        Surgical Outcomes of Cervical Myelopathy in Patients with Athetoid Cerebral Palsy: A 5-Year Follow-Up

        Study Design: Observational cohort study. Purpose: To assess the surgical outcomes of posterior decompression and fusion for cervical myelopathy in patients with athetoid cerebral palsy. Overview of Literature: Patients with athetoid cerebral palsy demonstrate involuntary movements and develop severe cervical spondylosis with kyphosis. In these patients, surgery is often performed at an early age because of myelopathy. A few studies have reported about the long-term outcomes of surgical treatment; however, they contain insufficient information. Methods: From 2003 to 2008, 13 patients with cervical myelopathy due to athetoid cerebral palsy underwent posterior fusion surgery and were included in this study. The Japanese Orthopaedic Association (JOA) score, neck disability index (NDI), C2–7 angle on radiography, and need for additional surgical treatment were examined at 1 and 5 years postoperatively. Results: The mean C2–7 angle was −10.5°±21.1° preoperatively and was corrected to −2.9°±13.5° immediately postoperatively. This improvement was maintained for 5 years. The JOA score was 9.5±2.5 preoperatively and 12.2±1.7 at the 5-year follow-up. NDI was 17±6.9 preoperatively and 16±7.5 at the 5-year follow-up. Patient satisfaction with surgery on a 100-point scale was 62.2±22.5 at the 5-year follow-up. Three patients needed additional surgery for loosening of screws. These results demonstrate good surgical outcomes for posterior fusion at 5 years. Conclusions: Posterior decompression and fusion should be considered a viable option for cervical myelopathy in patients with athetoid cerebral palsy.

      • The Risk of Hepatitis Infection in Tropical and Subtropical Countries : Foreign Sojourners Versus Local Residents 外國人滯留者 對 現地住民을 中心으로

        熱帶地域 또는 開發途上國에 滯留하는 先進國의 外國人 滯留者들은 肝炎感染의 높은 危險性을 지니고 있다. 日本海外協力駐在員들은 肝炎罹患率을 보면 1978年度에서 1982年度까지 20~35年齡群에서 年平均 3.6%였다. 이 罹患率은 같은 年齡群에 있는 「버마」靑年으로서 肝炎으로 入院한 admission rate보다 30培以上이 되는 셈이다. 海外駐在 美國宣敎師의 境遇도 비슷한 樣相을 띠고 있다. 肝炎罹患率은 東南亞細亞와 地中海沿岸國家에서 높았으나 오히려 「사하라」 南部의 「아프리카」地域國家에서는 낮았다. 地域沿岸國家中 Syria에서는 이 二年間 滯留中 肝炎罹患率이 31.8%로 나타나기도 하였다. 本 疫學的 硏究結果는 海外駐在員들의 肝炎豫防對策을 세움에 있어 有用한 資料가 될 것이다. Foreign sojourners from developed countries ran a high risk of hepatitis infection in tropical and/or developing countries. The incidence of hepatitis in the Japan Overseas Cooperation Volunteers, aged between 20 and 35 years, was 3.6 percent per year in 1978-1982. The incidence was more than 30 times higher than the admission rate of Burmemse youths of the same age group with hepatitis. Similar figures were obtained by the U.S. missionaries abroad. The incidence was high in Southeast Asian and peri-Mediterranean countries but was rather low in African countries south of the Sahara. The incidence of hepatitis in two years of assignment was as high as 32 percent of volunteers in some countries. some epidemiological data that would be useful for the planning of preventive measures were presented.

      • Effect of light on fruit body primordium formation of Ganoderma Iucidum on nutrient agar medium

        Phospholipase A_2(PLA_2)는 생체막의 주요 구성성분인 인지질의 sn-2위치에서 지방산 ester 결합을 가수분해하는 효소로 염증과 관련된 질병에 있어서 매우 중요한 역할을 하는 것으로 알려졌다. 본 연구에서는 항염증 치료제의 개발을 목표로 PLA_2의 특이적 저해물질을 탐색하던 중 Umbilicaria esculenta(石耳)의 메탄을 추출물로부터 PLA_2 저해활성을 갖는 두 화합물 G2, G3을 분리하였다. 활성물질의 분리는 U. esculenta의 메탄올 추출물을 에틸아세테이트로 추출하고 실리카 젤 컬럼, 크로로포름 추출, preparative TLC 등에 의하여 정제하였다. 각종 스펙트럼 분석 및 ^1H, ^13C, DEPT 등의 NMR 실험에 의하여 G2 화합물은 분자량 124, 분자식 C_7H_8O_2의 5-methyl-1,3-benzendiol(orcinol), G3 화합물은 분자량 182, 분자식 C_9H_10O_4의 2,4-dihydroxy-6-methyl-benzoic acid methyl ester(methyl orsellinic acid) 페놀성 화합물로 동정하였다. 본 화합물의 PLA_2 저해활성은 IC_50값이 G2의 경우 0.22 mM, G3의 경우 0.26 mM이었다. 그후 이 두화합물의 in vivo 염증모델에서 그 효능을 검토할 예정이다. Two depsides, medicinal herb products isolated from the methanol extract of Umbilicaria esculenta, inhibited human synovial fluid Phospholipase A_2 (PLA_2) (IC_50 of 0.22 and 0.26 mM, respectively). In the course of screening for antiinflammatory compounds from natural products, we successfully isolated two depsides PLA_2 inhibitory compounds, Orcinol nad methyl orsellinic acid. The compounds were indentified as orcinol and methyl orsellinic acid on the basis of various NMR studies including ^1H, ^13C and DEPT experiments.

