Archetype Model-Driven Development Framework for EHR Web System

Objectives: This article describes the Web application framework for Electronic Health Records (EHRs) we have developed to reduce construction costs for EHR sytems. Methods: The openEHR project has developed clinical model driven architecture for future-proof interoperable EHR systems. This project provides the specifications to standardize clinical domain model implementations, upon which the ISO/CEN 13606 standards are based. The reference implementation has been formally described in Eiffel. Moreover C# and Java implementations have been developed as reference. While scripting languages had been more popular because of their higher efficiency and faster development in recent years, they had not been involved in the openEHR implementations. From 2007, we have used the Ruby language and Ruby on Rails (RoR) as an agile development platform to implement EHR systems, which is in conformity with the openEHR specifications. Results: We implemented almost all of the specifications, the Archetype Definition Language parser, and RoR scaffold generator from archetype. Although some problems have emerged, most of them have been resolved. Conclusions: We have provided an agile EHR Web framework, which can build up Web systems from archetype models using RoR. The feasibility of the archetype model to provide semantic interoperability of EHRs has been demonstrated and we have verified that that it is suitable for the construction of EHR systems.
Integrated Production Planning and Scheduling Using Backward/Forward Scheduling Simulation

This paper deals with release time planning to reduce lead times of jobs in make-to-order productionenvironments. In traditional MRP-based planningsystem, lead times are estimated using historical data and pastexperience without precisely considering capacity constraints.The proposed method in this paper is based on precisebackward and forward scheduling simulation. First, backward simulation in which simulation starts at each due date ofjob and sends it through the shop floor based on the backward sequence of its part routing is applied. Next, forwardsimulation is applied to modify the release times obtained by backward simulation. A genetic algorithm is also used tominimize lead times of jobs in the final step. Numerical experiments of planning and scheduling with processing timeuncertainty are carried out to examine the effectiveness of the proposed method.

Background/Aims: The pharmacokinetics of tacrolimus (TAC) is known to be largely influenced by single-nucleotide polymorphisms (SNPs) in CYP3A5. Patients starting TAC require careful dose adjustment, owing to the wide range of optimal dosages, depending on their CYP3A5 expression status. Here, we evaluated whether individualization of TAC dosages based on CYP3A5 SNPs would improve its therapeutic efficacy in ulcerative colitis. Methods: Twenty-one patients were prospectively treated, with their initial dosage adjusted according to their CYP3A5 status (0.1, 0.15, and 0.2 mg/kg/day for CYP3A5*3/*3, CYP3A5*1/*3, and CYP3A5*1/*1, respectively). Their clinical outcomes were compared with those of patients treated with a fixed dose (0.1 mg/kg/day). Results: The first blood trough level of CYP3A5 expressors, CYP3A5*1/*3 or CYP3A5*1/*1, and the overall rate in achieving the target blood trough level within a week in the individualized-dose group were significantly higher than those in the fixed-dose group (5.15±2.33 ng/mL vs. 9.63±0.79 ng/mL, P=0.035 and 12.5% vs. 66.7%, P=0.01). The remission rate at 2 weeks in the expressors was as high as that in the nonexpressors, CYP3A5*3/*3, in the individualized-dose group. Conclusions: Individualized TAC treatment is effective against ulcerative colitis regardless of the CYP3A5 genotype. (Intest Res 2019;17:218-226)

This work focuses on the analysis of bioaerosols in the atmosphere at higher altitudes over Noto Peninsula, Japan. We carried out direct sampling via aircraft, separated cultures, and identified present isolates. Atmospheric bioaerosols at higher altitudes were collected using a Cessna 404 aircraft for an hour at an altitude of 3,500 m over the Noto Peninsula. The aircraft-based direct sampling system was devised to improve upon the system of balloon-based sampling. In order to examine pre-existing microorganism contamination on the surface of the aircraft body, bioaerosol sampling was carried out just before takeoff using the same method as atmospheric sampling. Identification was carried out by a homology search for 16S or 18S rDNA isolate sequences in DNA databases (GenBank). Isolate sampling just before takeoff revealed Stretpomyces sp., Micrococcus sp., and Cladosporium sp. One additional strain, Bacillus sp., was isolated from the sample after bioaerosol collection at high altitude. As the microorganism contamination on the aircraft body before takeoff differed from that while in the air, the presence of additional, higher atmosphere-based microorganisms was confirmed. It was found that Bacillus sp. was floating at an altitude of 3,500 m over Noto Peninsula.

Extracolonic involvement of the gastrointestinal tract is extremely uncommon in ulcerative colitis (UC) and rarely found in the upper gastrointestinal tract or in postoperative cases since it typically responds to steroids. Here we report a case of UC complicated by extensive ileal inflammation that was refractory to steroids. A 20-year-old man was diagnosed with UC of typical pancolitis without ileal involvement and started treatment with pH-dependent mesalazine and oral prednisolone. Although his symptoms transiently resolved, the condition flared when the steroid dose was tapered down. Computed tomography revealed marked thickening of the ileal wall, and capsule endoscopy and balloon-assisted enteroscopy found diffuse mucosal inflammation with ulcers in the ileum. On the contrary, the inflammation in the colon and rectum was improving. Since the response to the second steroid course was inadequate, treatment with adalimumab and 6-mercaptopurine was initiated and finally achieved clinical and endoscopic remission. The investigation of small intestinal lesions is necessary in patients with UC whose clinical deterioration cannot be explained by colonic lesions. (Intest Res 2017;15:535-539)
ALUMINUM BIAS SPUTTERING FOR FILM COVERAGE IMPROVEMENT

A filter was installed between the sputtering target and the substrate so as to reduce the aluminum deposition arriving with a shallow angle at the substrate in order to study the step coverage improvement by the bias application. It is deduced that the aluminum flowage occurs so that the surface energy is minimized around the via hole. Aluminum films deposited with the pulsed biasing were characterized in which the possible microstructual damage by argon bombardment is to be minimized. This biasing technique should provide with a better process for the step coverage improvement. A sputtering cathode was developed for aluminum bias sputtering which has a substrate current capability as high as 2A for a 5 inch wafer at the substrate voltage around-50V.
