking, and hypertension, diabetes, stress, physical activity), and oral health-related variables were collected. Multiple logistic regression analysis was used to evaluate the association between obesity and periodontitis. Results: In males, 26.5% were overweight and 39.1% were obese. In females, 22.8% were overweight and 30.7% were obese. The higher the BMI was, the lower socioeconomic status was. Periodontitis war higher among them with the lower economic level, the lower education, hypertension and diabetes, and stress. Moreover, periodontitis was significantly associated with obesity in multivariate logistic analyses. The odds ratio (OR) (95% confidence interval [CI]) was 1.28 (95% CI, 1.18-1.39) with adjusted for confounders. This OR increased in female (OR [95% CI], 1.39 [1.22-1.56]). Conclusion: This study demonstrates that obesity is closely associated with oral status, especially with periodontitis in Korean adults. This association was strong in female. Therefore, integrated intervention programs are required to prevent obesity and periodontits for Koreans on a national scale by sex.

        A Clinical Test of Dental Hypersensitivity by Use of the Dentifrice with the Desensitization Agents

        Objective: In order to examine the desensitization effect for the dentifrice with such ingredient as Zinc Chloride and Kalium Phosphate 1 and 2, by the experiment in vitro and in vivo. Methods: Ninety-nine adults volunteers who had a little hyper-sensitive dentin were divided with 3 groups as experimental, negative or positive control group, after in vitro experiment as closing ability for dentinal tubules by use of each dentifrice, and observed them with scanning electro microscope (SEM). Clinical test for desensitization effect was checked by use of electric pulp tester after using each dentifrice contained Zinc Chloride or Kalium Phosphate. Questionnaire method was added in order to check the subject’s response for satisfaction. Results: More closing effect on the dentinal tubules with the precipitation materials through coagulation of protein by were seen in experimental group than in negative or positive control group, observed with SEM in vitro study. The most in experimental group were satisfied to use the dentifrice with desensitization ingredients, examined by questionnaire method (p<0.05). It revealed more leveled in Electro-Pulp test in experimental group than in negative or positive control group for hypersensitivity check for 6 days use (p<0.05). Conclusion: Dentifrice with Zinc Chloride and Kalium Phosphate together was recommended to use for hypersensitivity dental patient.

        Study on Korean Adults' Subjective Oral Status & Dental Health Behavior according to Occupation

        Objective: The purpose of this study is to offer basic knowledge in developing more specific policies for dental health sector based on the analysis of subjective oral status and dental health behavior of Korean adults according to their occupation by referring to the Statistics Korea's Korea Standard Classification of Occupations (KSCO). Methods: Data on dental health behaviors of adults aged between 18 and 93 were extracted from the 2008 National Health and Nutrition Examination Survey (NHANES), and the survey examined a total of 7,280 people (3,096 males, 4,184 females). For statistical analysis, this thesis adopted SPSS (12.0) to run Perason’s chi-square test and Frequency analysis with the level of statistical significance being 0.05. Results: In the subjective oral status, the 54.4 elementary occupations, 53.1% skilled agricultural and fishery workers showed bad state (p<0.001). The craft, plant and machine operators and assemblers showed smoking as 48.7%, past smoking as 27.4% (p<0.001). About brushing before sleep, 79.1% skilled agricultural and fishery workers answered that they don’t do it (p<0.001). The skilled agricultural and fishery workers showed uncomfortable mastication as 52.3%, uncomfortable pronunciation as 21.2%, The clerks showed not uncomfortable mastication as 68.0%, not uncomfortable pronunciation as 92.3% (p<0.001). Conclusion: We need to explore how to make dental institutions more accessible by many, how to facilitate dental health education programs centering more on preventive measures than follow-up treatments and how to better organize dental care system suitable for each occupation.

        Comparison of Major Dental Disease between Workers and Korean Adults

        Objective: This study was conducted to be used as a fundamental material for developing the program and industrial dental health policy necessary to establish the constant dental health care system for workers. Methods: To efficiently maintain and improve dental health, this study conducted a comparison analysis of the dental diagnosis results of 3,125 workers and the National Oral Health Survey results of 3,353 Korean adults. The subjects aged 18 to 64 years were categorized into age groups, and then they were compared in terms of dental caries, missing teeth, gingivitis, calculus deposition, and oral treatment need. Results: With regard to the standardized prevalence rate of dental caries, workers accounted for 4.4% and Korea adults 1.7%. The prevalence rate of severe dental diseases of workers aged 35 to 54 years was the same highest as Korean adults. Regarding the priority of oral treatment need, in the case of workers, scaling ranked in the 1st place, caries treatment in the 2nd, and tooth brushing education in the 3rd; in the case of Korean adults, caries treatment ranked in the 1st place, scaling in the second, and extraction in the 3rd. Conclusion: Given the above results, it is necessary to plan and perform life-cycle based dental health projects in order to efficiently care for workers’ dental diseases and improve their dental health and to prevent dental caries and gingivitis by prioritizing industrial dental health policies.

