OBJECTIVES: Low prevalence and prognostic relevance of KRAS mutations in Korean pancreatic ductal adenocarcinomas (PDACs) need to be validated with sensitive detection method. METHODS: Peptide nucleic acid (PNA)–mediated polymerase chain reaction (PCR) clamping was used to precisely detect KRAS mutation in 72 paraffinized tumor samples and was validated by pancreatic cell lines to compare the efficiency of direct sequencing. RESULTS: The PNA-mediated PCR clamping detected mutant allele proportions of as low as 0.5% against a background of wild-type DNA and was 20-fold more sensitive than direct sequencing through the validation of pancreatic cell lines. Peptide nucleic acid–mediated PCR clamping detected KRAS mutations in 47.2% of 72 PDACs. Low tumor cellularity and low PCR amplification efficiency led to be undetected or failed by direct sequencing in pancreatic paraffinized samples.KRAS mutations were an independent worse prognostic factor predicting a reduced progression-free survival rate in the postoperative chemotherapy group. CONCLUSIONS: Peptide nucleic acid clamp real-time PCR was a sensitive method for detecting KRAS status in paraffinized PDAC samples. We identified a low KRAS mutation rate among the Korean PDAC patients using PNA clamp real-time PCR, potentially implicating epidemiological characteristics. The low KRAS mutation rate and its prognostic role may suggest the further survival benefit in Korean PDAC patients.

Objective: To assess the efficacy and safety of non-surgical management of heterotopic interstitial pregnancy. Methods: The medical records of 6 women diagnosed with heterotopic interstitial pregnancy who had received non-surgical treatment at the CHA Bundang Medical Center unit between January 2007 and December 2017 were analyzed. Three cases of heterotopic interstitial pregnancies were treated with ultrasound-guided local potassium chloride (KCl) injections. Two cases were managed expectantly. One case of heterotopic quadruplet pregnancy with twin pregnancy with a left interstitial and a tubal pregnancy was treated with an ultrasound-guided local KCl injection and laparoscopic left salpingectomy. Treatment-related complications and obstetric outcomes were measured. Results: Three cases were successfully treated with ultrasound-guided local KCl injection and the intrauterine pregnancies in all 3 cases successfully progressed to term without any complications and resulted in the birth of 3 healthy babies delivered vaginally. One case that was treated expectantly was successfully delivered at full term, whereas the other case resulted in a spontaneous abortion. Quadruplet heterotopic pregnancy was successfully managed with an ultrasound-guided local KCl injection and laparoscopic salpingectomy. The intrauterine twin pregnancy successfully progressed to term, with the birth of healthy babies delivered via an elective cesarean section. at 37+0 weeks of gestation. Conclusion: Non-surgical management with local KCl injection under ultrasound guidance could be a safer and more effective treatment option than surgical management for hemodynamically stable patients with fetal cardiac activity in the interstitial pregnancy. Expectant management could be an option for the patients with the absence of fetal cardiac activity.

실외 실험적 온난화 및 강수 처리에 따른 소나무와 낙엽송 유묘의 초기 생장 특성

본 연구는 기후변화와 관련하여 양묘과정에서 온도와 강수 변화에 따른 주요 침엽수의 생존 및 생장 특성 변화를 알아보고자 수행하였다. 소나무와 낙엽송 노지묘(1-0)를 대상으로 대조구 기준 3℃의 온도를 상승시키거나 ±40%의 강수를 조절한 6처리 생육환경[온도 2처리(대조: TC, 증가: TW) × 강수 3처리(대조: PC, 감소: PD, 증가: PI)] 실외 실험구를 조성하였으며, 생존율, 근원경, 묘고, 물질생산량 및 묘목품질지수 변화를 조사하여 이원분산분석을 수행하였다. 소나무는 온도와 강수 처리에 따른 생존율 차이가 없었지만, 강수가 증가할수록 묘목품질지수가 낮아지는 경향을 보였다. 낙엽송은 온도상승과 강수 감소에 따라 고사율이 증가하였으며, 묘목품질지수는 두 요인 간 상호작용을 보이면서 온도대조-강수증가 처리구에서 가장 낮게 나타났다. 따라서 양묘과정에서 소나무는 강수 증가, 낙엽송은 온도 증가 또는 강수 감소에서 낮은 묘목 생산량과 품질이 예상되며, 기후변화에 따른 두 수종의 특이적 민감도를 확인할 수 있었다. 향후 지구온난화와 가뭄·폭우 등의 강수 변화에 의해 수종별 묘목 생존과 품질의 변화가 예상되기 때문에 각 수종의 생육 반응 특성에 따른 적합한 양묘시업 대응 전략이 필요할 것으로 판단된다. This study aimed to investigate the effects of climate change on the survival and growth performance of Pinus densiflora and Larix kaempferi seedlings using open-field experimental warming and precipitation manipulation. We measured the survival rate, root-collar diameter, and height, and then calculated the seedling quality index (SQI) of 2-year-old seedlings under 6 treatments [2 temperatures (TC: Control; TW: Warming) × 3 precipitation manipulations (PC: Control; PD: Decreased; PI: Increased)] and performed a two-way ANOVA to test for differences.The air temperature of the warming plots was 3°C higher than that of the control plots, while the precipitation manipulation plots received ±40% of the precipitation received by the control plots. Temperature and precipitation treatments did not significantly affect the survival rate of P. densiflora; however, the SQI of P. densifloradecreased with increasing precipitation. In contrast, the mortality rate of L. kaempferi increased with increasing temperature and decreasing precipitation. Furthermore, in L. kaempferi, TC × PI treatment resulted in the lowest SQI with a significant interaction effect observed between the two factors. In summary, low seedling production and quality should be expected in P. densiflora as precipitation increases and in L. kaempferi as temperature increases or precipitation decreases. These results indicate species-specific sensitivities to climate change of two plant species at the nursery stage. With the occurrence of global warming, the frequencies of drought and heavy rainfall events are increased, and this could affect the survival and seedling quality of tree species. Therefore, it is necessary to improve nursery techniques by establishing new adaptation strategies based on species-specific growth performance responses. 1

