        Purpose:The objective of this study was conducted to find out solution that physical therapist can get own clinic. Method:This is a literature study with books and reviewing the dismiss a suit of the court of constitutional law. Results:Physical therapist can have own clinic with legislature by the legislation and the Executive activities. Physical therapist is not only head of rehabilitation but all so controller. Physical therapist has different roll for the patient than medical doctor. Conclusion:Physical therapist was educated for the patient`s activities. Physical therapist can increase task performance of the patient and life of quality.

        탄력 밴드를 결합한 고유수용성신경근촉진법 상지 패턴이 편마비 환자의 신체정렬과 체중지지에 미치는 영향

        Purpose:The purpose of this study was to investigate how upper-extremity-movement of PNF(proprioceptive neuromuscular facilitation) with elastic band has effected on arrangement of body and weight-bearing. Methods:The object has worked with 20 patients with hemiplegia inmated and admitted a hospital in Pusan. At random they have divided into two groups - 10 experimental group who were applied typical movement therapy for stability of trunk, and 10 control group who were carried out upper-extremity-movement of PNF with elastic band. This study has carried out 30 minute a day in six weeks from 21 September 2009 to 31 October 2009. The object has used electric weighing scales to compare weight-bearing of two groups, and used full-spine X-ray to make comparison of arrangement of body. It has used Wilcoxson Signed-Rank Test Approach to analyse data of comparison how each groups was changed through this experiment and Kolmogorov-Smirnov Test has operated for verification of valence. Results:A comparison of the percentage weight-bearing, Cobb`s angle, pelvis height, and shoulder heights obtained before and after the 6-week exercise revealed statistically significant differences for the experimental group; however, there was no such difference in the case of the control group. It hasn`t appeared the statistically significant difference of valence in the comparison of each groups after this experiment. Conclusion:These results of this study is that upper-extremity-movement of PNF with elastic band improves the arrangement of body and weight-bearing of patients with hemiplegia. It will be a base to be used to objective data for more aid in clinical therapy.

        무릎외반의 균형 검사 및 보행 중에 근활성도와 발바닥압의 평가

        PURPOSE: This study sought to evaluate muscle activity and foot pressure during gait, and balance in female college students with genu valgum. METHOD: Participants were assigned based on their Q-angle to genu valgum group greater than 20° (GVG, n = 12), unilateral genu valgum group greater than 20° (UVG, n = 11), and control group (CON, n = 13). All subjects were evaluated for balance (Trace length, C90 area, C90 angle, and the Romberg test), muscle activity (gluteus medius; GM, tensor fasciae latae; TFL, vastus medialis; VM, vastus lateralis; VL, biceps femoris; BF, gastrocnemius; GCM and tibialis anterior; TA) and foot pressure (F/F ratio, R/F ratio, Hallux, 2~5 toe, 1st MT, 2~4 MT, 5th MT, Midfoot, M/heel, and L/heel) during gait. RESULTS: Romberg test showed significantly increased loss of balance in the UVG group compared with the CON. In the forward position, the imbalance was significantly increased in the UVG and GVG groups compared to the CON. Muscle activity of VL, GCM, and TA significantly increased in the GVG group compared with the CON. Static foot pressure, 1st MT significantly increased in the GVG compared to the CON group. The 5th MT significantly decreased in the CON compared with the GVG group. The R/F ratio significantly decreased in the GVG compared to the CON group. In dynamic foot pressure, the 2∼5 toe significantly increased in the GVG compared with the UVG group. The left 5th MT significantly decreased in the UVG compared with the CON and GVG groups. CONCLUSION: These results indicate that genu valgum has a negative effect on balance, muscle activity, and foot pressure during gait in female college students.

        젊은 만성허리통증자의 허리부위 불안정성 검사 양성반응 수와 호흡패턴변화율, 통증 간의 상관관계분석

        PURPOSE: This study examined the correlation between the visual analog scale (VAS) and the rate of change in the respiration patterns according to the result of Lumbar Instability Tests (LITs) in young people with chronic low back pain (CLBP) METHODS: Thirty-six adults, aged 20-40 years with CLBP, participated in this study. The general characteristics and VAS of the participants were recorded by the subjects themselves and seven structure and functional LITs were conducted. According to the positive response number, the positive group was divided into four groups (group 1: n=8, group 2: n=9, group 3: n=10, group 4: n=9). The breathing pattern change tests were performed in three states: during forced breathing exercise and motor control tests. A total of 13 positive lists were set, each of which was scored by 1 or 2 points according to the severity. After the positive lists were scored, the breathing pattern changing rate (BPCR) and VAS were compared according to the positive response number of LITs, and the correlation between them was analyzed. RESULTS: A strong correlation was observed between the number of positive of LITs and BPCR (r= .863, p= .000) and a moderate correlation between the positive number of LITs and VAS (r= .508, p= .002). In addition, there was a poor correlation between the BPCR and VAS (r= .434, p= .008). CONCLUSION: In young CLBP people, when the structural and functional instability are both present, the changes in the respiratory pattern of the whole body can be varied and broader, and the pain scale also increases.

