The Korean Society for Electrolyte and Blood Pressure Research, in collaboration with the Korean Society of Nephrology, has published a clinical practice guideline (CPG) document for hyponatremia treatment. The document is based on an extensive evidence-based review of the diagnosis, evaluation, and treatment of hyponatremia with the multidisciplinary participation of representative experts in hyponatremia with methodologist support for guideline development. This CPG consists of 12 recommendations (two for diagnosis, eight for treatment, and two for special situations) based on eight detailed topics and nine key questions. Each recommendation begins with statements graded by the strength of the recommendations and the quality of the evidence. Each statement is followed by rationale supporting the recommendations. The committee issued conditional recommendations in favor of rapid intermittent bolus administration of hypertonic saline in severe hyponatremia, the use of vasopressin receptor antagonists in heart failure with hypervolemic hyponatremia, and syndrome of inappropriate antidiuresis with moderate to severe hyponatremia, the individualization of desmopressin use, and strong recommendation on the administration of isotonic fluids as maintenance fluid therapy in hospitalized pediatric patients. We hope that this CPG will provide useful recommendations in practice, with the aim of providing clinical support for shared decision-making to improve patient outcomes.
Highly selective colorimetric detection of Zn(II) ions using label-free silver nanoparticles

<P><B>Abstract</B></P> <P>A simple and facile colorimetric method for the detection of zinc ions (Zn<SUP>2+</SUP>) using label-free silver nanoparticles (AgNPs) has been developed. Zn<SUP>2+</SUP> ions induced the aggregation of label-free AgNPs in solution, resulting in a color change from yellow to forest green, which was accompanied by the appearance of a new surface plasmon absorption band at 635nm. The sensitivity of label-free AgNPs towards other metal ions was negligible, with the exception of Fe<SUP>3+</SUP>, which caused a modest interference. The binding site and sensing mechanism for Zn<SUP>2+</SUP> ions in label-free AgNPs were characterized by Fourier Transform infrared spectroscopy, ultraviolet-visible spectroscopy, and transmission electron microscopy. The aggregation of AgNPs in the presence of Zn<SUP>2+</SUP> ions was inhibited by polyvinyl alcohol, which formed a shell on the surface of AgNPs. Furthermore, Zn<SUP>2+</SUP> ions coordinated with BH<SUB>4</SUB> <SUP>−</SUP> on AgNPs through a triangular sixdentate binding structure, thus inducing aggregation of AgNPs in solution.</P> <P>The sensor conditions were optimized with 6.0μM of sodium dodecylbenzenesulfonate (NaDDBS) and 0.23mM AgNPs at pH 6. The absorption ratios (A<SUB>635</SUB>/A<SUB>390</SUB>) of the label-free AgNPs solution exhibited a linear correlation with Zn<SUP>2+</SUP> ion concentration within the range of 0.0−7.6×10<SUP>−6</SUP> molL<SUP>−1</SUP>, with a limit of detection of 0.36×10<SUP>−6</SUP> molL<SUP>−1</SUP>. HOW TO DEVELOPE NEW PRO BIOTIC WITH ANTI Helicobacter pylori FUNCTION

Lactic acid producing bacteria were isolated from baby feces and characterized to be used as a probiotic with anti Helicobacter pylori functions. The selected bacteria had inhibition activity on the adherance and growth of H. pylori. These bacteria had additional beneficial characteristics for the probiotic such as antibacterial activity, antitumor activity, immunostimulation activity, resistance to antibiotic and bile salt, ability to bind to the intestinal cells, and safe for the human use.

한강의 오염도 측정을 위한 생물학적 지표로 대장균의 계절별 분포도를 측정하여 한강의 오염도를 예측하고, 상수원 및 레저용 수질에 적합한지를 파악하고자 하였다. 2000년 4월부터 2001년 3월까지 일년동안 매달 1-2회 한강 지류인 중랑천에서 대장균을 분리하였다. 우리나라의 수질 규격으로 식수원의 일반세균 규격은 100 CFU/㎖ 이고 대장균의 기준은 원수의 경우 50 CFU/㎖. 정수된 후에는 음성이어야 하며, 레저용수의 대장균 기준은 음성이고, 하수처리장의 경우 ㎖당 1,000개 이하이다. 본 실혐의 결과는 1년 동안의 중랑천의 수질이 상수원이나 레저용으로 부적당한 것으로 나타났는데, 4월과 1월에는 대장균이 검출되지 않아 상수원이나 레저용수로 적합할 것으로 나타났으나, 일반세균의 기준치인 100 CFU/㎖을 크게 벗어나 상수원이나 레저용으로 사용하기 부적당한 것으로 나타났다. 4월과 1월을 제외하고는 대장균이 검출되어 상수원이나 레저용으로 적합지 못한 것 으로 나타났고, 특히 9월과 10월에는 하수처리장의 기준치인 1,000 CFU/㎖에 비해 각각 1.140 CFU/㎖과 1.030 CFU/㎖ 이 검출되어 보다 엄격한 처리가 필요한 것으로 나타났다. 또한 최근에 심각한 문제로 대두되고 있는 항생제내성 세균의 환경중 분포경향을 파악하고자 하여 항생제 내성 대장균의 분포경향을 관찰하였다. Cephalothin 내성 대장균은 5월 이후로 증가되어 9월에 최대치를 보이다가 점차 감소하는 것으로 나타났고, norfloxacin 내성 대장균은 2000년 5월부터 9월까지 발견되었다가 그 이후로는 발견되지 않았다. 이러한 결과는 한강의 수질을 보전하기 위해서 중랑천 주변의 생활하수 및 축산폐수의 하수 처리를 엄격히 하여 수질을 보전해야 함을 시사하고 있다. In order to measure the contamination level of Han River, Escherichia coli, the biological indicator for the pollution, was observed to check the quality of water whether it is proper as the source for drinking water or recreational impoundment. Surface water of Joongrang stream, a branch of Han River, was collected monthly from April 2000 to March 2001. Guideline of E. coli for drinking water or recreational impoundment is negative and for sewage is less than 1,000 CFU/㎖. All year round, Joongrang stream was not proper for drinking water or recreation. CFUs of total bacteria were exceeded the guideline for source of drinking water, even though E. coli was not detected on April and January. Except these months, E. coli was detected especially during a period from September to October and CFUs of E. coli detected in Joongrang stream were 1,140 CFU/㎖ and 1,030 CFU/㎖ exceeding the guideline for sewage treatment. When CFU of antibiotic-resistant E. coli were counted in order to understand the influence of the misuse or overuse of antibiotics to the environmental pollution. Cephalothin-resistant E. coli were present all around the year except April and Febraury. And norfloxacin-resistant E. coli were detected from May to September. This showed that the Joongrang stream must be treated before it entered to the mainstream.
Evaluation of Twenty Fluoroqunolones with MRSA

