      • Arthroscopic mattress locking suture technique for the repair of small- and medium-sized rotator cuff tears

        Background: To prospectively compare the outcomes of an arthroscopic mattress locking (ML) suture repair and a simple suture repair of small or medium-sized rotator cuff tears. Methods: The 92 patients who had received an unsuccessful conservative treatment of small or medium-sized rotator cuff tears between April 2007 and October 2010 and who required an additional arthroscopic treatment were enrolled in the study. The patients were arbitrarily divided into the type of arthroscopic repair they received; 27 patients who received the arthroscopic ML suture were put into group 1, and 65 patients who received the simple suture were put into group 2. The mean age of patients at the time of surgery was 58 years, and the mean duration to final follow-up was 30 months. To measure the outcome of the treatment, visual analogue scale (VAS) for pain (clinical score), UCLA, American Shoulder and Elbow Surgeons (ASES) and Korean shoulder scoring (KSS) for the range of motion (functional scores) were measured for each patient. Lastly, the magnetic resonance imaging (MRI) scans of the patient’s shoulder were taken at the final follow-up. Results: Although the mean scores for clinical and functional outcomes (VAS, KSS, UCLA, and ASES scores) had improved from after the operation in both groups (P < 0.001), we found no significant differences in these improvements between the two groups (P > 0.001). Through MRI scans at the follow-up, we found that the proportion of re-tears was significantly lower in group 1 (2 patients, 7.41%) than in group 2 (12 patients, 18.46%). Conclusion: We found no significant difference in the clinical or functional outcomes of an arthroscopic ML suture repair and the simple suture repair of small-sized or medium-sized rotator cuff tears after a follow-up period of at least one year. However, the proportion of re-tears was significantly lower in patients who received ML sutures than in patients who received simple sutures.

      • Comparison of clinical outcomes of arthroscopic biceps long head tenodesis with or without tenotomy

        Background: The present study aims to evaluate the clinical outcomes of tenodesis followed by tenotomy and tenodesis without tenotomy for biceps long head tendon lesions during arthroscopic small- to medium-sized full-thickness rotator cuff repair. Methods: This study was conducted on 33 patients who were available for at least 1-year regular follow-up after tenodesis of the long head of the biceps during arthroscopic small- to medium-sized full thickness rotator cuff repair between November 2009 and November 2010. Group I consisted of 20 patients who underwent tenodesis with tenotomy and group II consisted of 13 patients who underwent tenodesis without tenotomy. The outcomes were evaluated by visual analogue scale (VAS), UCLA scores, Speed’s test, and direct tenderness on the bicipital groove. Results: At the last follow-up, the VAS decreased from a preoperative mean of 7.5 to a postoperative mean of 3.0 in group I and from a preoperative mean of 8.0 to a postoperative mean of 3.2 in group II (P < 0.05). The UCLA score increased from a preoperative mean of 15.7 to a postoperative mean of 32.3 in group I and from a preoperative mean of 13.8 to a postoperative mean of 29.4 in group II (P < 0.05). The number of positive Speed’s test decreased from 13 preoperatively to 5 postoperatively in group I and from 9 preoperatively to 2 postoperatively in group II (P < 0.05). The number of patients with direct tenderness on the bicipital groove decreased from 17 preoperatively to 3 postoperatively in group I and from 12 preoperatively to 4 postoperatively in group II (P < 0.05). Conclusion: The use of tenotomy in combination with tenodesis for biceps long head tendon lesions had no significant impact on the clinical outcomes of arthroscopic rotator cuff repair.

        Risk factors for injuries related to player substitutions during Korean professional football league matches

        Background: This study aimed to examine the risk factors for injuries related to player substitutions during Korean professional football league matches. Methods: Ninety-five professional football players who were evaluated by the team’s medical staff for match-related injuries were included in this study. The players were divided into group I (n = 43; did not return) and II (n = 52; returned) based on information regarding their ability to return to the match after injury. We collected the demographic data of players and match-related information and investigated the risk factors of injuries that caused player substitutions using a paired t-test and logistic regression analysis. Results: The demographic characteristics of both groups were not statistically different. Lower limb injuries were more common in group I, whereas head and neck, trunk, and upper limb injuries were more common in group II. Contact injuries were common in both groups. The proportion of noncontact injuries among all injuries was 41.8% and 11.5% in group I and II, respectively. Depending on the location and mechanism of injury, 100% of noncontact trunk injuries, 81.0% of noncontact lower limb injuries, 25.0% of contact upper limb injuries, and 17.4% of contact head and neck injuries caused player substitutions. Lower limb and noncontact injuries increased the risk of substitution by 7.2- and 3.5-fold, respectively. Conclusion: Lower limb and noncontact injuries were identified as the major risk factors causing player substitutions during Korean professional football league matches. This information should be useful for preparing strategies to prevent such injuries in the future.

