        Meibomian Glands and Ocular Surface Changes After Closure of Meibomian Gland Orifices in Rabbits

        <P>Conclusions: Meibomian gland orifice closure reduced corneal wetting property and induced meibomian gland duct dilation accompanied by shrinkage of secretory acini. Meibography could not detect early changes in the meibomian gland after closure of the orifice. Therefore, when the orifice is obstructed, more active treatments are needed before structural changes occur.</P>

        <P>Conclusions: Patients with chemotherapy-induced LD obstruction had greater meibomian gland loss and thinner lipid layers than did patients without LD obstruction. Chemotherapeutic agents that induce LD obstruction can also block the meibomian gland orifice, resulting in obstructive meibomian gland dysfunction through the same mechanism.</P>

        Comparison of Meibomian Gland Loss and Expressed Meibum Grade Between the Upper and Lower Eyelids in Patients With Obstructive Meibomian Gland Dysfunction

        PURPOSE:: The aim of this study was to compare meibomian gland loss (MGL) and expressed meibum grade between upper and lower eyelids in patients with obstructive meibomian gland dysfunction (MGD) and to evaluate the correlation between these 2 parameters and other clinical measurements. METHODS:: Twenty-six eyes of 26 patients with obstructive MGD were enrolled. Upper and lower MGLs were evaluated using noncontact meibography. Expressed meibum quality was assessed in 8 glands of the central third area of the upper and lower eyelids on a scale of 0 to 3 for each gland (total score range, 0–24). Tear film stability was evaluated based on tear break-up time (TBUT), and corneal staining was graded according to the National Eye Institute scale (range, 0–15). RESULTS:: The mean MGL in the lower eyelids (24.1% ± 10.8%) was significantly greater than that of the upper eyelids (11.2% ± 5.2%) (P < 0.001). The mean expressed meibum grade in the lower eyelids (16.5 ± 5.1) was also significantly larger than that of the upper eyelids (11.2 ± 5.2) (P < 0.001). MGL was significantly correlated with expressed meibum grade in both eyelids (r = 0.451, P = 0.021 in the upper eyelids; r = 0.626, P = 0.001 in the lower eyelids). The meibum grades of both the upper and lower eyelids were negatively correlated with TBUT and positively correlated with corneal staining score. However, the MGL in both the eyelids was not correlated with TBUT or with corneal staining score. CONCLUSIONS:: In patients with obstructive MGD, MGL and meibum grade in the lower eyelids were significantly greater than those of the upper eyelids. Although MGL and meibum quality showed a positive correlation with each other, TBUT and corneal staining score were significantly correlated with only meibum grade, and not with MGL.

        Clinical Outcomes of Intrascleral Fixation of Intraocular Lens Compared to Ciliary Sulcus Implantation and Transscleral Fixation

        Purpose: To compare the clinical outcomes of intrascleral fixation of the three-piece intraocular lenses (IOLs) 2.5 mm posteriorto the limbus with ciliary sulcus implantation and transscleral fixation 2.5 mm posterior to the limbus. Methods: Sixty-five eyes of 65 patients who underwent ciliary sulcus implantation or transscleral or intrascleral fixation ofthe AMO Sensar AR40e IOL were retrospectively reviewed. The postoperative refractive prediction error, back-calculatedeffective lens position (ELP), corrected distance visual acuity (CDVA), and postoperative residual cylinder were compared. Results: There were significant differences in the median (interquartile range) postoperative refractive prediction error (diopters[D]) among the three groups (p < 0.001): for ciliary sulcus implantation (33 eyes), −0.89 D (−1.21 to −0.56 D); for transscleralfixation (10 eyes), −0.40 D (−0.78 to −0.22 D); and for intrascleral fixation (22 eyes), 0.01 D (−0.28 to 0.34 D). Significantdifferences (p < 0.001) were observed in the median back-calculated ELP: for ciliary sulcus implantation, 4.35 mm (3.95 to4.55 mm); for transscleral fixation, 4.51 mm (4.34 to 4.76 mm); and for intrascleral fixation, 4.90 mm (4.56 to 5.35 mm). Therewere no differences in the median postoperative CDVA (0, 0.10, and 0 logarithm of the minimum angle of resolution, respectively;p = 0.083) and the residual cylinder (−0.75, −1.50, and −0.63 D, respectively; p = 0.074) among three groups. Conclusions: Intrascleral fixation showed no myopic shift and the most posterior lens position, while ciliary sulcus implantationinduced the greatest myopic shift and the most anterior lens position. However, there was no significant difference in thepostoperative CDVA or astigmatism among the eyes with different IOL insertion methods, demonstrating good IOL stabilityand vision outcomes.

        Effect of Cataract Grade according to Wide-Field Fundus Images on Measurement of Macular Thickness in Cataract Patients

