Accurate Determination of Childhood Food Allergy Prevalence and Correction of Unnecessary Avoidance

Purpose: Because the true prevalence of food allergy (FA), as based on the results of an oral food challenge test (OFC), is unknown, it is likely that children with suspected FA unnecessarily eliminate potentially causative foods. This study aimed to identify the prevalence of FA and to determine the proportion of children who unnecessarily eliminate food. Methods: To identify children with FA, a primary survey was conducted via a questionnaire with all children aged 0-18 years in Niijima village (remote islands of Japan). In the secondary survey, a detailed medical interview was conducted by doctors with children who currently did not eat some foods. The third survey involved serum food-specific immunoglobulin E (IgE) tests and an OFC for children with suspected FA. Results: Of 376 enrolled children, 374 (99.5%) completed the questionnaire. Some foods were eliminated by 18.6% and 13.0% of all children and those ≥6 years old, respectively. The target population for the secondary survey included 69 children who all completed the medical interview. The target population for the third survey consisted of 35 children, of whom 26 (74.3%) underwent the blood test. An OFC was performed 35 times with 20 children. As a result, the prevalence of FA was 4.9% in children of all ages and 4.7% in those ≥6 years old. Moreover, 55.0% children were able to cease eliminating food intake. Conclusions: It is possible that a considerable number of children unnecessarily eliminate food because of suspected FA.
Downregulation of bcl - xL Is Relevant to UV - induced Apoptosis in Fibroblasts

Exposure to ultraviolet light (UV) induces apoptosis in mammalian cells. The caspase group of proteases is required for the apoptosis. This pathway is initiated by a release of cytochrome c from the mitochondria into the cytosol. Several Bcl-2 family proteins can regulate the release of cytochrome c by stabilizing the mitochondrial membrane. Here we show that expression of the endogenous bcl-xL was strongly downregulated in NIH3T3 cells within 2 h after UV-C irradiation, and that of bax was upregulated from 8 h after irradiation. Apoptosis was induced in more than 50% of the NIH3T3 cells 48 h after irradiation. Constitutive overexpression of bcl-xL in NIH3T3 cells protected the UV-induced apoptosis by preventing the loss of mitochondrial membrane potential and the activation of caspase 9. These results suggest that downregulation of Bcl-xL is relevant to UV-induced apoptosis of fibroblasts.
Influence of Cognitive and Functional Impairment on Depression among Institutional Older Residents

The purpose of the study was to examine the relationships between cognitive and functional impairment and depression in terms of Lazarus and Folkmans theoretical perspective. We analyzed the responses of 208 older adults residing in the special elderly nursing homes and/or the health care facilities for the elderly. The results of our statistical analyses was as follows: (1) The exploratory factor analyses for two stress-appraisal scales of cognitive and functional impairment provided one-factor solution, respectively. The confirmatory factor analyses indicated the each one-factor measurement model yielded adequate indices of fit. It is evidence for their construct validity and unidimensionality. (2) Our path analytic model using structural equation modeling demonstrated that indirect effects of the cognitive and functional impairment on the depression through an increase in the stress-appraisals of their impairment. Significant direct effects of both cognitive and functional impairment on the depression were not found. These results indicate that stress-appraisals of cognitive and functional impairment play an important role in mediating the relationship between cognitive and functional impairment as a stressor, and depression as a stress response. Therefore, it is necessary to concern stress-appraisals of cognitive and functional impairment in designing and establishing an effective mental health care system for institutional older residents.

Background/Aims: This study aimed to examine the diagnostic ability of endoscopic ultrasonography (EUS) for major vascularinvasion in pancreatic cancer and to evaluate the relationship between EUS findings and pathological distance. Methods: In total, 57 consecutive patients who underwent EUS for pancreatic cancer before surgery were retrospectively reviewed. EUSimage findings were divided into four types according to the relationship between the tumor and major vessel (types 1 and 2: invasion,types 3 and 4: non-invasion). We also compared the EUS findings and pathologically measured distances between the tumors andevaluated vessels. Results: The sensitivity, specificity, and accuracy of EUS diagnosis for vascular invasion were 89%, 92%, and 91%, respectively, in theveins and 83%, 94%, and 93%, respectively, in the arteries. The pathologically evaluated distances of cases with type 2 EUS findings weresignificantly shorter than those of cases with type 3 EUS findings in both the major veins (median [interquartile range], 96 [0–742] µmvs. 2,833 [1,076–5,694] µm, p=0.012) and arteries (623 [0–854] µm vs. 3,097 [1,396–6,000] µm, p=0.0061). All cases with a distance of≥1,000 µm between the tumors and main vessels were correctly diagnosed. Conclusions: Tumors at a distance ≥1,000 µm from the main vessels were correctly diagnosed by EUS.

