Purpose: Whether subtotal or total colectomy with primary anastomosis (PA) is safer than Hartmann procedure (HP) for left-sided colon cancer obstruction or perforation remains controversial. The purpose of this study was to compare postoperative morbidity, mortality, and defecation frequency between PA and HP for left-sided colon cancer obstruction or perforation. Methods: This retrospective study enrolled 54 patients from January 2014 to February 2018 who underwent emergency surgery due to left-sided colon cancer obstruction or perforation. Results: PA was carried out in 20 patients while HP was performed for 34 patients. Thirty-day mortality did not show significant difference between the two groups (15.0% vs. 14.7%, P=1.000). No anastomotic leakage occurred in PA group while three (8.8%) cases of stump leakage occurred in HP group. Stoma repair was performed for 13 cases (44.8%) and stoma reformation was performed for one case in HP group (7.7%). Stoma related complications occurred in five cases (17.24%). For patients after stoma repair, defecation frequency at 3 months after operation was 2.91±2.88 times per day in PA group and 2.86±2.63 times per day in HP group. At 1 year after operation, defecation frequency was changed to 1.40±1.12 times per day in PA group and 1.17±0.39 times per day in HP group. Conclusion: Primary ileosigmoid or ileorectal anastomosis for left-sided colon cancer obstruction or perforation is safe, and shows similar outcome of defecation frequency compared to H

<P><B>Abstract</B></P> <P>Understanding the frictional properties of phosphorene is essential for reliable fabrication and sustainable operation of phosphorene-based nanotechnology devices. Although recent studies have revealed that phosphorene exhibits anisotropic frictional characteristics, the detailed mechanisms are not well analyzed, and the influence of some experimental parameters (e.g., spring stiffness, tip load force, and tip size), which are very sensitive to atomic frictional forces, were not considered. This study was carried out to address the above shortcomings. The anisotropic frictional behavior of phosphorene and its detailed mechanism were analyzed using potential energy profiles. Also, the effects of spring stiffness and tip load force on the stick-slip behaviors were investigated. Furthermore, we studied the sub-nanoscale stick-slip behavior during the nanoscale slip motion. The nanoscale and sub-nanoscale stick-slip phenomena were originated from the tip's behavior of passing over the bond between the phosphorus atoms and over the puckered honeycomb structure along the zigzag and armchair directions. We utilized a simple one-dimensional model to explain the energy profiles. The influence of tip size on the stick-slip behavior was also examined and found related to the initial nanoscale slip velocity. As the tip diameter decreased, a high-frequency sub-nanoscale stick-slip phenomenon and shorter nanoscale slip duration were identified.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Detailed mechanisms of anisotropic frictional characteristics of phosphorene are analyzed. </LI> <LI> The influence of some experimental parameters (e.g., spring stiffness, tip load force, and tip size) on stick-slip behaviors was investigated in nano / sub-nano scale. </LI> <LI> The nanoscale and sub-nanoscale stick-slip phenomena were affected by phosphorene lattice structure at each scan direction. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Background: Current guidelines suggest the criteria for discontinuation of nucleos(t)ide analogues (NA) in selected patients. However treatment induced virological response is not permanent. Aim of our study is to evaluate the clinical significance of HBsAg titer in predicting sustained virologic response after NA therapy discontinuation. Methods: From Jun 1998 to Dec 2010, medical record of 81 chronic hepatitis B patients who discontinued NA was analyzed retrospectively. Sustained virologic response (SVR) was arbitrarily defined as undetectable HBV DNA by real-time PCR(with lower limit of detection of 116 copies/mL, 20 IU/mL) persisted more than 12 months after treatment discontinuation. Results: Median age was 51 years, 54 (67%) patients were male, and 50 (62%)patients were HBeAg positive. Median baseline ALT, HBV DNA and HBsAg were 292 IU/mL, 7.1log10 IU/mL and 3.3log10 IU/mL. NA were lamivudine (n=53), adefovir (n=15), lamivudine combined with adefovir (n=4), and entecavir (n=9). Median treatment duration and follow-up period were 26 and 27 months. 11/81 (14%) patients had SVR. The cumulative relapse rates were 37/81 (46%) at 6 months and 42/81 (52%) 12 months after treatment discontinuation. The baseline ALT, HBV DNA and presence of HBeAg were not different between patients with or without SVR. In univariate analysis, age, treatment duration and HBsAg level at treatment discontinuation were different in patients with or without SVR; 51 vs. 43 years, p=0.033; 53 vs. 25 months, p=0.011; 2.1 vs. 3.3log10 IU/mL, p=0.003. In multivariate analysis, only HBsAg level at treatment discontinuation remained as an independent factor associated with SVR (p=0.019). The cutoff value of HBsAg level <2log10 IU/mL was predictive of SVR [(AUROC, 0.991; 95% confidence interval[CI], 0.000-1.000; p<0.05); sensitivity, 100%; specificity, 93%; positive predictive value, 69%; negative predictive value, 100%]. Conclusions: Large proportion of patients treated with oral antivirals relapsed after the treatment discontinuation. In the decision of the treatment discontinuation, HBsAg level <2log10 IU/mL at treatment discontinuation can predict sustained viral suppression in selected patients.

