        Correlation of Upper Lateral Cartilage Collapse and Nasal Septal Deviation

        Background and Objectives: This study aimed to elucidate the relationship between nasal septal deviation (NSD) and upper lateral cartilage (ULC) collapse.Methods: We conducted a retrospective review of 142 paranasal sinus computed tomography scans from patients who had undergone septoplasty due to confirmed NSD. The ULC angle was measured on both the narrow and wide sides of the nasal cavities, and these measurements were compared to evaluate the extent of ULC collapse in relation to the degree of NSD and patient age. The correlation between the degree of NSD and the degree of ULC collapse was also analyzed.Results: The mean ULC angle was found to be 13.4°±2.7° on the narrow side and 14.3°±2.7° on the wide side, with a statistically significant difference between the two (p<0.001). When comparing caudal and non-caudal NSD patients, there was no significant difference in the degree of ULC on either side (p=0.166). When comparing the ULC angle between two age groups (≥50 vs. <50 years), the difference in ULC angles was significantly greater in the group of patients aged under 50 years (0.3°±3.8° vs. 1.1°±2.2°, p=0.014). There was a significant positive correlation between the degree of ULC collapse and the degree of septal deviation (r=0.214, p=0.01).Conclusion: NSD was associated with ULC collapse on the narrow side and a narrow internal nasal valve area. This result indicates that clinicians should check for concomitant ULC collapse in patients with NSD.

        Laser technology in rhinology: a comprehensive review

        The integration of lasers with otorhinolaryngological interventions has paved the way for innovative treatments. This review delves into the varied applications of lasers in these fields of medicine. Carbon-dioxide (CO2) lasers are commonly used for their cost-effectiveness. However, they pose challenges in hemostasis. Diode and potassium titanyl phosphate (KTP) lasers, with their flexible fiber delivery, stand out as crucial equipment for multiple procedures. Low-level laser therapy shows therapeutic potential to address allergic rhinitis. Laser turbinate surgeries have garnered attention for their long-term benefits in treating turbinate hypertrophy and vasomotor rhinitis. Dacryocystorhinostomy leverages lasers for surgical precision. However, use of lasers to treat congenital choanal atresia raises concerns regarding the risk of postsurgical restenosis. The use of lasers in tumor removal, particularly endoscopic laser surgeries for tumor debulking, marks a shift from conventional rhinological approaches. Magnetic resonance imaging-guided laser-induced thermal therapy, especially for clival chordoma, is a notable advancement. In essence, lasers offer significant benefits in otorhinolaryngology but come with challenges, emphasizing the need for further research to maximize their clinical benefits.

        Intranasal Photobiomodulation Therapy for Brain Conditions: A Review

        The effects of low-level laser irradiation on cells and tissues, known as photobiomodulation therapy (PBMT), are the basis of photomedicine. Several investigations have evaluated the therapeutic effects of PBMT for neuronal regeneration and differentiation in animal models and humans. Recently, intranasal PBMT (iN-PBMT) has shown potential as a treatment method for neurologic disorders. In this review, we have summarized the various modes of iN-PBMT delivery and their application in the treatment of brain disorders.

        Effect of Photobiomodulation on the Mesenchymal Stem Cells

        Photobiomodulation forms the basis of photomedicine and is defined as the effect of coherent or non-coherent light sources, such as low-level lasers and light-emitting diodes, on cells and tissues. This treatment technique affects cell functions, proliferation, and migration, and plays an important role in tissue regeneration. Mesenchymal stem cells (MSCs) are known to be beneficial for tissue regeneration, and the combination of stem cell therapy and laser therapy appears to positively affect treatment outcomes. In general, a low-power laser has a positive effect on MSCs, thereby facilitating improvements in different disease models. This study elucidates the mechanisms and effects of low-power laser irradiation on the proliferation, migration, and differentiation of various MSCs that have been examined in different studies.

