Background: We hypothesized that the recent change of sepsis definition by sepsis-3 would facilitate the measurement of timing of sepsis for trauma patients presenting with initial systemic inflammatory response syndrome. Moreover, we investigated factors associated with sepsis according to the sepsis-3 definition. Methods: Trauma patients in a single level I trauma center were retrospectively reviewed from January 2014 to December 2016. Exclusion criteria were younger than 18 years, Injury Severity Score (ISS) <15, length of stay <8 days, transferred from other hospitals, uncertain trauma history, and incomplete medical records. A binary logistic regression test was used to identify the risk factors for sepsis-3. Results: A total of 3,869 patients were considered and, after a process of exclusion, 422 patients were reviewed. Fifty patients (11.85%) were diagnosed with sepsis. The sepsis group presented with higher mortality (14 [28.0%] vs. 17 [4.6%], P<0.001) and longer intensive care unit stay (23 days [range, 11 to 35 days] vs. 3 days [range, 1 to 9 days], P<0.001). Multivariate analysis demonstrated that, in men, high lactate level and red blood cell transfusion within 24 hours were risk factors for sepsis. The median timing of sepsis-3 was at 8 hospital days and 4 postoperative days. The most common focus was the respiratory system. Conclusions: Sepsis defined by sepsis-3 remains a critical issue in severe trauma patients. Male patients with higher ISS, lactate level, and red blood cell transfusion should be cared for with caution. Reassessment of sepsis should be considered at day 8 of hospital stay or day 4 postoperatively.
A Fatal Case of Disseminated Intestinal Mucormycosis in a Patient with Vibrio Sepsis

Mucormycosis is a fungal infection that primarily causes opportunistic infections. Gastrointestinal mucormycosis is a rare infection that can occur in immunocompromised patients, nevertheless, prompt diagnosis and treatment is essential because it can be fatal. Gastrointestinal mucormycosis can only be diagnosed based on the findings of a pathological examination. Mucormycosis should be included in the differential diagnosis if the condition of patients with underlying immunocompromised conditions or diseases does not improve with general intensive care.

이 글은 영화 <레 미제라블>을 바울의 칭의론과 율법관을 통해 살펴본다. 장발장과 자베르의 대조적인 삶을 복음과 율법이라는 주요한 신학적 테마에 기반하여 <레 미제라블>이 지니고 있는 바울 신학적 징후를 고찰한다. 이를 위해 바울신학을 <레 미제라블>에 강제하기보다는 <레 미제라블>의 전망에서 바울의 칭의론과 율법관이 지닌 현재적 의미를 레비나스와 데리다의 타자에 대한 환대의 윤리에서 살펴본다. 이러한 시도를 통해 성서의 서사와 메시지가 할리우드 영화 속에서 여전히 근원적 서사(proto-narrative)로 작동하고 있다는 사실을 발견한다. <레미제라블>은 바울의 율법관과 칭의론을 현대인들이 이해하는데 훌륭한 렌즈로 작용할 수 있다. 이 연구는 고통받는 ‘레 미제라블’을 향한 연민과 연대만이 하나님의 은총에 다다를 수 있다는 바울의 깨달음이 어떻게 장발장의 삶을 통해 구현되는 가를 보여준다. This article aims to explore the movie <Les Miserables> through Paul’s idea of justification and the law. The contrasting life of Jangbaljang and Jaber is examined based on the main theological theme of Paul’s theology about the gospel and the Law. To this end, rather than forcing Paul’s theology into <Les Miserables>, I investigate its modern meaning implicated in Paul’s theory of justification and the law, based on Levinas and Derrida s ethics of hospitality. Through these attempts, it is discovered that the biblical narrative and message are still working as proto-narratives in Hollywood movies. <Les Miserables> can provide an excellent lens for modern people to understand Paul s view of law and justification. This paper shows how the recognition that compassion and solidarity toward suffering in “Les Miserables” can reach the grace of God which is realized through the life of Jangbaljang.
Columnar 구조를 갖는 Se-Ge 이온 레지스트의 CF_4-RIE식각 및 FIB 노출 특성

비정질 Se-Ge 무기질 레지스트는 저 에너지 집속이온빔(low-energy focused-ion-beam; LE-FIB)에 의해 비교적 높은 분해능(resolution)과 건식식각시 높은대비도(contrast)를 갖는 특성을 가지 있다. Se-Ge 레 지스트가 비록 여러 장점을 가지고 있지만 일반 무기질 폴리머 레지스트에 비교하면 비교적 낮은 감도 (Sensitivity)를 띄는 단점을 내재하고 있다. 그래서 본 실험은 Columnar구조를 갖는 Se_75Ge_25 이온 레지스 트에 Ga^+ LE-FlB를 노출하고 CF_4-반응성 이온식각(reactive-ion-etching: RIE)을 수행하여 감도의 개선 및 그에 따라 형성된 sub-0.1μm 패턴특성에 대해 조사하였다 Se_75Ge_25 이온 레지스트를0˚, 60˚ 그리고 80˚의 증착각도를 갖도록 Si 기판위에 증착시키고, 증착된 레지스트중 일부는 유리질 천이온도(T_g: ~220℃)에서 열처리를 하였다. 80˚ 증착후 T_g-열처리된 레지스트의 threshold dose(D_t)는 약 7×10^14〔ions/cm^2〕정도로 측정되었고. 이것은 0˚로 증착되고 열처리되지 않은resist의 D_t와 비교하여 50%정도 개선된 값이다. 이 실험을 통해 0.06~0.09μm의 fine pattern을 얻을 수 있었다. Amorphous Se-Ge inorganic resist has high resolution and high contrast for dry-development by reactive-ion etching(RIE) and the exposure by low-energy focused-ion-beam(LE-FIB). Though this resist has several advantages its sensitivity is 1-2 order lower than that of conventional organic Polymer resist In this work, the sub-0.1m patterning characteristics of the columnar structural Se-Ge resist have been studied using Ga^+ LE-FIB exposure and CF_4-RIE development. The Se-Ge ion resist were normally(0˚) and obliquely(60˚ and 80˚) deposited on Si substrate and parts of resist were annealed for several minutes at glass transition temperature(T_g ~220℃). In case of a 80˚-obliquely deposited and T_g-annealed resist, the threshold dose(D_t) is evaluated to be approximately 7101^4(ions/cm^2). which corresponds to half of that for non-annealed resist(and normally deposited). The patterns with 0.06~o.09m linewidths have been successfully fabricated.