      • Resistance exercise improves cardiac function and mitochondrial efficiency in diabetic rat hearts

        <P>Metabolic disturbance and mitochondrial dysfunction are a hallmark of diabetic cardiomyopathy (DC). Resistance exercise (RE) not only enhances the condition of healthy individuals but could also improve the status of those with disease. However, the beneficial effects of RE in the prevention of DC and mitochondrial dysfunction are uncertain. Therefore, this study investigated whether RE attenuates DC by improving mitochondrial function using an in vivo rat model of diabetes. Fourteen Otsuka Long-Evans Tokushima Fatty rats were assigned to sedentary control (SC, n = 7) and RE (n = 7) groups at 28 weeks of age. Long-Evans Tokushima Otsuka rats were used as the non-diabetic control. The RE rats were trained by 20 repetitions of climbing a ladder 5 days per week. RE rats exhibited higher glucose uptake and lower lipid profiles, indicating changes in energy metabolism. RE rats significantly increased the ejection fraction and fractional shortening compared with the SC rats. Isolated mitochondria in RE rats showed increase in mitochondrial numbers, which were accompanied by higher expression of mitochondrial biogenesis proteins such as proliferator-activated receptor-gamma coactivator-1 alpha and TFAM. Moreover, RE rats reduced proton leakage and reactive oxygen species production, with higher membrane potential. These results were accompanied by higher superoxide dismutase 2 and lower uncoupling protein 2 (UCP2) and UCP3 levels in RE rats. These data suggest that RE is effective at ameliorating DC by improving mitochondrial function, which may contribute to the maintenance of diabetic cardiac contractility.</P>

      • Effect of Peak Current Limiting in Series-Connection SFCL With Two Magnetically Coupled Circuits Using E-I Core

        <P>In this paper, we proposed a series-connection-type superconducting fault current limiter (SFCL) that can prevent the internal magnetic flux generation of cores during normal operation and prevent the saturation of cores due to a sudden magnetic flux generation at the initial stage of fault occurrence while limiting the peak current. The proposed SFCL does not mediate two cores, as previously reported, but mediates a single E-I core having a configuration of two magnetically coupled circuits using the first, second, and third windings. The other magnetic path is formed by the third coil and the other superconducting element isolated from two coils. Through a short-circuit experiment, we analyzed the operating status of the two superconducting elements and peak current limiting properties according to the amplitude of the fault current before and after the failure. Furthermore, the peak current limiting characteristics according to the winding directions, as well as the current and the voltage of each coil, were compared and analyzed.</P>

      • Continuous electrochemical detection of hydrogen peroxide by Au-Ag bimetallic nanoparticles in microfluidic devices

        <P><B>Abstract</B></P> <P>We developed electrochemical microfluidic devices containing Au-Ag bimetallic nanoparticles on single-walled carbon nanotube (SWCNT) electrodes for precise, rapid and non-enzymatic electrochemical detection of hydrogen peroxide (H<SUB>2</SUB>O<SUB>2</SUB>). A homogeneous SWCNT layer on a glass substrate was patterned by a standard photolithography method and an O<SUB>2</SUB>-plasma technique. The patterned SWCNT layers were used as working electrode, counter electrode, and reference electrode in a 1mm wide and 23μm deep microchannel. To enhance the electrochemical catalytic properties of the working electrode, Au covered Ag nanoparticles (NPs) were electrochemically deposited on the SWCNT working electrode with current streaming solution into a microfluidic channel. The AgNPs had catalytic activity to improve the reduction of H<SUB>2</SUB>O<SUB>2</SUB> combined with AuNPs, which could support catalytic reaction of AgNPs. The reduction in H<SUB>2</SUB>O<SUB>2</SUB> in fabricated devices was monitored at an optimized condition and was linear with H<SUB>2</SUB>O<SUB>2</SUB> concentration, resulting in a sensitivity of 13.1μA·cm<SUP>−2</SUP>·mM<SUP>−1</SUP>, a correlation coefficient of r<SUP>2</SUP> =0.985, and limit of detection 26.8μM (S/N=3). For the analysis of real samples, diluted antiseptic solutions containing 3wt% H<SUB>2</SUB>O<SUB>2</SUB> were used to determine the concentration of H<SUB>2</SUB>O<SUB>2</SUB> in the pharmaceutical sample. The recovery values ranged from 97% to 107%, and relative standard deviations were below 5%.
