        Pancreatic pseudocysts usually develop as a complication of acute pancreatitis or repeated attacks of chronic pancreatitis. Pseudocysts may also develop as a result of traumatic injuries or neoplasms. An uncommon complication is massive hemorrhage inside the pancreatic pseudocyst. The exact mechanism of hemorrhage is unknown. Enzymatic digestion, pressure erosion from the cyst mass or a combination of these processes may play an important role. Gastrointestinal hemorrhage associated with a pancreatic pseudocyst can occur in 10 percent of cases. When hemorrhage occurs because of erosion into a blood vessel involved in the pseudocyst, surgery is indicated since the episodes of hemorrhage are often severe and may even be fatal. When there is colonic involvement, various complications may occur. The most dangerous one is spontaneous rupture into the colon, which requires immediate surgical treatment because of high incidence of fulminating sepsis or massive hemorrhage. We examined a sixty eight-year-old man complaining of hematochezia and dizziness, and reported that it was a case of massive lower gastrointestinal hemorrhage caused by pseudocyst of the pancreas ruptured into the colon associated with chronic pancreatitis. On operative findings, the peripancreatic fatty tissue had severe adhesions to transverse colon and tail portion of pancreas, where the pseudocyst had formed. A distal pancreatectomy, splenectomy and resection of transverse colon were performed. By microscopic examination, hemorrhagic necrosis and inflammatory change were observed in the pancreas specimen. The tail portion of pancreas had a 6×6 cm sized cystic mass and was proven to be a pseudocyst with no epithelial lining.

        The purpose of the study was to analyze the variables affecting farmers' adoption process of agricultural innovations in Korea. Specific objectives of the study were to ; (1) review the adoption process and the variables associated with this process, (2) investigate the rate of adoption of selected agricultural innovations in the fields of rice production, animal husbandary and horticulture, (3) analyze communication media and change agencies or institutions which affected the adoption of agricultural innovations, and (4) identify the relations of the farmers' adoption and communication exposure behavior. Data for the study were collected from 393 cluster sampled farmers out of selected 17 rural villages in Korea. Five graduate students and three faculty members of Seoul National University participated in conducting interview with pre-tested questionnairs. The data were analyzed by HP 3,000 computer in the College of Agriculture, Seoul National University. Major statistical techniques used for the study were frequency distribution, chi-square and discriminant analysis. Statistical significance was tested at .05 level. The major findings of the study were as follows : 1. Variables associated with the agricultural adoption process were classified into the following six categories ; (1) perceived characteristics of agricultural innovations, (2) personality and characteristics of adopters, (3) the patterns of communication media or channels, (4) efforts of change agents. (5) priorities in agricultural development policies, and (6) characteristics of social system. 2. Rats of adoption were about 83% in application of silicate fertilizers on rice seed bed, 41% in feeding calf starter known as artificial milk to calves, 56% vinyl mulching practices in vegetable cultivation, and 67% in spreading raw rice straw on paddy land to improve soil conditions. 3. Time lag between awareness and adoption appeared to be about seven months in application of silicate fertilizers on rice seed bed, and about a year in feeding calf starter. 4. Change agent interpersonal communication revealed to be the most important factor affecting farmers' awareness in application of silicate fertilizers on rice seed bed, in feeding calf starter, and in spreading raw rice straw on paddy land, while localite interpersonal communication in vinyl mulching practices in vegetable cultivation. 5. Change agent interpersonal communication appeared to be the most important factor affecting farmers' adoption in application of silicate fertilizers on rice seed bed and in feeding calf-starter, Localite interpersonal communications were more important in vinyl mulching practices of vegetable cultivation and in spreading raw rice straw. Localite interpersonal communications were more important factor at adoption stage than at awarenss stage. 6. Agricultural extension agencies appeared to be the most important factor affecting farmers' adoption in application of silicate fertilizers on rice seed bed, in feeding calf starter, and in vinyl mulching practices. Based on the results of this study, the following are recommended for further development of agricultural extension services and effective adoption process of agricultural innovation. 1. The variables associated with the agricultural innovation decision process should be taken into consideration to improve effectiveness of change agencies or agricultural extension services. 2. Effective use of communication media along with the feedback messages from the change agency as well as client system be considered to increase effectiveness and meaningfulness of both systems. 3. Innovative agricultural research be continually sharpen its focus on the highest priority needs of client system by utilizing feedback from farmers through the communication link of change agents. 4. A long-term educational approach by using various communication media be emphasized, and regular field survey on innovation-decision process will prevent discontinuance of innovations.

