        ToleRability of BevacizUmab in elderly Ovarian cancer patients (TURBO study): a case-control study of a real-life experience

        Objective: Bevacizumab maintenance following platinum-based chemotherapy is an effective treatment for epithelial ovarian cancer (EOC), both in primary and recurrent disease. Our aim was to identify criteria to select elderly patients who can safely benefit from bevacizumab addition. Methods: This is a case-control study on patients with primary or recurrent EOC who received platinum-based chemotherapy plus bevacizumab, between January 2015 and December 2016. Patient characteristics, treatment details and adverse events were reviewed and analyzed in 2 settings: younger (<65 years, group 1) and elderly (≥65 years, group 2). A binary logistic model was applied to correlate clinical variables and severe (grade ≥3) toxicity risk. Results: Overall, 283 patients with EOC were included, with 72 (25.4%) older patients compared with 211 (74.6%) younger women. Bevacizumab had been administered to 234 patients (82.7%) as first-line treatment and in 49 (17.3%) with recurrent disease. At diagnosis, elderly patients presented with at least one comorbidity and were taking at least 1 medication in 84.7% and 80.6% of the cases respectively, compared with correspondingly 47.4% and 37.4% in group 1 (p<0.001). Nonetheless, the occurrence of serious (grade ≥3) adverse events did not increase among the older group. Creatinine serum levels >1.1 g/dL, estimated glomerular filtration rate (eGFR) ≤60 mL/min, ≥3 comorbidities were independently associated with a higher severe toxicity. Conclusions: Elderly patients with EOC can safely be treated with bevacizumab; factors other than age, as higher creatinine serum levels, eGFR and number of comorbidities should be considered to better estimate bevacizumab-related toxicity risk.

        Probabilistic analysis of a partially-restrained steel-concrete composite frame

        The paper investigates the seismic performance of a Partially-Restrained (PR) steel-concrete composite frame using the probabilistic approach. The analysed frame was tested at the ELSA laboratory of the Joint Research Centre of Ispra (Italy), while the representative beam-to-column composite connections were tested at the Universities of Pisa, Milan and Trento (Italy). The component modelling of both interior and exterior composite joints is described first, including the experimental-numerical validation. The Latin Hypercube method has been used to draw the probabilistic distribution curves of joints, and then the whole PR composite frame has been analysed. Pushover and incremental dynamic analyses have been carried out using the non-linear FE code SAP2000 version 9.1. The fragility and performance curves of the PR composite frame have been determined for four damage limit states.

        FE assessment of dissipative devices for the blast mitigation of glazing façades supported by prestressed cables

        The paper focuses on the dynamic response of a blast-invested glass-steel curtain wall supported by single-way pretensioned cables. In order to mitigate the critical components of the façade from severe structural damage, an innovative system able to absorb and dissipate part of the blast-induced stresses in the critical façade components is proposed. To improve the blast reliability of the studied glazing system, specifically, rigid-plastic and elastoplastic devices are introduced at the base and at the top of the vertical bearing cables. Several combinations and mechanical calibrations of these devices are numerically investigated and the most structurally and economically advantageous solution is identified. In conclusion, a simple analytical formulation totally derived from energetic considerations is also suggested for a preliminary estimation of the maximum dynamic effects in single-way cable-supported façades subjected to high-level blast loads.

        Analysis of rigid and semi-rigid steel-concrete composite joints under monotonic loading Part II: Parametric study and comparison with the Eurocode 4 proposal

        This paper analyses the response of rigid and semi-rigid steel-concrete composite joints under monotonic loading. The influence of some important parameters, such as the presence of column web stiffening and the mechanical properties of component materials, is investigated by using a three-dimensional finite element modelling based on the Abaqus code. Numerical and experimental responses of different types of composite joints are also compared with the analytical results obtained using the component approach proposed by Eurocode 4. The results obtained with this approach generally fit well with the numerical and experimental values in terms of strength. Conversely, some significant limits arise when evaluating initial stiffness and non-linear behaviour of the composite joint.

        The paper concerns the modelling of rigid and semi-rigid steel-concrete composite joints under monotonic loading through use of the Abaqus program, a widespread finite element code. By comparing numerical and experimental results obtained on cruciform tests, it is shown that the proposed modelling allows a good fit of the global joint response in terms of moment-rotation law. Even the local response in terms of stresses and strains is adequately predicted. Hence, this numerical approach may represent a useful tool for attaining a better understanding of experimental results. It may also be used to perform parametric analyses and to calibrate simplified mechanical models for practical applications.

        Influence of viscous phenomena on steel-concrete composite beams with normal or high performance slab

        The aim of the paper is to present some results about the influence of rheological phenomena on steel-concrete composite beams. Both the cases of slab with normal and high performance concrete for one and two-span beams are analysed. A new finite element model that allows taking into account creep, shrinkage and cracking in tensile zones for concrete, along with non-linear behaviour of connection, steel beam and reinforcement, has been used. The main parameters that affect the response of the composite beam under the service load are highlighted. The influence of shrinkage on the slip over the supports is analysed, together with the cracking along the beam. At last, by performing a collapse analysis after a long-term analysis, the influence of rheological phenomena on the ductility demand of connection and reinforcement is analysed.

