<P>Synaptic adhesion molecules have been extensively studied for their contribution to the regulation of synapse development through trans-synaptic adhesions. However, accumulating evidence increasingly indicates that synaptic adhesion molecules are also involved in the regulation of excitatory synaptic transmission and plasticity, often through direct or close associations with excitatory neurotransmitter receptors. Synaptic adhesion molecule IgSF11 regulates synaptic transmission and plasticity

<P>Synaptic adhesion molecules regulate synapse development and plasticity through mechanisms that include trans-synaptic adhesion and recruitment of diverse synaptic proteins. We found that the immunoglobulin superfamily member 11 (IgSF11), a homophilic adhesion molecule that preferentially expressed in the brain, is a dual-binding partner of the postsynaptic scaffolding protein PSD-95 and AMPA glutamate receptors (AMPARs). IgSF11 required PSD-95 binding for its excitatory synaptic localization. In addition, IgSF11 stabilized synaptic AMPARs, as determined by IgSF11 knockdown-induced suppression of AMPAR-mediated synaptic transmission and increased surface mobility of AMPARs, measured by high-throughput, single-molecule tracking. IgSF11 deletion in mice led to the suppression of AMPAR-mediated synaptic transmission in the dentate gyrus and long-term potentiation in the CA1 region of the hippocampus. IgSF11 did not regulate the functional characteristics of AMPARs, including desensitization, deactivation or recovery. These results suggest that IgSF11 regulates excitatory synaptic transmission and plasticity through its tripartite interactions with PSD-95 and AMPARs.</P>

Cerebrospinal fluid (CSF) leakage is a rare complication that can occur due to dural defects during spinal surgery, hindering the improvement of the surgical site and increasing the possibility of infection. DuraSeal® is a dural sealing adhesive that prevents CSF leakage and is used as an adjunct to enable the watertight repair of dural defects when the dura is damaged during spinal surgery. In the present case, DuraSeal® was applied to repair a dural defect in the surgical area after thoracic spine surgery, and no neurological problems occurred immediately after surgery. However, a day later, the patient’s paraparesis worsened; therefore, reoperation was performed and the symptoms improved.

Objective: The objective of this nationwide, long-term follow-up study was to explore the connection between congestive heart failure (CHF) and ossification of the posterior longitudinal ligament (OPLL) in Korea.Methods: Patient information was collected from the Health Screening cohort of the National Health Insurance Service. Individuals diagnosed with OPLL were identified using specific International Classification of Diseases, 10th revision codes (M48.8, M48.81, M48.82, and M48.83). A total of 1,289 OPLL patients and 6,445 controls were included in the study, selected through 1:5 age and sex matching. The data spanned from January 1, 2004 to July 31, 2015. To compute the incidence rate of CHF in each group, the Kaplan-Meier method was employed. Additionally, Cox proportional-hazards regression analysis was utilized to estimate the hazard ratio of CHF.Results: CHF was present in 19 patients (1.47%) in the OPLL group and 71 patients (1.10%) in the control group. After accounting for age and sex, the hazard ratio for CHF in the OPLL group was 3.164 (95% confidence interval [CI], 1.867-5.360). When additionally considering income and underlying diseases, the hazard ratio for CHF within the OPLL group was 3.355 (95% CI, 1.977-5.694). All subgroups of OPLL patients exhibited an increased risk ratio for CHF across parameters such as sex, age, diabetes, hypertension, and dyslipidemia.Conclusion: According to this nationwide longitudinal study, an elevated incidence rate of CHF was associated with OPLL.
Biomechanical characterization of three iliac screw fixation techniques: A finite element study