임상에서 분리한 533개의 methicillin resistant Staphylococcus aureus (MRSA)를 20개의 불화 퀴놀론에 대한 감수성 정도에 따라 5개의 군으로 나누었다. 각 군에서 5개의 균주를 무작위로 선발하여 각 항생제의 투과도, 배출효과 및 DNA gyrase에 대한 억제 효과에 관하여 연구하였다. 대부분의 MRSA가 불화퀴놀론에 대하여 내성을 나타냈으며, 특히 기존의 불화퀴놀론인 ciprofloxacin, norfloxacin, ofloxacin, sparfloxacln에 대하여는 40% 이상의 MRSA가 내성을 나타냈다. MIC 및 disk test의 결과에 의하면 기존의 불화 퀴놀론보다는 KR-series가 MRSA에 대하여 더 좋은 억제효과를 나타냈다. 그 중 KR10984와 KR11048이 가장 강력한 억제효과를 나타냈으며, 대부분의 MRSA가 이들에 대해 감수성을 보였다. 세포내로의 투과도가 좋거나 배출이 잘 안되는 불화퀴놀론보다는 불화 퀴놀론의 작용부위인 DNA gyrase에 대한 억제도가 좋은 화합물이 낮은 MIC 값을 나타냈다. C-7기에 1-amino-5-azaspiro-[2,4]heptene-5-yl기를 갖는 불화 퀴놀론들이 DNA gyrase에 대하여 높은 억제효과를 나타내 이런 경우에 해당되었다.
IP Geolocation with a Crowd-sourcing Broadband Performance Tool

<P>In this paper, we propose an IP geolocation DB creation method based on a crowd-sourcing Internet broadband performance measurement tagged with locations and present an IP geolocation DB based on 7 years of Internet broadband performance data in Korea. Compared with other commercial IP geolocation DBs, our crowd-sourcing IP geolocation DB shows increased accuracy with fine-grained granularity. We confirm that the low accuracy of commercial IP geolocation DBs mainly results from selecting a single representative location for a large IP block from the Whois registry DB, parsing city names in a naive way, and resolving the wrong geolocation coordinates. We also found that the geographic location of IP blocks has continuously changed but has been stable. Although our IP geolocation DB is limited to Korea, the 32 million broadband performance test records over 7 years provide wide coverage as well as fine-grained accuracy.</P>
Understanding Mind-Body Interaction from the Perspective of East Asian Medicine

<P><B>Objective</B></P><P> Attempts to understand the emotion have evolved from the perspective of an independent cognitive system of the mind to that of an interactive response involving the body. This study aimed to quantify and visualize relationships between different emotions and bodily organ systems from the perspective of East Asian medicine.</P><P><B> Methods</B></P><P> Term frequency-inverse document frequency (tf-idf) method was used to quantify the significance of Five Viscera and the gallbladder relative to seven different emotions through the classical medical text of DongUiBoGam. Bodily organs that corresponded to different emotions were visualized using a body template.</P><P><B> Results</B></P><P> The emotions had superior tf-idf values with the following bodily organs: anger with the liver, happiness with the heart, thoughtfulness with the heart and spleen, sadness with the heart and lungs, fear with the kidneys and the heart, surprise with the heart and the gallbladder, and anxiety with the heart and the lungs. Specific patterns between the emotions and corresponding bodily organ systems were identified.</P><P><B> Conclusion</B></P><P> The present findings will further the current understanding of the relationship between the mind and body from the perspective of East Asian medicine. Western medicine characterizes emotional disorders using “neural” language while East Asian medicine uses “somatic” language. </P>
