        Venous Outflow Profiles Are Linked to Clinical Outcomes in Ischemic Stroke Patients with Extensive Baseline Infarct

        Background and Purpose The benefit of endovascular thrombectomy (EVT) treatment is still unclear in stroke patients presenting with extensive baseline infarct. The use of additional imaging biomarkers could improve clinical outcome prediction and individualized EVT selection in this vulnerable cohort. We hypothesized that cerebral venous outflow (VO) may be associated with functional outcomes in patients with low Alberta Stroke Program Early CT Score (ASPECTS). Methods We conducted a retrospective multicenter cohort study of patients with acute ischemic stroke due to large vessel occlusion (AIS-LVO). Extensive baseline infarct was defined by an ASPECTS of ≤5 on admission computed tomography (CT). VO profiles were assessed on admission CT angiography using the Cortical Vein Opacification Score (COVES). Favorable VO was defined as COVES ≥3. Multivariable logistic regression was used to determine the association between cerebral VO and good clinical outcomes (90-day modified Rankin Scale score of ≤3). Results A total of 98 patients met the inclusion criteria. Patients with extensive baseline infarct and favorable VO achieved significantly more often good clinical outcomes compared to patients with unfavorable VO (45.5% vs. 10.5%, P<0.001). Higher COVES were strongly associated with good clinical outcomes (odds ratio, 2.17; 95% confidence interval, 1.15 to 4.57; P=0.024), independent of ASPECTS, National Institutes of Health Stroke Scale, and success of EVT. Conclusions Cerebral VO profiles are associated with good clinical outcomes in AIS-LVO patients with extensive baseline infarct. VO profiles could serve as a useful additional imaging biomarker for treatment selection and outcome prediction in low ASPECTS patients.

      • Design of Chemical Industrial Park Integrated Information Management Platform Based on Cloud Computing and IOT (The Internet of Things) Technologies

        Chemical industry is the fundamental strategic industry in national economy and one of pillar industries in China. A wealth of chemical industrial parks has been built one after another in many places, where lots of chemical enterprises are established. In the normal operation of those enterprises, their raw materials or products manufactured, transported and stored are usually featured by flammable, explosive or toxic properties, resulting in frequent occurrence of pollution and personal accidents and causing great threats to surrounding environment. This thesis realizes an integrated information management platform based on currently advanced cloud computing and IOT (The Internet of Things) technologies, so as for integrated management of chemical industrial parks. This thesis designs the architectural structure of said integrated information management platform, expounds the roles and realization approaches of the basic support platform and the business service platform, analyzes the principles and methods for realization of regionally coordinate emergency management operation mechanism based on the information management platform, that of IOT architecture technology and that of RFID and Wireless Sensor Network (WSN), and discusses that the standard unification of heterogeneous data under cloud computing is realized by means of multi-source data integration middleware. The said platform can accomplish the whole-process, real-time and dynamic follow-up and control of chemicals and hazardous materials and also provide effective service for users’ decision-making by mining and analyzing tremendous data of a chemical industrial park with Agent-middleware technology. The information management platform is capable of assessing the safety factors of chemical enterprises at peacetime, giving necessary warning before accidents and offering effective technical support for rescue via expert system, emergency plan system, monitoring system and positioning system. The realization of chemical industrial park integrated information management platform based on cloud computing and IOT technologies will greatly improve the management level of chemical industrial parks, intensify surveillance, achieve information resource sharing and lower the probability of accident occurrence.

        Local Immunoglobulin E in the Nasal Mucosa: Clinical Implications

        Immunoglobulin E (IgE) can be highly elevated in the airway mucosa independently of IgE serum levels and atopic status. Mostly, systemic markers are assessed to investigate inflammation in airway disease for research or clinical practice. A more accurate but more cumbersome approach to determine inflammation at the target organ would be to evaluate markers locally. We review evidence for local production of IgE in allergic rhinitis (AR) and chronic rhinosinusitis with nasal polyps (CRSwNP). Diagnostic and therapeutic consequences in clinical practice are discussed. We describe that the airway mucosa has the intrinsic capability to produce IgE. Moreover, not only do IgE-positive B cells reside within the mucosa, but all tools are present locally for affinity maturation by somatic hypermutation (SHM), clonal expansion, and class switch recombination to IgE. Recognizing local IgE in the absence of systemic IgE has diagnostic and therapeutic consequences. Therefore, we emphasize the importance of local IgE in patients with a history of AR or CRSwNP.

      • Immiscible sulfide melts in primitive oceanic magmas: Evidence and implications from picrite lavas (Eastern Kamchatka, Russia)

        <P><B>Abstract</B></P><P>Silicate-sulfide liquid immiscibility in mantle-derived magmas has important control on the budget of siderophile and chalcophile metals, and is considered to be instrumental in the origin orthomagmatic sulfide deposits. Data on primitive sulfide melts in natural samples, even those representing most voluminous magmatism in oceanic rifts, are very scarce due to the small size and poor preservation of incipient sulfide melt globules. Here we present the first detailed report of the crystallized sulfides melts in the oceanic picrites of the (presumably) Cretaceous age Kamchatsky Mys ophiolite complex in Eastern Kamchatka (Far East Russia). Sulfide melts are present in three forms; (1) as inclusions in olivine (87.1-89.6 mol% Fo), (2) interstitial to the groundmass minerals (clinopyroxene, plagioclase, and Ti-magnetite) of studied picrites, and (3) as daughter phases in silicate melt inclusions hosted by olivine and Cr-spinel phenocrysts. The sulfide melt inclusions in olivine and the groundmass of studied rocks are composed of several sulfide phases that correspond to the monosulfide (Fe-Ni;<I>Mss</I>) and intermediate (Fe-Cu-Ni;<I>Iss</I>) solid solutions. Several <0.5 μm Pd-Sn, Pt-Ag, and Au-Ag phases are recorded within the matrix sulfides, commonly along phase boundaries and fractures. Major elements (S, Fe, Cu, Ni, Co), platinum group elements (PGE), and gold analyzed in the homogenized olivine-hosted sulfide melt inclusions, and phases identified in the matrix sulfides record the range of magmatic sulfide compositions. The most primitive sulfide liquids are notably enriched in Ni and Cu [(Ni+Cu)/Fe, at% > 0.5], continuously evolve with crystallization of (e.g., increasing Cu/Ni and Au/PGE) and demonstrate metal fractionation between<I>Mss</I>and<I>Iss</I>. Although the compositional systematics found in this study are consistent with those previously recorded, the compositions of individual sulfide phases are strongly affected by the noble metal (PGE, Au) “nuggets” that exsolve at subsolidus temperatures and form during serpentinization of the rocks. We conclude that the budget of noble metals in the studied picrites is controlled by sulfides, but the abundances of Pt and Au are influenced by mobility in post-magmatic alteration. Our data can be also used for modeling sulfide saturation at crustal pressures and understanding behavior of the noble metals in primitive oceanic magmas.</P>
