        A Case of Nasal Septum Gossypiboma 14 Years After Septorhinoplasty

        Gossypiboma, an infrequent surgical complication, describes a mass of cotton material inadvertently left in the body cavity after an operation. It is an extremely rare iatrogenic complication of nasal surgery, with only a few cases reported in literature to date. Here we present a case of gossypiboma in the nasal septum of a 35-year-old male patient who previously underwent septorhinoplasty fourteen years prior. He was treated by endoscopic endonasal surgery to remove the lesion. Pathologic findings showed a foreign body (gauze filament) with a giant cell reaction. This report will be helpful for treating patients with similar histories in the future.

      • 비정상 난류 유동장에서 수직 분사 액주의 분열 및 기화에 관한 LES

        비정상 난류 유동장으로 분사되는 액체 제트의 액주 분열과 미립화 현상에 관한 LES를 수행하였다. 기체상태의 공기 유동 해석에 오일러리안 해법을 사용하고, 액적 추적을 위하여 라그랑지안 해법을 사용하여 기체-액체간 이상유동(two phase flow) 해석을 수행하였다. 액주의 1차 및 2차 분열이 관찰되었다. 일정한 속도로 유입되는 공기유동 중에 액체 분사 속도를 달리하여, 액체-기체 운동량 플럭스 비의 변화를 고려하여 액체 제트의 침투깊이를 조사하였으며 실험결과와 유사함을 알 수 있었다. 제트 후류에서 입자 평균직경에 대한 분석을 수행하였다. LES(Large eddy simulation) of breakup and atomization of a liquid jet into cross turbulent flow was performed. Two phase flow between a gas phase and a liquid phase was modeled by a mixed numerical scheme of both Eulerian and Lagrangian methods for gas and liquid phases respectively. The first and second breakup of liquid column was observed. The penetration depth in cross flow was comparable with experimental data for several variant of a liquid-gas momentum flux ratio by varying liquid injection velocities. SMD(Sauter Mean Diameter) distribution downstream of jet was analyzed.

        Particle Filtering Based Estimation of Consistent Motion and Disparity With Reduced Search Points

        <P>A particle filtering based block-wise estimation method for estimation of motion in a video sequence and joint estimation of disparity and motion in a stereo video sequence is proposed. Parameters of motion and disparity of a block in a sequence are defined as a state, and evolution of the state with respect to the block index is tracked with particle filtering. The state is assumed to be dependent on the states of neighboring blocks. Estimated motion and disparity fields are consistent and suitable for intermediate frame or view generation. The particle filter provides a method to effectively sample the search space. The particles are concentrated in regions where the probability density function for the state has large values. Hence, the locations of the particles are good candidates for search points of a fast search method. The proposed method can estimate motion and disparity with a fraction of search points necessary for conventional estimation methods.</P>

      • Gene Expression of Somatostatin Receptor Subtype 5 in Growth hormone (GH)-Secreting Pituitary Adenomas

        A great variability in the plasma GH response to a somatostatin (SRIH) analog, octreotide, was observed among acromegalic patients. A marked decrease in SRIH receptor (SSTR) density was postulated as a mechanism of resistance to native SRIH or octreotide. SSTR subtype 5 (SSTR5) is believed to be specific to the pituitary gland. Recently the possibility has been suggested that GH-secreting adenomas with gsp oncogene may have higher expression of SSTR than those without the oncogene. Therefore, we investigated the individual variation of SSTR5 expression in GH-secreting adenomas, and whether the SSTR5 gene expression is increased in gsp-positive adenomas. Genomic DNA and total RNA were prepared from the adenomas of 18 acromegalic patients. The point mutations at codon 201 and 227 of the stimulatory G protein alpha-subunit were examined by direct PCR sequencing. The expression of SSTR5 mRNA was determined by RT-PCR with in vitro transcription. gsp oncogene was found in 7 tumors (39%). In vitro transcript of SSTR5 cDNA was found in all the tumors. The amount of SSTR5 cDNA transcript, expressed as arbitary unit, was considerably variable from 19.6 to 220. 5 units. Relatively high expression of SSTR5 (≥100 units) was observed in 11 tumors (61%). The clinical characteristics of the patients with relatively high expression of SSTR5 did not differ from those of the patients with low expression. There was no correlation between the amount of SSTR5 cDNA transcript and the GH response to octreotide. The amount of SSTR5 cDNA transcript of the gsp-positive tumors did not differ from that of the gsp-negative tumors. These results suggest that SSTR5 expression is not a major determinant for the GH response to octreotide, and its expression is not increased in gsp-positive adenomas.