The purpose of this study is to understand the daily lives of adolescents during the COVID-19 pandemic and to provide educational implications for enhancing the competencies of adolescents living in rapidly-changing environments. To this end, the photovoice method, consisting of orientation, documentation, discussion, and analysis, was employed to study nine adolescent participants, who were interested in sharing aspects of their daily lives. The results yielded four themes and nine sub-themes. The first theme is “home life”, which is composed of two sub-themes: most comfortable to be alone and EA (eating alone) for lunch. The second theme is “leisure life”, which consists of two sub-themes: the virtual world of playing with friends and exercise is the only way out to breathe. The third theme is “school life”, which consists of three sub-themes: time for inner exploration and reflection, cracks in daily life due to excessive autonomy, and pros and cons of virtual classes. The fourth theme is “the voice of adolescents”, which consists of two sub-themes: requiring adults to set a golden example and anxious voices due to an uncertain future. This photovoice method of study is meaningful in that it explores the daily lives of adolescents during the COVID-19 pandemic and provides valuable educational implications.

Background: Korean standard series (KSS) have not changed for 20 years. Moreover, the Korea Food and Drug Administration has suspended import licensing of Cosmetic series (Chemotechnique<sup>Ⓡ</sup>). Objective: To analyze trends in the positive rates of allergens in patch test using KSS and present a literature review on the comparison of the results those of European baseline series (EBS) and North American Contact Dermatitis Group (NACDG) screening series. Methods: Epidemiologic findings and the positive rate of KSS allergens were analyzed from patients who visited the Department of Dermatology in Ewha Womans University Mokdong Hospital between 1998 and 2018. Literature reviews were conducted on the comparison of the results with those of the 2015∼2016 NACDG, 2013∼2014 ESSCA, and standard antigens in K-camp. Results: The positive rates of KSS allergens from 126 patients in 2018 were compared with those from 184 patients during 1998 to 2007. The top 3 allergens in the two study periods were common; nickel sulfate, cobalt chloride, and potassium dichromate. Remarkably, the positive rates for balsam of Peru and paraben mix increased from 7.1% to 17.5% and from 2.7% to 7.9%, respectively. As compared with the results of NACDG standard series and EBS, KSS lacked fragrance and cosmetic series of allergens, especially fragrance mix II, methyldibromoglutaronitrile, and methylisothiazolinone, and epoxy series of allergens. Conclusion: Our results, especially the increasing trends of balsam of Peru and paraben mix, would be further confirmed in a multicenter study. Additionally, in view of the increase in fragrance and preservative allergens, KSS would need the addition of related allergens. (Korean J Dermatol 2020;58(6):371∼381)

This study examines the relationship between basic psychological needs, exercise outcome expectation, and exercise adherence intention in COVID-19 environment, and verifies the mediating effect of exercise outcome expectation. For this purpose, self-administration method was applied to 267 exercise participants for more than 3 months since COVID-19, and collected data were analyzed by IBM SPSS Statistics 26.0 and AMOS 21.0 according to the purpose of this study. The result is as follows. Competence was significant between men and women. In terms of age, physical exercise outcome expectation was statistically significant. According to the type of exercise participation(online, offline, online and offline), competence and social exercise outcome expectation were significant. The mediating effect of exercise outcome expectation was verified in the relationship between the three factors (autonomy, competance, and relatedness) of basic psychological needs. Offline exercise participation had a higher level of satisfaction than online participation in terms of competence, relatedness, and social exercise outcome expectation. Nevertheless, the online exercise program partially maintained the level of participants basic psychological needs or exercise outcome expectation. If the online exercise program is properly organized to maintain basic psychological needs and exercise outcome expectation, participants can continue exercise without drop-out and be less affected by environmental change.