<P><B>Abstract</B></P> <P>We aim to characterize the biomechanical properties of a modified iliac screw fixation method compared with the classic iliac screw fixation and the S2 alar iliac screw (S2AI) fixation using a FEM.</P> <P>A three-dimensional, non-linear FEM of lumbosacral spine and pelvis (L1-pelvis) was modified to simulate 3 different iliac screw fixations based on posterior screw fusion. The peak von Mises stress (PVMS) values of the iliac screws in the 3 different iliac screw fixations were recorded in during flexion/extension/axial rotation/lateral bending. The interaction stress which arose between the screw head and the shaft of iliac screws, was also measured for each case.</P> <P>The PVMS values of the 3 different iliac screw fixation techniques were lower than the fatigue strength levels under physiological loadings. PVMS of iliac screws was observed in the screw shaft for S2AI, in the screw neck for the modified iliac screw technique, and in the offset connectors of the classic iliac screw technique. The interaction between the screw head and the neck was compressed in modified iliac screw fixation technique. On the other hand, distraction force was observed in the S2AI technique between the screw head and the screw shaft.</P> <P>This FEM study supports our previous clinical results, which found that the modified iliac screw fixation technique can be an effective alternative sacropelvic fixation technique comparable to the classic iliac screw and the S2AI technique.</P> <P><B>Highlights</B></P> <P> <UL> <LI> We previously reported a modified iliac screw fixation technique which addressed the limitations of the classic iliac screw fixation and the S2 alar iliac screw (S2AI) fixation techniques. </LI> <LI> The PVMS values of the 3 different iliac screw fixation techniques were lower than the fatigue strength levels under physiological loadings. </LI> <LI> PVMS of iliac screws was observed in the screw shaft for S2AI, in the screw neck for the modified iliac screw technique, and in the offset connectors of the classic iliac screw technique. </LI> <LI> The interaction between the screw head and the neck was compressed in modified iliac screw fixation technique. PJS는 위장관의 다발성 과오종성 용종증과 피부 점막의 색소 침착을 특징으로 하며, 상염색체 우성으로 유전하는 드문 질환으로, 위장관 출혈이나 장폐색, 장중첩증 등의 합병증을 야기할 수 있다. PJS에서는 위장관 및 타 장기에서의 악성종양의 발생 빈도가 높으나, 위장관에서 보고된 종양의 경우 대부분 선암이며, PJS가 신경내분비종양과 동반된 경우는 아직 국내에 보고된 바 없다. 저자들은 PJS 환자에서 발생한 소장 기원의 소세포 신경내분비암종 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Peutz-Jeghers syndrome (PJS) is an autosomal dominant inherited disorder characterized by hamartomatous polyps in the gastro-intestinal tract and mucocutaneous melanin pigmentation. Hamartomas are not generally regarded as premalignant, although pa-tients with PJS are at increased risk for common and unusual types of gastrointestinal and non-gastrointestinal malignancies. However, most of the reported gastrointestinal malignancies have been adenocarcinomas, and few reports of an association of this syndrome with a neuroendocrine tumor (NET) have been published. Moreover, no case of this syndrome with NET has been reported in Korea. Here, we report a 21-year old male with PJS who had a small bowel neuroendocrine carcinoma. (Korean J Med 2013; 84:698-703)

Intelligent Transport Data Collection Strategy in Software Defined Edge Vehicular Network

Software Defined Network (SDN) and edge computing are emerging technologies in information and communication technology to minimize data collection latency on the Internet of Vehicles (IoV). The car has the potential to contribute to the development of smart cities by accessing urban data that are collected and reported to a remote data centers through wireless communication This paper presents the study on intelligent transport data collection strategy in a Software- Defined Edge vehicular networking. The two sensing data collection schemes are cooperative vehicular and edge prediction mode based Software Defined Vehicular Network (SDVN). We prove that urban data collection cost through a novel algorithm called car sensing platform for urban data collection algorithm to find an optimal strategy. We set up a simulation scenario based on realistic traffic and communication features and demonstrate the scalability of the proposed intelligent transport data collection strategy. It can be seen that the edge cell predictive trajectory decision-based SDEVN mode scheme reduces greatly the cost when the road seems to have a high number of the vehicle.