        The Relationship between Sensory Processing Abilities and Gross and Fine Motor Capabilities of Children with Cerebral Palsy

        PURPOSE: The purpose of this study was to investigate the difference and relationship between sensory processing abilities, gross motor and fine motor capabilities in children with cerebral palsy. METHODS: 104 children with cerebral palsy participated in the study. Sensory processing abilities of the subjects were measured by Short Sensory Profile (SSP). Gross and fine motor abilities were each measured using the Gross Motor Function Classification System (GMFCS) and Manual Ability Classification System (MACS), respectively. RESULTS: There were significant correlations between SSP level and GMFCS (R=.72, p<.00) or MACS (R=.77, p<.00) levels. Significant differences were showed each gross motor (p=.01) and fine motor level (p=.00) among sensory processing level of children. In addition, sub-items of sensory processing as Tactile sensitivity, Movement sensitivity, Auditory filtering and Low energy/Weak were significantly were showed significant correlations gross motor and fine motor level (p=.01). Also, multiple regression result was showed that as MACS level and GMFCS level were higher, the SSP total score was higher all of participants (adjusted R<sup>2</sup>=.62). CONCLUSION: Sensory processing abilities of children with cerebral palsy were related with gross motor and fine motor capabilities. Also gross motor and fine motor capabilities are as higher, the sensory processing skill was well of cerebral palsy.

        치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고

        PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

        크런치 운동 시 부수적인 기합 기법과 청각적 격려 적용이 정상인의 복부 근육 활성도에 미치는 영향 : 무작위 대조 예비 연구

        PURPOSE: The purpose of this study was to investigate the effects of a supplementary Ki-hap technique and verbal encouragement on the activation of abdominal muscles during performance of crunch exercise in healthy subjects. METHODS: 93 subjects were randomly allocated to three group, crunch exercise group (CG), crunch exercise with Ki-hap group (CKG), and crunch exercise with Ki-hap and verbal encouragement group (CKVG), with 31 subjects in each group, respectively. The interventions were conducted over three trials in each group, and measurements were performed on each subject by one examiner in three trials. The activation of rectus abdominis (RA), external oblique (EO), and internal oblique (IO) muscles were evaluated using electromyography (EMG) during performance of crunch exercise with non-Ki-hap, Ki-hap and Ki-hap combined with verbal encouragement. RESULTS: Our results showed a significantly greater increase in the EMG patterns of all muscles during performance of crunch exercise in the CKG (p<0.05) compared to the CG and CKVG. The results also showed that there were significantly greater increase in the activation of EO and IO muscles in those of the CKVG (p<0.05) compared with the CG. CONCLUSION: These findings demonstrated that addition of the Ki-hap technique and verbal encouragement during performance of crunch exercise, at the same time, would suggest positive evidence for improving activation of abdominal muscles.

        요통유무에 따른 복부 드로우인과 골반바닥근 수축훈련이 배가로근과 골반바닥근에 미치는 영향

        PURPOSE: This study aimed to identify differences in the thickness of the transverse abdominis (TrA) and pelvic floor muscles (PFM) between those with (LBP) and without low back pain (non-LBP). The standardized methods of contraction for the TrA and PFM were used to perform the abdominal draw-in maneuver (ADIM) and pelvic floor muscle contraction (PFC), respectively. METHODS: This study included 27 young men and women, who were verbally instructed regarding the maneuvers (LBP, n=14; non-LBP, n=13). For all subjects, TrA and PFM thickness were evaluated by ultrasonography during ADIM and PFC. RESULTS: The results of this study showed that TrA thickness increased during ADIM and PFC to a greater degree in the non-LBP group than in the LBP group (p < 0.01). PFM thickness increased more during PFC in the non-LBP group than in the LBP group (p < 0.05). Both groups showed greater increases in TrA thickness during ADIM than during PFC (p < 0.01), and greater increases in PFM thickness during PFC than during ADIM (p < 0.05). CONCLUSION: These results suggest that receiving verbal instructions on how to use each muscle for a specific maneuver was more beneficial than other verbal instruction. Further studies are needed determine how our results may be applied beneficially in research on this topic.

        15분간의 저측굴곡근 정적 스트레칭이 기립 균형에 미치는 단기 효과

        Purpose The purpose of this study was to examine immediate effects of 15 minutes plantarflexor static stretching in quite stance. Methods Twenty-nine subjects were measured static balance during 1 minute in quite stance with eye closed condition before and after 15 minutes plantarflexor static stretching. Static stretching range was limited from 15° to 20° dorsiflexion within comfortable range. Results The result of this study showed that postural sway significantly increased during 1 minute quiet stance after 15 minutes static stretching(p<.05). Before stretching, postural sway significantly decreased during 41-60 seconds compared to 0-20, 21-40 seconds(p<.05), After stretching, postural sway was decreased significantly over time 0-20, 21-40, and 41-60 seconds (p<.05), Conclusion The prolong plantarflexor static stretching may require biomechanical, neurological adaptations prior to walking or sport activities for safety.

        발목관절의 능동적 배측굴곡 시 정상과 망치 발가락 대상자의 전경골근과 장지신근의 근 활성도 비의 비교

        Purpose:The purposes of this study were to compare the muscle activity ratio of tibialis anterior (TA) / extensor digitorum longus (EDL) during the active ankle dorsiflexion in subjects with the normal toe (NT) and the hammer toe (HT). Methods:Nineteen subjects with the NT group and nineteen subjects with the HT group were recruited for this study. The muscle activities of TA and EDL were measured by using surface electromyography (EMG) and the angles of ankle dorsiflexion and eversion of the subtalar joint were measured by using 3-dementional motion analysis during the active ankle dorsiflexion in prone position. Results:The muscle activity ratio of TA / EDL was significantly lower in the HT group compared to the NT group (p<.05). The angle of ankle dorsiflexion was significantly lower in the HT group compared to the NT group (p<.05). Conclusions:These results suggest that muscle imbalance between TA and EDL muscle and decreased ankle dorsiflexion range of motion may contribute to hammer toe deformity. Further studies are needed to confirm that the correcting of this imbalance and the increasing ankle dorsiflexion could improve toe alignment in the subjects with HT.