        Dentinal Micro-hardness and Enamel Surface Change in Deciduous Teeth by Intake Some Candies Containing Vitamins

        Objective: The purpose of the study was to compare the effect on the decalcification of deciduous teeth for several candies containing vitamins as an ingredient. Methods: Twenty-two extracted deciduous teeth were collected and divided, with 8 enamel specimens and 80 dentine specimens. Three kinds of vitamin candies -V food, Vitamin 1000 and PungPung- were prepared for affected solutions, after measuring pH for each solution, Micro-hardness was measured for 80 of the dentinal specimens, and divided by the kinds of solutions and the applied frequency: ten, thirty, and sixty applications. SEM observation was done for 8 enamel specimens divided for the same method. Results: 1. PH was measured as 6.36 in water as control, 5.63 in V food, 2.62 in Vitamin C 1000 and 3.40 in PungPung-i. 2. Micro-hardness value was revealed as the highest in control group, followed by V food group, PungPung-I, and Vitamin C 1000 in sequence (p<0.05). 3. The results of observation by SEM (×3,000) show that smooth and fine surfaces were seen in control enamel surfaces and in the V food specimens. 4. Micro-hardness value was revealed that for more frequent application into the candy solution, less micro-hardness was observed (p<0.05). 5. The results of observation by SEM (×3,000) show that rough and decalcified phenomena were seen in the Vitamin C 1000 and PungPung-i specimens, and the roughness was more severe with increasing frequency of applying the candy solution. Conclusion: The above results show that V food is recommended for intake for children because of the safe level of pH at 5.6 and Vitamin C component, and intake of low pH Vitamin candy for long or frequent uses should be limited because of the risk for decalcification of deciduous teeth.

        Antiseptic Effect of Vitamedi on the Environment of the Dental Laboratory Room

        Objective: To examine the amounts of environmental surface microorganisms at the dental laboratory room. Methods: The author counted microorganisms using an ATP meter at 23 sites in the dental laboratory room to describe septic conditions for dental technicians and compare RLU before and after by use of Vitamedi solution spray with Flugun for 10 seconds at 1 m distance. Results: The average RLU on the laboratory furniture was decreased from 175.25±96.72 to 43.29±25.77 RLU, after 10 minutes' Vitamedi spray with Flugun (p<0.05). The average RLU on the laboratory equipment was decreased from 143.43±116.07 RLU to 24.57±17.56 RLU after using Vitamedi spray with Flugun (p<0.05). The average RLU on laboratory instruments was decreased from 66.13±64.33 RLU to 10.63±7.07 RLU, after using Vitamedi spray with Flugun (p<0.05). The most RLU was in the impression body at 905 RLU and 342 RLU after using Vitamedi spray. Conclusion: Use of Vitamedi with Flugun recommended in the dental laboratory room, to promote a septic environment.

        Relative Light Unit (RLU) Count at the Dental Environment by Use of Flugun™ and PCMX as a Low Temperature Spray Typed Sterilization Agent

        Objective: The author has measured RLU by use of ART meter at such site of dental environment as dental unit chair, dental light, tray, head rest and spittoon, in order to check the sanitary environment of dental clinic and to reveal the effect of PCMX spray with FlugunTM. Methods: 3% PCMX was used with FlugunTM as an experimental group and the mixed solution of 10% NaOCl and 70% Alcohol was sed as a control group, to compare the RLU before and after sterilization or each group, measuring RLU by use of ATP luminescence at each site and analysed by paired t-test. Results: 1. RLU was the most at the spittoon, before sterilization both in control and in experimental group. 2. RLU decrease rate was more in control group than in experimental group (p<0.05), measured at the spittoon, even though it revealed decrease of RLU after sterilization than before, both in control and in experimental group. 3. RLU decrease rates were more in experimental group than in control group, measured at such site as at the dental light, at the unit tray and at the head rest of the unit chair, but there was no significant different in each group (p>0.05). Conclusion: 3% PCMX with FlugunTM spray was recommended to use for promotion of total hygiene environment of dental clinic for a limited time and not for target area, by consideration.

        Marginal Micro-Leakage of the Sealant according to the Enamel Preparation

        Objective: The author has experimented on the marginal leakage of the pit and fissure sealant by enamel preparation, according to the preparation and the proper sealant in clinical use. Methods: 3 kinds of the sealant was chosen and 84 permanent extracted Premolar for orthodontic treatment were selected by deviding with 6 groups, as applying etchant or etching after preparation with a Tungsten carvide bur, for 3 kinds of the sealant group. Methylen blue was applied to infiltrate from the margine of sealant for each group. Stereo microscope was used to observe the micro-leakage after cutting longitudinally, and measured the depth penetration by use of Raadal M method. The obtained results were as follows. Results: 1. The average degree of the micro-leakage were less in enamel preparation with bur group than in enamel etching group (p<0.01), otherwise, no significant different both in Clinpro sealant and Seal it groups (p>0.05). 2. The comparison of the micro- leakage was shown as higher in Seal-it and Bosthworth group than in Clinpro group, in enamel etching group (p<0.05), otherwise, heigher in Bosthworth group than Seal-it or Clinpro group, in enamel etching after preparation with bur group, in statistically, by Duncan test (p<0.05). Conclusion: Unfilled type of sealant was recommended to use for sound enamel sealing and both in unfilled and nano typed filler contained sealant were recommended to use for sealing on the initical caries lesion, in clinical.