      • Transcriptional Activation of Rat Somatostatin Receptor Gene Expression by Intracellular cAMP in GH3 Cells

        An earlier study demonstrated that pituitary tumors of good octreotide responders have high density of somatostatin receptor (SSTR). We previously reported that growth hormone (GH)-secreting pituitary tumors with Gα_(s), mutations show good response to octreotide. As Gα_(s) mutations constitutively increase intracellular cAMP, we therefore hypothesized that the mutations may induce high expression of SSTR. As a pilot study for the hypothesis, we investigated whether SSTR gene expression is activated by intracellular cAMP. As somatostatin response of the pituitary gland is believed to be mediated through SSTR2 we examined SSTR2, mRNA expression in various rat organs (cerebral cortex, hypothalamus, cerebellum, brain stem, pituitary, thyroid, adrenal gland, and testis) and tumor cell lines (C6 glioma, AtT-20, GH3, GH1, CA77, and PC12) by Northern blot assay. By using human SSTR2 cDNA probe, we could not detect any mRNA even after 2 weeks in all the rat organs and the cell lines except AtT-20 which produces ACTH. Therefore, we used reverse transcription-polymerase chain reaction (RT-PCR) to detect very small amount of the SSTR2 message. SSTR2 transcript was found in most organs (brain stem>adrenal>hypothalamus>cerebellum>pituitary>testis) and cell lines (AtT-20>GH3>PC12 CA77). GH3 cells were stimulated with forskolin and isobutylmethylxanthine, cholera toxin, and GHRH for 2 hours. The in vitro transcript of SSTR2 was increased remarkably, at 30 min whereas β-actin transcript was comparable. These preliminary data suggest that SSTR2 gene expression is transcriptionally activated by intracellular cAMP, supporting our hypothesis that Gα_(s) mutations induce high expression of SSTR to compensate the putative stimulation of somatotroph. It remains to be investigated whether the mutations of Gα_(s) activate SSTR gene expression.

      • Shank2 Deletion in Parvalbumin Neurons Leads to Moderate Hyperactivity, Enhanced Self-Grooming and Suppressed Seizure Susceptibility in Mice

        <P>Shank2 is an abundant postsynaptic scaffolding protein implicated in neurodevelopmental and psychiatric disorders, including autism spectrum disorders (ASD). Deletion of <I>Shank2</I> in mice has been shown to induce social deficits, repetitive behaviors, and hyperactivity, but the identity of the cell types that contribute to these phenotypes has remained unclear. Here, we report a conditional mouse line with a <I>Shank2</I> deletion restricted to parvalbumin (PV)-positive neurons (<I>Pv-Cre;Shank2</I><SUP>fl/fl</SUP> mice). These mice display moderate hyperactivity in both novel and familiar environments and enhanced self-grooming in novel, but not familiar, environments. In contrast, they showed normal levels of social interaction, anxiety-like behavior, and learning and memory. Basal brain rhythms in <I>Pv-Cre;Shank2</I><SUP>fl/fl</SUP> mice, measured by electroencephalography, were normal, but susceptibility to pentylenetetrazole (PTZ)-induced seizures was decreased. These results suggest that <I>Shank2</I> deletion in PV-positive neurons leads to hyperactivity, enhanced self-grooming and suppressed brain excitation.</P>

      • Parallel Block Sequential Closed-Form Matting With Fan-Shaped Partitions

        <P>Applying alpha matting to large images is a challenging task because of its computational complexity. This paper provides a divide and conquer strategy for performing closed-form matting. The matting problem, defined for an entire image, is broken down into systems of linear equations defined for very small blocks of the image. The sizes of the small systems are small enough for us to find solutions efficiently using a direct sparse linear equation system solver. The small systems are solved following a sequential order such that the alpha matte grows from a user scribble. With the block sequential application, matting is performed on fan-shaped partitions in parallel on multiple processing cores. Experiments on large test images as well as on standard benchmark test images show that the proposed parallel block sequential matting provides high quality alpha mattes with good scalability.</P>

      • 성장호르몬분비성 뇌하수체 선종의 성장호르몬 방출호르몬 수용체 유전자의 발현과 내분비적 양상과의 관계

        Growth hormone-releasing hormone (GHRH) plays a key role in the regulation of the proliferation and differentiation of somatomammotroph cells as well as secretion of GH. The actions of GHRH are mediated through the GHRH receptor (GHRH-R) that is a G protein coupled receptor with seven transmembrane domains. It has been demonstrated that alternative splicing occurs in the third cytoplasmic domain of rat and human GHRH-R mRNA. However, the clinical significance of the alternative splicing remains to be unsolved. To find an insight into the clinical significance, we investigate the correlation between the GHRH-R gene expression and a variety of clinical clinical and endocrinological findings in 11 acromegalic patients. Three different sized cDNA fragments 250 bp, 700 bp and 810 bp, were found after RT-PCR. The amount of 250 bp fragment was higher than those of the other two fragments. The clinical findings (age, size, GH level, frequency of paradoxical response to TRH or GnRH. octreotide response, hypothalamc somatostatinergic activity of the group with high expression of the 250 bp fragment did not significantly differ from those of the group with low expression. The GHRH-R gene expression of tumors with gsp oncogene did not significantly differ from that of tumors without gsp oncogene. These results suggest that the expression of GHRH-R gene may not be an important determinant for tumor growth, and the lower GH response to GHRH of tumors with gsp oncogene may not be attributed to the lower expression of GHRH-R gene. The expression of GHRH-R is likely to be regulated by a certain property of tumors for GH secretion and growth.