      • Investigation of the Extracellular Matrix Effect for the QCM/CCD Cell Activity Monitoring System

        <P>A real-time quartz crystal microbalance (QCM) cell activity monitoring system coupled with micro CCD cameras was developed to investigate the cultured cell activity, which could measure the viscoelastic characteristics of the cell with the QCM and observe the cell morphology changes with CCD camera simultaneously. Both the viscoelastic characteristics and the shape of the cultured cell are important factors to estimate the cell activity and the cell adhesion. The extracellular matrix (ECM) on the surface of the QCM is essential to culture the cell stably in the QCM monitoring system. To find the ECM optimization condition, the adhesive strength of cultured cells on the ECM modified glass surface was measured by using rotating water stream and CCD camera. After culturing HepG2 cells for 24 hours on the ECM modified glass plates, the glass plates were dipped in the PBS solution and rotated with 1,000, 1,300, and 1,500 rpm for 30 seconds. The adhesiveness of ECMs was investigated by calculating the remained cells after rotating. Four types of ECM, such as amino group, carboxyl group, collagen monomer, and collagen polymer, were used and tested. The current paper improves the sensing system of previous report so that measurements of four ECMs can be simultaneously conducted under the same conditions in order to enhance reliability. A collagen polymer exposed ECM was the most stable on an adhesiveness point of view, but not suitable for the QCM cell activity monitoring due to the decrease of the QCM sensitivity. The sensitivity of the QCM cell activity monitoring system using collagen monomer as ECM is about 2.6 times better than that using collagen polymer. A collagen monomer exposed ECM was more stable than amino group and carboxyl group exposed ECMs based on an adhesiveness point of view. Therefore, a collagen monomer exposed ECM was the most stable and suitable for the QCM cell activity monitoring system among the four ECMs. The changes of the resonance frequency and the resonance resistance of the ECM film with the cultured cells were investigated and compared the results of CCD camera images. From these results, we showed the QCM cell activity monitoring system coupled with the micro CCD camera could be applied to the evaluation of the cell activities.</P>

        Microscopic detection of feactive oxygen species during ectomycorrhiza establishment between Rhizopogon roseolus (shoro) and Pinus thunbergii

        Detections of reactive oxygen species (ROS) during ectomycorrhiza establishment between Rhizopogon roseolus (shoro) and Pinus thunbergii were made microscopically using a nitro blue tetrazolium (NBT) staining. Roots of P. thunbergii were aseptically infected with R. roseolus mycelium by using a Petri dish technique. From 2- to 4-week period after inoculation, initial mycorrhizal formation could be observed. Lateral root tips were treated with NBT and then observed under a light microscope. Depositions of blue formazan indicating O2- accumulation were detected mainly hyphal cells contacting with the roots surface. Observations of transverse section of the root revealed that depositions of blue formazan were also detected at the plasma membranes of the epidermal cells where the fungal hyphae were adhesively contacted. In the non-inoculated P. thunbergii roots, depositions of formazan were observed in root hair cells but not in epidermal cells. From 4- to 8-week period after inoculation, dichotomous mycorrhizas and extraradical mycelia were clearly observed. A section from the mycorrhiza treated with NBT showed that root tissue was surrounded by fungal mantle sheath, in which highly intensive reaction with NBT was demonstrated. The reactive formazan complexes were apparent in Hartig net hyphae between epidermal and cortical cells of the root. After 16 weeks following inoculation, morphology of mycorrhizas became variable, viz., initial, dichotomous and browned mycorrhizas. The browned mycorrhizas were characterized by wrinkled surfaces and sparse extraradical mycelia. The browned mycorrhizas were collected and treated with NBT. A section from the specimen showed that depositions were slightly observed only in the part of extraradical mycelia. These results suggest that O2- generations from both fungus and plant are involved with the early establishment of ectomycorrhizas between R. roseolus and P. thunbergii.