        Primary unruptured appendiceal mucocele is a rare disease. It is very difficult to diagnose preoperatively because of the rarity of classical symptoms and low incidence. Usually it is discovered incidentally during abdominal ultrasonography(US), computed tomography(CT), or appendectomy for acute appendicitis. If untreated, one type of mucoceles may rupture producing a potentially fatal entity known as pseudomyxoma peritonei, therefore preoperative diagnosis is very important. With the advent of colonoscopy, abdominal US and CT, it has been possible to preoperatively diagnose mucocele of appendix. We experienced a case of appendiceal mucocele in a 60 year-old woman who had symptoms of lower abdominal discomfort and right lower quadrant abdominal pain, and it was diagnosed preoperatively by colonoscopy, abdominal US and CT. So we present this case with review of literature.

        <P>Membrane proteins sense extracellular cues and transduce intracellular signaling to coordinate directionality and speed during cellular migration. They are often localized to specific regions, as with lipid rafts or tetraspanin-enriched microdomains; however, the dynamic interactions of tetraspanins with diverse receptors within tetraspanin-enriched microdomains on cellular surfaces remain largely unexplored. Here, we investigated effects of tetraspan(in) TM4SF5 (transmembrane 4 L6 family member 5)-enriched microdomains (T5ERMs) on the directionality of cell migration. Physical association of TM4SF5 with epidermal growth factor receptor (EGFR) and integrin alpha 5 was visualized by live fluorescence cross-correlation spectroscopy and higher-resolution microscopy at the leading edge of migratory cells, presumably forming TM4SF5-enriched microdomains. Whereas TM4SF5 and EGFR colocalized at themigrating leading region more than at the rear, TM4SF5 and integrin a5 colocalized evenly throughout cells. Cholesterol depletion and disruption in TM4SF5 post-translational modifications, including N-glycosylation and palmitoylation, altered TM4SF5 interactions and cellular localization, which led to less cellular migration speed and directionality in 2-or 3-dimensional conditions. TM4SF5 controlled directional cell migration and invasion, and importantly, these TM4SF5 functions were dependent on cholesterol, TM4SF5 post-translational modifications, and EGFR and integrin alpha 5 activity.         Parameters for successful pig islet isolation as determined using 68 specific-pathogen-free miniature pigs

        <P>Abstract: Background: </P><P>Islet cell transplantation is a novel therapeutic modality for the cure of diabetes. Pig islet cells are an attractive substitute for human islet cells; however, they are known to be particularly difficult to isolate because of a weak islet capsule and a tendency to be fragmented during enzymatic digestion. Therefore, parameters favoring successful pig islet isolation were investigated using specific-pathogen-free (SPF) miniature pigs.</P><P>Methods: </P><P>Sixty-eight SPF miniature pigs were used for islet isolation. Birth weight, body weight, age, sex, pregnancy history, and the fasting blood glucose levels of each pig were determined. Each pig’s general condition was assessed with regard to feeding status and physical activity. Pancreas procurement was performed by one surgical team. Anesthesia duration, operation duration, procedure quality, and perfusate type were recorded. After pancreatectomy, a biopsy was performed for islet density analysis. Decapsulation, cannulation duration, degree of distension, and cold ischemic time were assessed. During islet isolation, pancreas weight, digestion time, and digested tissue proportion were recorded. Isolation results were evaluated by total islet equivalents (IEQ), islet equivalents per gram of pancreas (IEQ/g), isolation index, islet recovery rate, purity, and visual grade. To identify the predictors of higher islet isolation yield, we performed binary logistic regression analysis with significant (P < 0.05) variables from the univariate analysis.</P><P>Results: </P><P>The pigs were categorized into high (n = 34) and low yield (n = 34) groups according to the median IEQ/g or total IEQ values. Body weight and age were significantly different between the two groups. Being male or a positive history of pregnancy in females was factors favoring successful islet isolation. General condition assessments failed to estimate islet isolation results. Long anesthesia duration, which might have caused ischemic injury to the pancreas, negatively affected islet isolation results. Decapsulation, cannulation duration, and subsequent pancreas distension were significantly important in successful islet isolation. Inter-lot variability of Liberase was not observed because of screening processes performed before purchase. Isolation index and islet recovery rate correlated well with islet yields.</P><P>Conclusions: </P><P>Multivariate analysis using total IEQ and IEQ/g as outcome variables indicated that age older than 2, being male and moderate distension by Liberase injection are major determinants influencing successful islet isolation.</P>

