        Seaweed polysaccharides-based gel to contest foot odour

        This study demonstrated a one-pot process for the preparation of seaweed-based antibacterial foot care gel for the removal of odor. Foot gel comprised 97.5% water, 1.2% seaweed polymer, 0.2% bioactive molecule, and 1.1% preservative. The antibacterial efficiency of foot gel was tested against 14 bacterial strains, isolated from foot swab samples from five different healthy volunteers having foul foot odor. The 16S rRNA partial gene sequencing identification revealed all 14 bacterial strains were different Bacillus species. Foot gel was characterized using FTIR, Rheology, TGA, and gelling properties. The volunteer's feet were treated with just two drops (around 200 L) of foot care gel before wearing socks, which showed restriction in the growth of bacterial strains causing a foul odor within a few hours, therefore resulting in restrained the foul foot odor. Furthermore, the volunteer’s remarks about feeling freshness, coolness, dryness, and smooth feet after applying the foot gel were very encouraging. The shelf life of foot gel was monitored for up to six months and during that time, no significant change in rheological characteristics and antibacterial efficiency was observed. Foot gel was compared with commercially available foot care products and foul odor removal efficiency was found identical in both. Results of this study showed that the odor-removing efficacy of developed foot gel was found better than other commercial products.

The mechanism involved domino catalysis type approach with consumption and regeneration of catalyst in two catalytic cycles. This method proved to be very lucrative and gives very good yield. The method described here represents an environmentally benign alternative to classical approach.

        Morphotectonic analysis of Sheer Khadd River basin using geospatial tools

        In the present study, a quantitative morphotectonic analysis of Sheer Khadd River basin has been carried out based on geomorphic and morphometric indices such as hypsometric integral, drainage basin asymmetry, mountain front sinuosity, basin elongation ratio, valley floor width to valley height ratio, river sinuosity and stream length gradient index using ASTER digital elevation model (DEM) and Google Earth images to understand the morphotectonics of the basin. The results indicate that Sheer Khadd River basin is tilting towards east and is elongated in shape due to active faulting and folding activity in the terrain. A moderate hypsometric integral value indicates that the basin is still under mature stage of erosion and reflecting a complexity in topography. Fluctuations in stream length gradient index over fault zones indicate irregularities in the drainage course due to the presence of fluvial knick points. The results of morphotectonic and morphometric analysis using DEM data is useful tool for morphotectonic evaluation of any complex terrain.

        Complete Bony Segment Flap Failure without Skin Paddle Compromise in Fibular Free Flap: A Case Report of a Rare Early Complication

        The free fibula flap is sometimes associated with partial flap loss. A 62-year-old man with squamous carcinoma of the lower alveolus underwent anterior arch resection and reconstruction with a fibula osteocutaneous flap containing three bony segments. There were two perforators in the skin flap; the first perforator was proximal to the bony segments and the other perforator was at the level of the middle segment. Atherosclerosis was observed in the anastomotic vessels. During surgery, thrombosis occurred in the arterial anastomosis, but was revised before the operation was over. The patient’s early postoperative course was uneventful. In the second week, purulent discharge was observed from the drains. Flap exploration showed devascularization of bony segments and periosteum, but the skin paddle was viable. The peroneal vessel had thrombosed distal to the takeoff of the proximal perforator and the viable skin paddle perfused by proximal perforators falsely indicated the well-being of underlying bones. The de-vascularized bones and sloughed periosteum were removed at the time of exploration, after which the skin paddle was reoriented for coverage of the defect. The patient completed chemoradiation without any bony reconstruction afterwards. This is the only reported case of early total bony flap portion loss with a viable skin paddle in free fibula flap transfer.

        Evaluation of Direct Anterior Approach for Revision Total Hip Arthroplasty: A Systematic Review

        The direct anterior approach (DAA) is an established approach for total hip arthroplasty (THA) but has been sparingly tried for revisions. The purpose of this study was to examine the available literature in order to consolidate information available on revision THA using the DAA. A PubMed, Embase, and Scopus search was performed using relevant keywords. Studies reporting on patients undergoing revision THA using DAA were included for analysis. In a review of the literature, nine studies matched the pre-decided inclusion criteria with 319 hip joints undergoing revision THA. Mean follow-up of all included studies was 34 months. The indications of revision after primary THA in decreasing order were aseptic loosening (53%), prosthetic joint infection (20.7%), peri-prosthetic fracture (16.9%), dislocation (7.2%), psoas impingement (1.9%), polyethylene wear (1.2%), pain (0.6%), and instability (0.3%). Of the 319 revisions evaluated, 107 underwent a stem revision, 142 underwent cup revision, 49 underwent a combined revision, and 21 underwent isolated liner/head change. A statistically significant improvement in functional score (P<0.05) was observed for all studies reporting on functional outcomes. A low complication rate (51/319, 16.0%), which includes dislocation (12), infection (12), loosening of the acetabular shell (5), peri-prosthetic fractures (6), haematoma (4), and transient nerve palsy (6), was reported. Based on available level Ⅲ-IV evidence, DAA appears to be a reliable alternative for revision of the failed hip arthroplasty with acceptable complication rates. Evidence of a higher quality is needed to further characterize its role in revision scenarios.

        Challenges in Microsurgical Free Tissue Transfer for a 1-Year-Old Child

        Challenges faced during complex reconstructions in the pediatric age group are rarely discussed in literature. This study is to share our experience in a 1-year-old patient. The patient had an exposed knee joint following trivial trauma, which was resurfaced with a free latissimus dorsi flap. On the patient’s 5th day in the ward, the flap was discolored and inadequate perfusion was noted. The patient also had loose stools overnight. Before she was re-examined, the dressings were loosened and the patient was kept warm and was started on intravenous fluids. When examined in the operating room, the vascularity of the flap had improved without any intervention. The monitoring skin paddle necrosed while the muscle survived. The patient then underwent skin grafting and had uneventful recovery. The postoperative monitoring in children is critical as they have poor reserves and are prone to complications. We believe pediatric microsurgery should be done in specialized microsurgical centers that also have good intensive care units.

        Wheat Drilling Directly in Combine Harvested Paddy Fields: a Comparative Input-Output Energy Analysis

        Purpose Agriculture’s usage of energy is inextricably connected to technological advancement and mechanization. In this study, the energy analysis of the M1, M2, and M3 wheat-growing methods in combine harvested paddy fields was computed. Method In M1, farmer’s practice (sowing of wheat after burning of paddy straw) was followed but in M2 and M3, a resource conservation technology (sowing of wheat with the Happy Seeder machine without burning of paddy straw) was used for direct drilling of wheat. Results The results indicate thatM1 had maximuminput energy consumption and output energy (23,862.63 and 123,182 MJ ha−1) than M2 andM3, whereas net energy gain was maximum inM3. The share of chemical and diesel energy was maximum in all the methods. The share of direct and non-renewable energy input sources was more than indirect and renewable energy input sources. Estimation of greenhouse gas emission was maximum in M1 followed by M2 and M3 and was mostly dependent on diesel fuel having the highest share among all inputs. The maximum energy use efficiency, energy efficiency ratio, energy productivity, and energy intensiveness, and minimum specific energy were in M3 followed by M2 and M1, which showed that M3 was the most efficient and clean method. Conclusion The conducted study is helpful for researchers/policymakers in framing and developing new agriculture policies and technologies on an energy consumption basis and also provide a feasible alternative to avoid paddy straw burning, declining the environmental pollution.