        Purpose: To investigate the effects of cataract grade based on wide-field fundus imaging on macular thickness measured by spectral domain optical coherence tomography (SD-OCT) and its signal-to-noise ratio (SNR). Methods: Two hundred cataract patients (200 eyes) with preoperative measurements by wide-field fundus imaging and macular SD-OCT were enrolled. Cataract severity was graded from 1 to 4 according to the degree of macular obscuring by cataract artifact in fundus photo images. Cataract grade based on wide-field fundus image, the Lens Opacity Classification System III, macular thickness, and SD-OCT SNR were compared. All SD-OCT B-scan images were evaluated to detect errors in retinal layer segmentation. Results: Cataract grade based on wide-field fundus imaging was positively correlated with grade of posterior subcapsular cataracts (rho = 0.486, p < 0.001), but not with nuclear opalescence or cortical cataract using the Lens Opacity Classification System III. Cataract grade was negatively correlated with total macular thickness (rho = -0.509, p < 0.001) and SD-OCT SNR (rho = -0.568, p < 0.001). SD-OCT SNR was positively correlated with total macular thickness (rho = 0.571, p < 0.001). Of 200 eyes, 97 (48.5%) had segmentation errors on SD-OCT. As cataract grade increased and SD-OCT SNR decreased, the percentage of eyes with segmentation errors on SD-OCT increased. All measurements of macular thickness in eyes without segmentation errors were significantly greater than those of eyes with segmentation errors. Conclusions: Posterior subcapsular cataracts had profound effects on cataract grade based on wide-field fundus imaging. As cataract grade based on wide-field fundus image increased, macular thickness tended to be underestimated due to segmentation errors in SD-OCT images. Segmentation errors in SD-OCT should be considered when evaluating macular thickness in eyes with cataracts.

        Comparison of Anterior, Posterior, and Total Corneal Astigmatism Measured Using a Single Scheimpflug Camera in Healthy and Keratoconus Eyes

        Purpose: To compare the effect of posterior corneal astigmatism on the estimation of total corneal astigmatism using anterior corneal measurements (simulated keratometry [K]) between eyes with keratoconus and healthy eyes. Methods: Thirty-three eyes of 33 patients with keratoconus of grade I or II and 33 eyes of 33 age- and sexmatched healthy control subjects were enrolled. Anterior, posterior, and total corneal cylinder powers and flat meridians measured by a single Scheimpflug camera were analyzed. The difference in corneal astigmatism between the simulated K and total cornea was evaluated. Results: The mean anterior, posterior, and total corneal cylinder powers of the keratoconus group (4.37 ± 1.73, 0.95 ± 0.39, and 4.36 ± 1.74 cylinder diopters [CD], respectively) were significantly greater than those of the control group (1.10 ± 0.68, 0.39 ± 0.18, and 0.97 ± 0.63 CD, respectively). The cylinder power difference between the simulated K and total cornea was positively correlated with the posterior corneal cylinder power and negatively correlated with the absolute flat meridian difference between the simulated K and total cornea in both groups. The mean magnitude of the vector difference between the astigmatism of the simulated K and total cornea of the keratoconus group (0.67 ± 0.67 CD) was significantly larger than that of the control group (0.28 ± 0.12 CD). Conclusions: Eyes with keratoconus had greater estimation errors of total corneal astigmatism based on anterior corneal measurement than did healthy eyes. Posterior corneal surface measurement should be more emphasized to determine the total corneal astigmatism in eyes with keratoconus.

        안과 전공의 수련과정 중 백내장 수술에 대한 설문조사

        Purpose: To investigate cataract surgical experiences during ophthalmology residency in Korea. Methods: A survey regarding cataract surgical experiences during residency was sent to 4th year ophthalmologic residents (112) from April to September 2010. Results: Of the 112 individuals to whom the surveys were sent, 35 (31.3%) responded. The majority of the respondents (65.7%) performed their first cataract operation as the primary surgeon in their 4th year of residency. The majority of the respondents (42.9%) also thought that continuous curvilinear capsulorhexis (CCC) was the most difficult procedure followed by nucleus removal (31.4%). Fourteen respondents (40.0%) experienced loss of lens fragments into the vitreous cavity. Nine (25.7%) respondents had experienced performing ECCE (Extracapsular cataract extraction) during residency training. Also, 48.6% performed 1 to 5 cataract procedures during their entire residency training while 20.0% performed 6 to 10. Conclusions: The experiences in cataract surgery during residency were investigated for the first time through a questionnaire survey. The Korean ophthalmology residency program needs to provide more opportunities for residents to perform cataract surgery during their training period. J Korean Ophthalmol Soc 2011;52(4):429-433

        Comparing the Effects of Particulate Matter on the Ocular Surfaces of Normal Eyes and a Dry Eye Rat Model

        <P>Conclusions: TiO2 nanoparticle exposure on the ocular surface had a more prominent effect in the EDE model than it did in the normal model. The ocular surface of dry eyes seems to be more vulnerable to fine dust of air pollution than that of normal eyes.</P>

        Latanoprost 점안 환자에서 발생한 단순포진결막염 1예

        Purpose: To report a case of herpetic conjunctivitis in a glaucoma patient using latanoprost for IOP control. Case summary: A 65-year-old female patient using latanoprost for six months reported ocular pain on her right eye. Under slit-lamp examination, there were only some punctate epithelial erosions and mild peri-limbal conjunctival injection at 7 o’clock direction. The patient used levofloxacin (Cravit??, Santen, Osaka, Japan) antibiotic eyedrops and dexamethasone (Forus ointment??, Samil, Seoul, Korea) every 6 hours in a day for each. However, the patient reported the aggravation of the symptom and the conjunctival injection became more prominent than 4 days before. With fluorescein stain and yellow filter, a geographic pattern of ulcer lesion was noticed in the 7 o’clock peri-limbal bulbar conjunctiva. She was diagnosed with herpetic conjunctivitis. She was educated to stop latanoprost and received antiviral therapy. After two weeks, the patient showed good response to the treatment and there was no recurrence. Conclusions: Atypical herpetic conjunctivitis which present with geographic ulcerative lesion might happen in the glaucoma patients using topical latanoprost. Because of that, one should not omit to check conjunctiva in those patients and the fluorescein stain with yellow filter can help to increase sensitivity on the conjunctival lesion in the slit-lamp examination.
