Predictors for margin of resection >4 mm in the management of periocular basal cell carcinoma

Purpose: To determine the margin of resection (MOR) for periocular basal cell carcinoma (BCC) and compare the outcomes of BCC treatment, namely Mohs micrographic surgery (MMS) and wide excision with later reconstruction (WELR). Methods: This is a retrospective, comparative, interventional study of patients who underwent surgical treatment of periocular BCC. One hundred forty-two patients were included. One hundred patients were treated with MMS and 42 with WELR. Inclusion criteria were primary periocular BCC with postoperative follow-up of ≥6 months, age more than 18-year-old. Exclusion criteria were, orbital extension, BCC origin outside the periocular area, or those associated with Gorling or nevoid BCC. The main outcome measure was variables associates with MOR >4 mm. Results: There was a positive correlation between the preoperative tumor horizontal and vertical diameter with the corresponding MOR, of 0.27 (p = 0.01) and 0.28 (p = 0.007), respectively. Receiver operating characteristics suggest that a tumor with a horizontal diameter ≥5 mm or a vertical diameter of ≥6 mm, might need MOR >4 mm. One patient in the MMS group had BCC recurrence compared to none in the WELR group, and one patient in the WELR had a positive surgical margin, which was cleared during the reconstruction. Conclusions: BCC tumor margins may extend far beyond clinical margins and the MOR required is often more than 3-4 mm. MMS ensures clear tumor margins but is not practical for all patients. A stratification system could help divide patients between the treatment strategies.

Saccharothrix algeriensis NRRL B-24137 produces naturally different dithiolopyrrolone derivatives. The enzymatic activity of pyrrothine N-acyltransferase was determined to be responsible for the transfer of an acyl group from acyl-CoA to pyrrothine core. This activity was also reported to be responsible for the diversity of the dithiolopyrrolone derivatives. Based on this fact, nine dithiolopyrrolone derivatives were produced in vitro via the crude extract of Sa. algeriensis. Three of them have never been obtained before by natural fermentation: acetoacetyl-pyrrothine, hydroxybutyryl-pyrrothine, and dimethyl thiolutin (holomycin). Two acyltransferase activities, acetyltransferase and benzoyltransferase catalyzing the incorporation of linear and cyclic acyl groups to the pyrrothine core, respectively, were biochemically characterized in this crude extract. The first one is responsible for formation of acetyl-pyrrothine and the second for benzoyl-pyrrothine. Both enzymes were sensitive to temperature changes: For example, the loss of acetyltransferase and benzoyltransferase activity was 53% and 80% respectively after pre-incubation of crude extract for 60 min at 20°C. The two enzymes were more active in neutral and basal media (pH 7-10) than in the acidic one (pH 3-6). The optimum temperature and pH of acetyltransferase were 40°C and 7, with a Km value of 7.9 μM and a Vmax of 0.63 μM/min when acetyl-CoA was used as limited substrate. Benzoyltransferase had a temperature and a pH optimum at 55°C and 9, a Km value of 14.7 μM, and a Vmax of 0.67 μM/min when benzoyl- CoA was used as limited substrate.

A 24-year-old man presented to King Saud Medical City emergency department with a retained bullet in his penis following a civilian exchange of gunfire. After an initial assessment, the patient was taken to the operating room. Penile exploration was performed, the bullet was extracted successfully, and the corpora cavernosa were repaired properly. A 6-week follow-up showed full healing with preservation of erectile function. Immediate surgical intervention is mandatory as the primary treatment for penile gunshot injury to ensure proper anatomical and functional recovery.

The aim of this work is to introduce for the first time the concept of D-set. This is done by defining a special type of cover called a D-cover. we present some results to study the properties of D-compact spaces and their relations with other topological spaces. Several examples are discussed to illustrate and support our main results. Our results extend and generalized many will known results in the literature.

The effect of modifiers on the optical features and radiation defying ability of the Eu3þ ions doped multi constituent glasses was examined. XRD has established the amorphous nature of the specimen. The presence of various functional/fundamental groups in the present glasses was analyzed through FTIR spectra. The physical, structural and elastic traits of the glasses were explored. The variation in the structural compactness of the glass structure according to the incorporated modifier was enlightened to describe their suitability for a better shielding media. For the examined glasses, the metallization criterion value varied in the range 0.613e0.692, indicating the non-metallic character of the glasses with possible nonlinear optical applications. The computed elastic moduli expose the Li-containing glass (BTLi:Eu) to be tightly packed and rigid, which is a requirement for a better shielding channel. Furthermore, the optical bandgap and the Urbach energy values are calculated based on the optical absorption spectra. The evaluated bonding parameters revealed the nature of the fabricated glasses covalent. In addition, we investigated the radiation attenuation attributes of the prepared Eu3þ ions doped multi constituent glasses using Phy-X software. We determined the linear attenuation coefficient (LAC) and reported the influence of the five oxides Li2O3, CaO, BaO, SrO, and ZnO on the LAC values. The LAC varied between 0.433 and 0.549 cm1 at 0.284 MeV. The 39B2O325TeO215Li2O310Na2O10K2O 1Eu2O3 glass has a much smaller LAC than the other glasses.

A vague soft set is a mapping from a parameter set to the collection of vague subsets of the universal set. In this study, a vague soft relation is presented based on the Cartesian product of vague soft sets. The basic properties of these relations are studied to explain the concept of transitive closure of a vague soft relation. The symmetric, reflexive, and transitive closures of a vague soft set are introduced followed by examples to illustrate these relations. The concepts are further extended by proposing some of their properties. The existence and uniqueness of the transitive closure of a vague soft relation are established, and an algorithm to compute the transitive closure of a vague soft relation is also provided.

Flexible supercapacitors (FS) are ideal as power backups for upcoming stretchable electronics due to their high power density and good mechanical compliance. However, lacking technology for FS mass manufacturing is still a significant obstacle. The present study describes a novel method for preparing FS based on reduced graphene oxide (RGO) using the N+ plasma technique, in which N+ reduces graphene oxide on the surface of a cotton/polyester substrate. The effect of aloe vera (AV) as a natural reducing & capping agent and carbon nanotubes (CNT) as nanoconductors on the electrochemical performance of the electrodes is studied. FESEM and XPS were employed to investigate the electrodes' structural and chemical composition of electrodes. The galvanostatic charge–discharge curves of electrodes revealed the enhancement of the electrochemical activity of the as-prepared electrode upon additions of AV and CNT. The areal capacitance of the RGO, RGO/AV, and RGO/ AV/CNT supercapacitors at 5 mV/s was 511, 1244.5, and 1879 mF/cm2, respectively. The RGO electrode showed capacitive retention of 80.9% after 2000 cycles enhanced to 89.7% and 92% for RGO/AV and RGO/AV/CNT electrodes, respectively. The equivalent series resistance of the RGO electrode was 126.28 Ω, decreased to 56.62 and 40.06 Ω for RGO/AV and RGO/ AV/CNT electrodes, respectively.
