        Dynamic Protein Adsorption-Desorption Analysis of Contact Lenses in a Three-Dimensional-Printed Eye Model

        Adsorption of secreted protein on contact lenses is a dynamic and complex behavior. To understand this behavior, we used three-dimensional (3D) printing technology to create an eye model that simulated the anterior segment of the actual human eyeball. In this model, the fluid inlet was connected to a syringe pump to mimic the rate of human tear secretion and the outlet was connected to an ultraviolet- visible (UV-Vis) spectrophotometer. The experimental results revealed that the symmetrical eye model with a 180° inlet-outlet angle was suitable for dynamic analysis of protein adsorption. In this model, protein adsorption was slow and desorption was rapid. The contact lens was soaked in poly(2-methacryloyloxyethyl phosphorylcholine- co-butyl methacrylate) (PMB) to confirm the anti-protein adsorption property of this polymer through dynamic adsorption and desorption eye model analysis.

        Reduction of Physical Strength and Enhancement of Anti-Protein and Anti-Lipid Adsorption Abilities of Contact Lenses by Adding 2-Methacryloyloxyethyl Phosphorylcholine

        Biocompatible 2-methacryloyloxyethyl phosphorylcholine (MPC) can enhance the adsorption of water molecules and is therefore used for manufacturing contact lenses. This study investigated the mechanical strength, anti-protein deposition, and anti-lipid adsorption effects of MPC addition to contact lenses. Experimental contact lenses produced by copolymerizing multiple ratios of MPC to 2-hydroxyethyl methacrylate (HEMA) were analyzed. Atomic force microscopy revealed that MPC addition increased surface roughness. The anti-protein deposition and anti-lipid adsorption effects on poly(HEMA-MPC) polymers of various phosphorylcholine quantities were experimentally confirmed. The water content of the contact lenses was proportional to the MPC content in the polymer. The hydrated PC moiety of MPC drastically altered the network of the poly-HEMA polymer by inserting water molecules, which were trapped in the concave region of the surface. MPC addition had negative effects on all examined strength factors because of structural destabilization of the copolymer through water insertion. The anti-deposition effects of MPC were verified by examining the lysozyme and lipid adsorption abilities of the prepared contact lenses. Our results revealed that MPC enhanced interactions of the poly(HEMA-MPC) copolymer with water molecules; these interactions weakened the mechanical strength of the copolymer but markedly improved the anti-adsorption property of the biomolecules. The optimal proportion of HEMA–MPC for contact lenses is in the range 14.9%-28.5%.

        Acupuncture for ventilator-dependent patients at a hospital-based respiratory care center: A randomized controlled trial

        Background: In intensive care units, mechanical ventilation is an important therapy to help patients with dysp- nea. However, long-term ventilator dependence would consume huge medical resources and increase the risk of morbidity and mortality. The aim of the study was to examine the efficacy of the acupuncture combined with western medical care on ventilator parameters in ventilator-dependent patients. Methods: In this clinical trial, 80 ventilator-dependent patients aged 20 to 80 years old were randomly assigned to acupuncture group and control group in the respiratory care center (RCC) of Changhua Christian Hospital. Besides regular medical care and therapy, participants in the acupuncture group received acupuncture therapy at the same 17 acu-points for 20 minutes once a day, a total of 12 sessions. The ventilator parameters were recorded to evaluate the respiratory efficiency for all participants. The primary outcome was rapid shallow breathing index (RSBI), and secondary outcomes were respiratory rate (RR), tidal volume (TV) and ventilation per minute (MV). Results: Though there was no significant difference in the parameter between the acupuncture group and the control group, we found the trend of decreasing RSBI in the acupuncture group. In subgroup analyses, the mean of RSBI significantly decreased 16.02 (with the SD in 60.84) in acupuncture group, while it increased 17.84 (with the SD in 39.38) in control group (p = 0.036) after 12 sessions. Conclusion: Acupuncture treatment can improve breathing ability of patients with respirator dependence in respiratory care center.

        Association of Interleukin-10 A-592C Polymorphism in Taiwanese Children with Kawasaki Disease

        Elevated serum levels of interleukin-10 (IL-10) have been reported in patients with Kawasaki disease (KD). IL-10 reduces the inflammatory actions of macrophages and T cells and it may play a significant role in the regulation of inflammatory vascular damage associated with systemic vasculitis. The aim of this study was to examine whether -592 IL-10 promoter polymorphism is a susceptibility or severity marker of KD in Chinese patients in Taiwan. The study included 105 KD patients and 100 normal controls. Genotype and allelic frequencies for the IL-10 gene polymorphism in both groups were compared. There were no significant between-group differences in the genotype distribution of IL-10 A-592C gene polymorphism (P=0.08). However, the frequency of the -592*A allele was significantly increased in the patients with KD compared with controls (71.9% vs. 61.0%, P=0.019). The odds ratio for developing KD in individuals with IL-10 -592*A allele was 1.64 (95% confidence interval, 1.06-2.52) compared to individuals with the IL-10-592*C allele. No significant difference was observed in the genotype and allelic frequencies for the IL-10 A-592C polymorphism between patients with and without coronary artery lesions. The IL-10-592*A allele may be involved in the development of KD in Taiwanese children.

        IL-33/ST2 axis mediates hyperplasia of intrarenal urothelium in obstructive renal injury

        The monolayered intrarenal urothelium covers the renal papilla and ureteropelvic junction (UPJ). In response to increased renal pressure during obstruction or ischemic injuries, intrarenal urothelial cells begin to proliferate and form a multilayered urothelium. Little is known regarding the mechanism and pathophysiological role of urothelium hyperplasia during renal obstruction. In this study, we investigated the expression of interleukin (IL)-33, an IL-1 family cytokine, in kidneys with unilateral ureteral obstruction (UUO)-induced obstructive injury. IL-33 levels in hydronephrotic urine and serum were upregulated 2 days after UUO. The number of ST2-expressing immune cells was increased in the UUO kidney. We found that IL-33 was upregulated in vimentin-positive cells in the cortical and medullar layers and the UPJ stroma. Moreover, IL-33 expression was predominantly induced in multilayered keratin 5- positive urothelial cells in the UPJ. IL-33 was not detected in terminally differentiated superficial umbrella cells expressing uroplakin 3a. In vivo, we confirmed that deficiency of IL33 or its receptor ST2 attenuated UUO-induced hyperplasia of the UPJ urothelium. Deficiency of IL33 attenuated the expression of UUO-induced type 2 inflammatory cytokines and upregulated uroplakins and urothelial differentiation signaling in UPJ tissues. Our results collectively suggest that the IL-33/ST2 axis mediates the activation of innate immune responses and contributes to urothelial hyperplasia by regulating urothelial differentiation in obstructive kidney injury.

        <P><B>Summary</B></P> <P>Pro-inflammatory cytokines produced in the tumor microenvironment lead to eradication of anti-tumor immunity and enhanced tumor cell survival. In the current study, we identified tumor necrosis factor alpha (TNF-α) as a major factor triggering cancer cell immunosuppression against T cell surveillance via stabilization of programmed cell death-ligand 1 (PD-L1). We demonstrated that COP9 signalosome 5 (CSN5), induced by NF-κB p65, is required for TNF-α-mediated PD-L1 stabilization in cancer cells. CSN5 inhibits the ubiquitination and degradation of PD-L1.