        Clinical Implications of Microsatellite Instability in Early Gastric Cancer

        Purpose: We aimed to evaluate the clinical characteristics of microsatellite instability in early gastric cancer. Materials and Methods: The microsatellite instability status of resected early gastric tumors was evaluated using two mononucleotide repeat markers (BAT25 and BAT26) and three dinucleotide repeat markers (D5S346, D2S123, and D17S250). Tumors with instability in two or more markers were defined as microsatellite instability-high (MSI-H) and others were classified as microsatellite stable (MSS). Results: Overall, 1,156 tumors were included in the analysis, with 85 (7.4%) classified as MSI-H compared with MSS tumors. For MSI-H tumors, there was a significant correlation with the female sex, older age, tumor location in the lower gastric body, intestinal histology, lymphovascular invasion (LVI), and submucosal invasion (P<0.05). There was also a trend toward an association with lymph node (LN) metastasis (P=0.056). In mucosal gastric cancer, there was no significant difference in MSI status in tumors with LN metastasis or tumors with LVI. In submucosal gastric cancer, LVI was more frequently observed in MSI-H than in MSS tumors (38.9% vs. 25.0%, P=0.027), but there was no difference in the presence of LN metastases. The prognosis of MSI-H tumors was similar to that of MSS tumors (log-rank test, P=0.797, the hazard ratio for MSI-H was adjusted by age, sex, pT stage, and the number of metastatic LNs, 0.932; 95% confidence interval, 0.423-2.054; P=0.861). Conclusions: MSI status was not useful in predicting prognosis in early gastric cancer. However, the frequent presence of LVI in early MSI-H gastric cancer may help guide the appropriate treatment for patients, such as endoscopic treatment or limited LN surgical dissection.

        슬러리점토의 압밀특성

        매립재로 이용되는 준설점토는 일반적으로 함수비가 매우 크며, 작은 압밀하중이 작용하더라도 큰 침하가 일어나는 특성을 지닌다. 이러한 초연약지반을 개량하는 공법설계를 위해서는 압밀특성을 구하는 실험이 실시되어야 한다. 그러나 일반적인 표준압밀시험방법으로는 슬러리상태의 흙이 압밀링의 벽면을 타고 상하부로 흘러나오기 때문에 실험이 불가능하다. 따라서, 본 연구에서는 Rowe cell 시험기를 사용하여 준설점토에 대한 압밀시험을 실시하였으며, 함수비의 변화가 압밀특성에 주는 영향을 검토하기 위해서 함수비를 100%, 110%, 120%, 133%, 150% 등 5종류로 변화시켜서 실험을 실시하였다. 실험결과로부터 준설매립지반의 개량을 위한 설계를 하는 경우, 실제 압밀시험을 하지 않고도, 현장 함수비가 측정되면 개략적인 압축지수와 압밀계수 및 변형률을 예측할 수 있는 식을 제안하였다. Dredged soil from sea has much higher water content than liquid limit of clay and even if small loads apply, it will suffer a great settlement. Therefore it is very difficult to perform a consolidation test with general consolidation apparatus because of high water content. In this study Rowe cell apparatus consolidation tests have been performed with 5 slurry clays of a water content of 100%, 110%, 120%, 133%, and 150%. From the test results the consolidation characteristics such as compression index, secondary compression index, consolidation coefficient, and strain have been investigated with a variation of water content of dredged soil. The equations to get consolidation constants such as a compression index, a coefficient of consolidation, and strain have been proposed with the field water content.
