The study focuses on the basic deformation mechanism of ECAP (Equal Channel Angular Pressing) which gives a large shear deformation without change in the reduction of area. A model deformation in which a plasticine is pressed through a transparent die shows a typical shear deformation in detail. ECAP was performed on a commercial pure aluminum and an Al-7%Si alloy in order to investigate the change in the microstructure after ECAP. Friction force between specimen and die acts to prevent uniform shear deformation of a specimen. ECAP significantly reduces grain size of both aluminum alloys below the order of micronmeter by giving a large shear deformation, which provides a new method for grain refinement. A drastic increase in the yield strength combined with a small increase in the strain hardening is observed in both ECA pressed aluminum alloys. The ECAP is believed to be very effective in the grain refinement.

Nanophthalmos is a rare disease that results from arrested development of the globe after closure of embryonic fissure and is characterized by a small eye with thickened sclera, uveal effusion, non-rhegmato-genous retinal detachment and narrow angle closure glaucoma. We experienced malignant glaucoma with nanophthalmos which has been taken intraocular surgery for cataract. We report this case with a review of the literature
Risk Factors for Mortality in Patients with Candidemia and the Usefulness of a Candida Score

Background: Although effective antifungal agents for the treatment of candidemia have recently been introduced, the mortality rate attributed to candidemia remains high (19~49%). Objective: This study aimed at evaluating the risk factors for mortality in patients with candidemia and at assessing the usefulness of a Candida Score in these patients. Methods: A cohort of patients with positive blood cultures for Candida species was retrospectively analyzed at Soonchunhyang University Hospital, a 750-bed teaching hospital, from May 2003 to February 2012. The Candida Score was calculated by assigning 1 point to any of total parenteral nutrition (TPN),surgery, or multifocal Candida species colonization, and 2 points to severe sepsis. Results: Sixty patients (68.3% men; mean age (standard deviation [SD]), 61.8 [18.9] years) with blood cultures positive for Candida species were identified. Most patients had been admitted to an intensive care unit (48 [80%]), were receiving broad-spectrum antibiotics (37 [61.7%]), had TPN (29[48.3%]), had diabetes mellitus (23 [38.3%]), and were receiving hemodialysis (10 [16.7%]). The mean (SD) Acute Physiology and Chronic Health Evaluation II (APACHE II) score was 19.60 (8.8). Twentythree patients (38.3%) had a Candida Score >2.5. The Candida species causing infection included C. albicans (41 [68.3%]), C. tropicalis (7 [11.7%]), C. parapsilosis (4 [6.7%]), C. krusei (3 [5%]), C. glabrata (3 [5%]), C. guilliermondii (1 [1.7%]), and C. catenulata (1 [1.7%]). Only 32 patients (53.3%)received adequate antifungal treatment. The candidemia-related mortality rate was 61.7% (n = 37patients). Multivariate logistic regression analysis demonstrated that a high APACHE II score (adjusted odds ratio [aOR], 1.2; 95% confidence interval [95% CI], 1.0~1.3; p = 0.01), presence of a malignancy (aOR, 14.8; 95% CI, 2.5~88.0; p = 0.003), and treatment with an antifungal agent (aOR, 0.2; 95% CI,0.0~1.0; p = 0.048) were associated with disease-related mortality. Conclusion: The risk factors for mortality in patients with candidemia are a high APACHE II scores and presence of a malignancy. However, the sensitivity of the Candida Score was not high (38.3%). New methods to rapidly identify candidemia and avoid delays in treatment with appropriate antifungal therapy are needed.

We analyzed postoperative astigmatic changes with Cravy vector analysis method in 58 eyes that had phacoemulsification and silicone intraocular lens inplatation through 3.2mm superior scleral tunnel incision (Group 1) and 25 eyes that had phacoenulsification and PMMA intraocular lens implantation through 5.7mm superior scleral tunnel incision(Group 2). Surgically induced astigmatism in group 1 was -0.11D at 1 day, -0.01D at 1 week, -0.20D at 1 month and -0.20D at 3 month. In group 2, it was -0.38D at 1 day, -0.38D at 1 week, -0.65D at 1 month and -0.72D at 3 month. There was no statistically significant difference between the two groups at postoperative 1 day(P>0.05). But, statistically significant difference between the two groups thereafter(P<%0.05). From our result, we concluded that larger superior scleral tunnel incision could induce larger astigmatism against the rule bue 3.2¼ scleral tunnel incision for foldable IOL insertion could induce only a smaller surgically induced astigmatism. The study is an attempt to understand the basic characteristics of ECAP(Equal Channel Angular Pressing) which gives large shear deformation without any change in the dimension of material. Microstructure comparison was performed between pure Al and Al-7 wt% Si alloys after the ECAP. Emphasis was put on the microstructural change of Al-Si alloys consisting of two phase (primary Al + eutectic Si) where eutectic Si appears different shape and size depending on the modification. The pressing leads to the development of subgrain bands of Al and the fracture of unmodified eutectic Si. The eutectic Si may form dislocation tangles by acting as an obstacle to movement of dislocation, which can be proved with the location of eutectic Si on recrystallized grain boundary. It is obvious that ECAP results in a fine grain structure in Al-Si alloy without decohesion between Al matrix and eutectic Si.
