Reliability and Validity of the Korean Version of Somatic Symptom Scale-8

Objective Somatic symptoms in psychiatry include underlying depression, anxiety, or other psychiatric disorders. This study aimed to conduct a validation study of a Korean version of the Somatic Symptom Scale-8 (K-SSS-8), and to utilize the K-SSS-8 effectively in clinical settings. Methods For reliabilty, test-retest reliability and internal consistency were analyzed. For construct validity, exploratory factor analysis(EFA) and confirmatory factor analysis (CFA) were conducted. Known-group validity was verified, Jonckheere-Terpstra test (J-T statistic) were used. Results Maternal Cronbach’s alpha was 0.85 and r value of test-retest reliability was 0.777. In the EFA, 2-, 3- and 4-factor model showed cumulative percentile for variance of 60% or more. In the CFA, the 3-factor model was found to be the most appropriated and simplest (χ2=10.992, df=17, CFI=1.000, TLI=1.022, RMSEA=0.000). The verifying the difference in K-SSS-8 also showed significant difference.(J-T statistic=-2.510, p<0.05). Conclusion K-SSS-8 can be useful for exploring symptoms such as panic symptoms, physical pain, and physiological symptoms experienced by patients in a short time. In addition, the K-SSS-8 is expected to be very useful for determining the current severity by using the severity categories and for establish additionally required assessment plans for depression and anxiety symptoms.

This paper deals with dynamic performance of the Cheju - Haenam HVDC system. The purpose of the study is to verify the control characteristics of the HVDC system and to analyze the dynamic performances by simulation study. Because the HVDC system consists of analogue and digital controller, the simulation is achieved with specification model incorporating nonlinear function. The dynamic performance simulations are performed by PSCAD/EMTDC.<br/>

Exfoliative cheilitis is a rare and chronic inflammatory condition affecting the vermilion of the lips and characterized by continuous peeling of the vermilion and production of a thick keratin scale. Conventional topical treatment of exfoliative cheilitis such as keratolytic lip balms, sunscreen, antifungal creams and topical steroids is helpful in some cases but it is not that effective in most cases. Conventional invasive treatment such as peeling with laser, cryosurgery and electrocautery is effective in some cases but it causes severe pain, discomforts, scarring and delayed wound healing. . In our case, a 42-year-old man was referred because of chronic thick hyperkeratotic plaques on both lips. It was resistant to conventional treatments. We treated him with pinhole method using Co2 laser. The lesion of the patient was almost improved without side effect and treatment effects lasted three to four months. This treatment was more effective with minimal side effects than conventional treatments. Clinicians should keep this treatment in mind while dealing with patients with treatment-resistant and chronic exfoliative cheilitis.

Background: Fractional CO<sub>2</sub> laser is widely used for the treatment of atrophic acne scar. Additionally, human stem cell conditioned media is anticipated to enhance the treatment effect and to reduce down time. Objectives: To analyze and objectively quantify how much the volume of the atrophic acne scar and skin pore improved in patients treated with fractional CO<sub>2</sub> laser and to analyze the effect of human stem cell conditioned media. Methods: This is a randomized left-right split-face study with fifteen subjects. Patients received single treatment of fractional CO<sub>2</sub> laser on both cheeks. After randomization, each patient was assigned to apply one side of the face with 100% human adipose-derived stem cell conditioned media and another side with normal saline. For the next 6 days, a solution containing 80% human adipose-derived stem cell conditioned media and hyaluronic acid was applied twice a day. To evaluate the effect of the treatment, we objectively measured volume and erythema by using Antera 3D CS at the 0, 2, 4, and 8 weeks. Results: It showed 15.0 to 23.5% improvement in scar volume and 15.9 to 37.6% improvement in skin pore volume with one treatment session of fractional CO<sub>2</sub> laser, both of which showed more improvement in the human stem cell conditioned media applied side. Conclusion: Fractional CO<sub>2</sub> laser in atrophic acne scar was estimated to be about 15% improvement at one treatment session and with human stem cell conditioned media, the clinical outcomes could be enhanced.
Two cases of keratoacanthoma associated B-RAF enzyme inhibitor (Vemurafenib)

Keratoacanthoma is a unique skin tumor which shows similar histopathologic features to SCC, but regresses voluntarily after rapid growth. Recently, there have been cases in which secondary cancer has been induced by a new anti-cancer drug. Vemurafenib is used for treatment of metastatic and unresectable melanomas with V600E mutations in the BRAF gene. BRAF is a component of the mitogen-activated protein kinase (MAPK) pathway, leading to RAS-RAF-MEK-ERK which is involved in cell proliferation, survival, and differentiation. Mutations in the BRAF gene cause a permanent activation of B-RAF and by extension, of the MAPK pathway, which results in uncontrolled growth and proliferation of cells. Vemurafenib, small-molecule inhibitor of BRAF(V600E) kinase, cause an inhibition of an over-activated MAPK signaling pathway downstream in BRAF kinase-expressing tumor cells. Proliferation of keratinocytes is common when using Vemurafenib, ranging from benign verrucous lesions to malignant. It usually occurs between the 7th and 8th weeks of treatment. In our two cases, a 58-year-old man and a 66-year-old woman, who had metastatic melanoma, were referred because of keratotic papules and nodules on face and neck. Both patients were treated with Vemurafenib for two and three months. We performed skin biopsy and the skin lesions of the both patients were diagnosed as keratoacanthoma. We report two cases of B-RAF enzyme inhibitor associated keratoacanthoma.
