        Delorme’s Procedure: An Effective Treatment for a Full-Thickness Rectal Prolapse in Young Patients

        Purpose: Delorme’s procedure is infrequently applied in young adults because of its assumed higher recurrence rate. The aim of this prospective study was to assess the efficacy of the Delorme’s technique in younger adults. Methods: Fifty-two consecutive patients were entered in our study. We followed patients for at least 30 months. Their complaints and clinical exam results were noted. Results: Our study included 52 patients (mean age, 38.44 years; standard deviation, 13.7 years). Of the included patients, 41(78.8%) were younger than 50 years of age, and 11 (21.1%) were older than 50 years of age. No postoperative mortalities or major complications were noted. Minor complications were seen in 5 patients (9.6%) after surgery. The mean hospital stay was 2.5 days. In the younger group (age ≤50 years), fecal incontinence was improved in 92.3% (12 out of 13 with previous incontinence) of the patients, and recurrence was seen in 9.75% (4 patients). In the older group (age >50 years), fecal incontinence was improved in 20% (1 out of 5 with previous incontinence) of the patients, and recurrence was seen in 18.2% (2patients). In 50% of the patients with a previous recurrence (3 out of 6 patients) following Delorme’s procedure as a secondary procedure, recurrence was observed. Conclusion: Delorme’s procedure, especially in younger patients, is a relatively safe and effective treatment and should not be restricted to older frail patients. This procedure may not be suitable for recurrent cases.

        The joint reaction of methanol and i-butane over the HZSM-5 zeolite

        The effects of i-butane addition to methanol in MTP reaction were investigated over an in-house prepared HZSM-5 catalyst. It was observed that, propylene yield would be enhanced when i-butane fed to the reactor along with methanol. The rising growth of the propylene yield continued to peak on till the balance in thermal condition established. Similar trends have been observed when water was added to the mixture. The effect of WHSV with fixed water composition on product distribution was also studied. The optimum point where the highest amount of propylene yielded was shown to be high depended upon the temperature and residence time.

        Transesterification of canola oil and methanol by lithium impregnated CaO–La2O3 mixed oxide for biodiesel synthesis

        CaO–La2O3 mixed oxides were synthesized by co-precipitation coupled with Li doping through wetimpregnation. These were used as catalysts for transesterification of canola oil and methanol towardbiodiesel production. To determine the structure and morphology of the prepared catalysts, they werecharacterized by the XRD, FESEM, BET, and basic strength measurements. Under optimum reactionconditions of methanol/oil molar ratio of 15:1, 5 wt% catalyst at 65 C, 96.3% conversion was obtained in2.5 h of reaction duration. Moreover, the catalyst demonstrated a rather high stability where reuse of uptofive cycles without significant loss of performance observed.

        Kinetic modeling of oxidative dehydrogenation of propane (ODHP) over a vanadium–graphene catalyst: Application of the DOE and ANN methodologies

        In this research the application of design of experiment (DOE) coupled with the artificial neural networks (ANN) in kinetic study of oxidative dehydrogenation of propane (ODHP) over a vanadium–graphene catalyst at 400–500 ℃ and a method of data collection/fitting for the experiments were presented. The proposed reaction network composed of consecutive and simultaneous reactions with kinetics expressed by simple power law equations involving a total of 20 unknown parameters (10 reaction orders and 5 rate constants each expressed in terms of a pre-exponential factors and activation energies) determined through non-linear regression analysis. Because of the complex nature of the system, neural networks were employed as an efficient and accurate tool to model the behavior of the system. Response surface methodology (RSM) and ANN methods were constructed based upon the DOE’s points and were then utilized for generating extra-simulated data. The three data sets including the original experimental data, those simulated by the ANN and RSM methods were subsequently used to fit power law kinetic rate expressions for the main ODHP and side reactions. The results of kinetic modeling with simulated data sets from the ANN and RSM models compared with collected experimental data. Both methods were able to satisfactorily fit the experimental data for which the ANN data set showed the best fitting amongst them all.

        Simulation of continuous catalytic conversion of glycerol into lactic acid in a microreactor system: A CFD study

        Hand in hand with the flourish of the biodiesel industry, glycerol (GLY) as an inconvenient by-product hasgenerated environmental and sustainability concerns. Devising measures for efficient transformation ofGLY into value-added products is a promising solution. In this contribution, the transformation of GLYto lactic acid (LA) as a valuable chemical was investigated in a continuous process for industrial applications. In this regard, a catalytic method using heterogeneous Cu nanoparticles in NaOH solution was studiedin a microreactor by a CFD simulation. A seven-inlet micromixer comprising an optimized mixing unitwas incorporated for uniform distribution of the species. The effects of various parameters upon the processperformance were considered and the optimum points were determined. Also, the extent of theinfluence of each variable on LA yield was evaluated using sensitivity analysis techniques. While higherLA yield could be obtained at extreme scenarios, optimum values of the Re and temperature for obtainingthe maximum performance under sensible operating conditions were determined to be 0.108 and 510.1 Kwhich led to the optimum yields of 67.8% and 59.5%, respectively. Moreover, the sensitivity analysisrevealed that the molar ratio of OH /GLY and temperature were the most and least significant parameters,respectively.

        Introduction of a handmade vacuum-assisted sponge drain for the treatment of anastomotic leakage after low anterior rectal resection

        Purpose: Anastomotic leakage, a known major postoperative complication, potentially leads to readmission, reoperation, and increased mortality rates in patients, such as rectal cancer patients following a low anterior resection (LAR). Currently, vacuum-assisted closure, as featured by B-Braun (B-Braun Medical B.V.), is already being used for the treatment of gastrointestinal leakages and fistulas. The main aim of this study was to introduce a novel method for creating a vacuum-assisted drain for the treatment of anastomotic leakage after LAR. Methods: All 10 patients, who underwent LAR surgery from 2018 to 2019, were diagnosed with anastomotic leakage and had received neoadjuvant chemotherapy prior to surgery. Therefore, patients were treated with a handmade vacuum-assisted drain and were revisited every 5 to 7 days for further evaluations and drain replacement until leakage resolution. Physical features of cavity, time of diagnose, and duration of treatment were analyzed correspondingly. The handmade vacuum-assisted sponge drain was prepared for each patient in each session of follow-up. Results: Eight out of 10 patients experienced complete closure of the defect. The mean delay time from the day of operation to the diagnosis of anastomotic leakage was 61.0±80.4 days while the mean time for leakage closure was 117.6±68.3 days. Eventually, 7 cases underwent ileostomy reversal with no complications during a 3-month follow-up. Conclusion: In this study, we evaluated the healing process of anastomotic leakage after the usage of a handmade vacuum-assisted sponge drain in a case series method. In our trial, we provided an innovative cost-benefit method easily applicable in the operating room.