        Experimental Approach to Explosive Nucloeosynthesis with RI Beams

        Experimental efforts to investigate stellar reactions under high-temperature and high density conditions have been made as a major program using the RI beams from the Center for Nuclear Study (CNS) low-energy in-flight RI beam separator (CRIB) at University of Tokyo in order to understand the evolution of the universe, as well as various stellar phenomena. Specically, two subjects of hydrogen burning are discussed here. One is a reaction study of the pp-chain and the second is of the explosive hydrogen burning, the rp-process. Some s-wave resonances have been identified by the thick target method in the crucial reaction processes in the hydrogen burning. The resonant scattering with the thick target method also succeeded in identifying inelastic resonant scattering, giving proton widths for the first excited state of the target nucleus. This provided very efficiently the reaction rate estimate for the process under high-temperature equilibrium conditions. Possibilities of the CRIB facility in near future are also briey discussed.         Acinetobacter Prosthetic Joint Infection Treated with Debridement and High-Dose Tigecycline

        Prosthesis retention is not recommended for multidrug-resistant Acinetobacter prosthetic joint infection due to its high failure rate. Nevertheless, replacing the prosthesis implies high morbidity and prolonged hospitalization. Although tigecycline is not approved for the treatment of prosthetic joint infection due to multidrug resistant Acinetobacter baumannii, its appropriate use may preclude prosthesis exchange. Since the area under the curve divided by the minimum inhibitory concentration is the best pharmacodynamic predictor of its efficacy, we used tigecycline at high dose, in order to optimize its efficacy and achieve implant retention in 3 patients who refused prosthesis exchange. All patients with prosthetic joint infections treated at our Institution are prospectively registered in a database. Three patients with early prosthetic joint infection of total hip arthroplasty due to multidrug resistant A. baumannii were treated with debridement, antibiotics and implant retention, using a high maintenance dose of tigecycline (100 mg every 12 hours). The cases were retrospectively reviewed. All patients signed informed consent for receiving off-label use of tigecycline. Tigecycline was well tolerated, allowing its administration at high maintenance dose for a median of 40 days (range 30–60). Two patients were then switched to minocycline at standard doses for a median of 3.3 months in order to complete treatment. Currently, none of the patients showed relapse. Increasing the dose of tigecycline could be considered as a means to better attain pharmacodynamic targets in patients with severe or difficult-to-treat infections. Tigecycline at high maintenance dose might be useful when retention of the implant is attempted for treatment for prosthetic joint infections due to multidrug resistant Acinetobacter. Although this approach might be promising, off-label use of tigecycline should be interpreted cautiously until prospective data are available. Tigecycline is probably under-dosed for the treatment of implant and biofilm associated infections.

        Whole-genome sequencing analysis of Shiga toxin-producing Escherichia coli O22:H8 isolated from cattle prediction pathogenesis and colonization factors and position in STEC universe phylogeny

        Shiga toxin-producing Escherichia coli (STEC) is a foodborne pathogen capable of causing illness in humans. In a previous study, our group showed that a STEC isolate belonging to O22:H8 serotype (strain 154) can interfere with STEC O157:H7 colonization both in vitro and in vivo. Using whole-genome sequencing and genomic comparative, we predicted a subset of genes acquired by O22:H8 strain 154 through horizontal gene transfer that might be responsible for the phenotype previously described by our group. Among them were identified genes related to the pathogenesis of non-LEE (locus of enterocyte effacement) STEC, specific metabolic processes, antibiotic resistance and genes encoding for the T6SS-1 that is related to inter-bacterial competition. In addition, we showed that this strain carries stx1c and stx2dact, a mucus-inducible variant. The results obtained in this study provide insights into STEC genomic plasticity and the importance of genomic islands in the adaptation and pathogenesis of this pathogen.

        Single nucleotide polymorphisms in candidate genes associated with milk yield in Argentinean Holstein and Holstein x Jersey cows

        Background: Research on loci influencing milk production traits of dairy cattle is one of the main topics of investigation in livestock. Many genomic regions and polymorphisms associated with dairy production have been reported worldwide. In this context, the purpose of this study was to identify candidate loci associated with milk yield in Argentinean dairy cattle. A database of candidate genes and single nucleotide polymorphisms (SNPs) for milk production and composition was developed. Thirty-nine SNPs belonging to 22 candidate genes were genotyped on 1643 animals (Holstein and Holstein x Jersey). The genotypes obtained were subjected to association studies considering the whole population and discriminating the population by Holstein breed percentage. Phenotypic data consisted of milk production values recorded during the first lactation of 1156 Holstein and 462 Holstein x Jersey cows from 18 dairy farms located in the central dairy area of Argentina. From these records, 305-day cumulative milk production values were predicted. Results: Eight SNPs (rs43375517, rs29004488, rs132812135, rs137651874, rs109191047, rs135164815, rs43706485, and rs41255693), located on six Bos taurus autosomes (BTA4, BTA6, BTA19, BTA20, BTA22, and BTA26), showed suggestive associations with 305-day cumulative milk production (under Benjamini-Hochberg procedure with a false discovery rate of 0.1). Two of those SNPs (rs43375517 and rs135164815) were significantly associated with milk production (Bonferroni adjusted p-values < 0.05) when considering the Holstein population. Conclusions: The results obtained are consistent with previously reported associations in other Holstein populations. Furthermore, the SNPs found to influence bovine milk production in this studymay be used as possible candidate SNPs for marker-assisted selection programs in Argentinean dairy cattle.