<P>In a previous search for the differentially expressed genes in keratinocyte differentiation, we identified neutrophil gelatinase-associated lipocalin (NGAL) as a calcium-induced gene. In this study, we further verified the expression of NGAL in cultured keratinocytes as well as in several skin diseases. Reverse transcription-polymerase chain reaction (RT-PCR), Western blot, and ELISA clearly showed that NGAL expression was markedly increased in calcium-induced keratinocyte differentiation in vitro. However, in our previous report, NGAL expression was not detected in normal skin tissue except for hair follicle by in situ hybridization and immunohistochemistry, indicating the difference of cell status between in vitro and in vitro conditions. Interestingly, NGAL expression was highly increased in psoriasis-like inflammatory disorders (lichen planus and pityriasis rubura pilaris) and skin cancers (keratoacanthoma and squamous cell carcinoma), implying that NGAL may be related with the epidermal hyperplasia. Collectively, these results reveal the potential importance of NGAL in the maintenance of skin homeostasis.</P>

As a result of continuous taxonomic studies on the Korean crabs, Carcinus aestuarii Nardo, 1847 belonging to the superfamily Portunoidea is newly reported from Korean waters. Carcinus aestuarii has characteristics as followings: cardiac, hepartic and brachial regions are divided by deep furrow; shape of three lobes in frontal area is flatter with hairy; inside of carpus is with one sharp tooth; the posterior-lateral margin of the carapace is concave, and so on. The examined specimen doesn‘t have hairy and bump on outer margin of the chelipeds which differed from the previous description of the specimens collected from Tokyo Bay, Japan. Here, the diagnosis and the picture of Korean specimen is provided. Korean portunoids currently consist of 20 species belonging to 10 genera.

<P>Using grand canonical Monte Carlo (GCMC) simulations, we studied whether the new DOE targets for on-board methane storage can be attained if heat of adsorption at low loading (<I>Q</I>st°) is increased in eight representative metal–organic frameworks (MOFs) with varied pore volumes and surface areas. Because the deliverable capacity is more relevant to practical applications, we focus our discussions on the volumetric and gravimetric deliverable capacities. We found that the optimal <I>Q</I>st° for maximum CH4 delivery is approximately 20 kJ/mol. However, the new DOE targets are difficult to achieve in terms of the deliverable capacities, even after increasing <I>Q</I>st°. When the storage and discharge pressures are 65 and 1 bar, respectively, the volumetric and gravimetric targets can be attained under special combinations of pore volume and <I>Q</I>st°. Our results suggest that storing and delivering CH4 at lower temperatures could be considered to attain the new targets using current technologies, although it would result in some additional cost.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/jpccck/2014/jpccck.2014.118.issue-34/jp504983e/production/images/medium/jp-2014-04983e_0010.gif'></P>