        Saddle nose deformity caused by CO2 laser vaporization of papillomas from the nasal septum: a case report

        A 48-year-old male presented with nasal obstruction due to multiple septal and nasal papillomas. He underwent nasal endoscopic CO2 laser vaporization for the papillomas. Recurrence was observed three months post-procedure, requiring a second laser treatment. The patient returned four years later with a saddle nose deformity, likely resulting from laser-induced damage to the septum’s structural components. This underscores the need for cautious use of lasers on delicate areas like the nasal septum. The patient underwent an augmentation rhinoplasty using GORE-TEX to correct the deformity, with satisfactory results. While lasers offer a minimally invasive approach to nasal lesions, potential complications like saddle nose deformity can arise. Regular follow-ups and patient education are essential.

        Phototherapy in Allergic Rhinitis: From In Vitro Studies to Clinical Trials

        Phototherapy has a profound immunosuppressive effect and is widely used for the treatment of inflammatory skin diseases. Since allergic rhinitis (AR) shares several common pathologic features with atopic dermatitis, intranasal phototherapy is a novel approach for treating AR. Phototherapy reduces the antigen presenting capacity of dendritic cells, induces apoptosis of immune cells, and inhibits synthesis and release of pro-inflammatory mediators from inflammatory cells. The effectiveness of phototherapy for AR has been extensively researched: from in vitro studies to several clinical trials. Data indicates the potential of intranasal phototherapy as an alternative treatment for AR as well as other inflammatory mucosal diseases. In this review, we introduce the in vitro, in vivo studies, and clinical trials which demonstrated the effect of phototherapy on AR.

        Radiofrequency Ablation and Microdebrider-Assisted Turbinoplasty: 5-Year Postoperative Outcomes

        Background and Objectives: Turbinoplasty may be an option in patients with persistent symptoms of allergic rhinitis (AR) despite adequate medication. The two most frequently used surgical techniques for turbinoplasty are radiofrequency ablation and microdebriderassisted turbinoplasty. This study compared the outcomes of these two surgical techniques and assessed the long-term treatment effects.Methods: This study included patients with AR who underwent turbinoplasty between January 2010 and June 2017. We examined the medication scores, overall treatment satisfaction scores, and medical records. Based on the type of surgery, patients were classified into microdebrider and coblator groups. A telephone survey was conducted to collect data on the 5-year post-treatment efficacy parameters. Scores for allergic symptoms, prescription medicines taken after surgery, and general satisfaction were compared.Results: Of the total 241 patients, 192 (79.7%) and 49 (20.3%) underwent microdebrider-assisted turbinoplasty and radiofrequency ablation turbinoplasty, respectively. Both the microdebrider and coblator groups showed remarkable improvements in AR symptoms at the 5-year follow-up (p<0.001). Although the degree of symptom improvement did not differ significantly between the two groups, the coblator group did show significant improvements in medication scores when compared with the microdebrider group (p<0.05), and there were significant differences in postoperative satisfaction scores between the two groups.Conclusion: In terms of 5-year postoperative outcomes, radiofrequency-ablation turbinoplasty and microdebrider-assisted turbinoplasty were equally beneficial for reducing nasal symptoms and enhancing medication ratings in patients with AR.

        DC 법으로 주조한 Al-10 wt.%Si-2 wt.%Cu-1 wt.%Mg-0.5 wt.%Mn 합금의 균질화

        Microstructure of a direct chill casted Al-10 wt.%Bi-2 wt.%Cu-1 wt.%Mg-0.5 wt.%Mn alloy φ180×2000 ㎜ in size was carefully investigated on the subsequent process after casting in order to search the optimum process condition suitable for mass production. Both the chemical modification by adding Sr into the melt and the rapid cooling rate by employing direct chill easting result in homogeneous cast structure consisting of primary Al and modified fine eutectic Si in spite of a large size of the ingot. The formation of ternary eutectics leads the segregation of CuMgAl₂ phase mainly in the center of the ingot. DSC reveals that ternary eutectics dissolves into the matrix around 520℃. The separation of network-structured fibrous eutectic Si to fine particle is found after one-hour heating above 460℃. Proper heat treatment causes equiaxed fine Si particle with the size of 10 ㎛ and complete dissolution of segregated CuMgAl₂ phase. Coarsening of separated Si particle is governed by diffusion controlled growth.