Background/Aims: This study aimed to identify the predictive factors for inaccurate endoscopic ultrasound-guided fine needleaspiration (EUS-FNA) diagnosis of swollen lymph nodes without rapid on-site cytopathological evaluation. Methods: Eighty-three consecutive patients who underwent EUS-FNA for abdominal or mediastinal lymph nodes from January2008 to June 2017 were included from a prospectively maintained EUS-FNA database and retrospectively reviewed. The sensitivity,specificity, and accuracy of EUS-FNA for the detection of neoplastic diseases were calculated. Candidate factors for inaccurate diagnosis(lymph node size and location, needle type, puncture route, number of passes, and causative disease) were evaluated by comparisonbetween accurately diagnosed cases and others. Results: The final diagnosis of the punctured lymph node was classified as neoplastic (65 cases: a metastatic lymph node, malignantlymphoma, or Crow-Fukase syndrome) or non-neoplastic (18 cases: a reactive node or amyloidosis). The sensitivity, specificity, andaccuracy were 83%, 94%, and 86%, respectively. On multivariate analyses, small size of the lymph node was the sole predictive factor forinaccurate EUS-FNA diagnosis with a significant difference (odds ratios, 19.8; 95% confidence intervals, 3.15–124; p=0.0015). Conclusions: The lymph node size of <16 mm was the only independent factor associated with inaccurate EUS-FNA diagnosis ofswollen lymph nodes.

The purpose of this study was to develop an IADL scale for elderly residing in health and welfare institutions with an exploratory, confirmatory and simultaneous factor analyses. Subjects were 7,251 residents of 111 special elderly nursing homes and 5,087 residents of 5 health care facilities for the elderly in Shizuoka prefecture. First, the exploratory factor analysis yield four factors: Feeding-related activities, Cleaning-related activities, Intellectual-living-related activities, and Mobility-related activities, respectively. Second, the confirmatory factor analysis indicated that the model comprised of 13 items was fit to our data. Third, the simultaneous factor analysis with six groups divided by sex(male, female) and age brackets(younger-old = 65-74, older-old = 75-84, oldest-old = 85 or more) showed that the model was fit to our data in each group. These results supported validity of the IADL scale for institutional elderly. These findings are expected to provide clinically useful implications particularly in evaluating outcomes of rehabilitation and in designing care-plan adequate to the functional level of institutional elderly.

This paper proposes a new compensation method for estimating temperature using a voltage disturbance observer. In conventinal resistance estimation method is weekness to other parameter variations in SPMSM, because it is for estimating potision control system. In addition, offline compensate method is hard, because rotor flux and inductance are changing depending on temperature. Hence, we proposed a method to estimate the temperature based on the motor model without the influence of back erectromotive force. However, problems such as voltage disturbances arise, because the system didnot consider the inductance variation depending on temperature. This paper proposes a new method to compensate these disturbances and confirm its effectiveness by experiments.

Understanding how outpatient physiotherapy impacts on specific motor symptoms in Parkinson's disease (PD) is important for multidisciplinary care, but these points have not been clarified. We investigated the impact of outpatient physiotherapy on individual motor symptoms in PD patients. Fifty-five PD patients participated in the prospective cohort study, which examined the changes in motor symptoms after 90 min of outpatient physiotherapy program (1×/week for 10 weeks) and at 3 months follow-up. Movement Disorder Society-Unified Parkinson's Disease Rating Scale (MDS-UPDRS) motor score and tremor, rigidity, bradykinesia, and axial scores were assessed and compared pre-intervention, post-intervention, and at follow-up. Significant level was set at 0.05. Their MDS-UPDRS motor score and axial score significantly decreased post-intervention and at the follow-up. In the analysis differentiating effects based on the severity of motor symptoms according to the MDS-UPDRS motor score, only the moderate-severe group showed significant decreases in their MDS-UPDRS motor score, bradykinesia, and axial scores post-intervention, as well as in their MDS-UPDRS motor score, rigidity, bradykinesia, and axial scores at the follow-up. These findings suggest the outpatient physiotherapy might provide benefits, particularly in managing axial symptoms and bradykinesia, for community dwelling PD patients with moderate-severe motor symptoms within a multidisciplinary care framework.
