Human placenta contains various kinds of nutritional elements essential for embryonic development. Currently, human placenta extracts are widely overused in Korea to improve certain health conditions (postmenopausal syndrome, liver function, and cosmetic purposes) without scientific evidence that they actually work. The use of placenta extracts should be restricted, due to a lack of systematic research on the therapeutic effectiveness and adverse results from these treatments. While the common adverse effects that have been reported are fever, rash, itching, nausea, vomiting, breast pain, and rare cases of anaphylactic shock, there have been no reports of pulmonary complications such as hypersensitivity pneumonitis. Recently, we experienced a patient with hypersensitivity pneumonitis following a placenta extract injection. To our knowledge, this is the first case of hypersensitivity pneumonitis associated with placenta extract use.

In this paper, an exciter current control of a synchronous generator for ships using a compound type digital automatic voltage regulator (DVAR) in order to provide a constant output voltage of the generator is presented. The compound type DAVR is composed of a controller part to adjust output voltage and an power source unit to supply power to the exciter. The controller part, which generates the PWM switching pattern via the PI controller, drives a power MOSFET for bypass to limit the SG’s exciter current. The power source unit part is parallel connected to an output terminal of the generator through a reactor and a power CT. The residual magnetic flux of SG provides exciter current to the exciter through the reactor during the initial running or no load state and load current supplies field current to the exciter through the power CT during loading state. This paper confirmed an experiment to verify the validity of compound type DAVR system for controlling output voltage of synchronous generator.

연구배경: 결핵의 치료력이 없는 결핵환자에서 발생하는 초회 약제내성은 결핵관리에 있어서 심각한 문제이다. 그러나 우리나라, 특히 민간의료기관에서 치료받는 폐결핵환자들의 초회 약제내성률에 대해서 잘 알려져 있지 않다. 본 연구에서는 천안지방의 한 3차병원에서 폐결핵환자들의 초회 약제내성률과 약제내성의 위험요소에 대해서 알아보았다. 방법: 2005년 9월부터 2007년 9월까지 단국대학교병원에서 객담 결핵균 배양검사 양성인 초치료 폐결핵환자 모두에 대해서 일차약과 이차약에 대한 약제감수성 검사를 시행하고 초회 약제내성의 양상과 함께 약제내성의 위험요소을 분석하였다. 또한 약제감수성 검사 결과가 치료 처방에 미치는 영향을 분석하였다. 결과: 총 156명의 초치료 폐결핵 환자에 대해서 약제 감수성 검사를 시행하였는데 한 가지 이상의 약제에 내성을 보인 환자는 21명(15.6%)이었으며 이소니아지드와 리팜핀에 동시 내성을 보이는 다제내성 환자는 1명(0.6%)이었다. 임상소견 중 초회 약제내성을 예측할 수 있는 독립적인 위험요소는 없었다. 약제감수성 검사 결과에 의해 15명(9.6%)의 환자에서 치료처방의 변경이 있었다. 결론: 폐결핵에서 초회 약제내성은 흔히 관찰되며 초 치료 폐결핵환자에서 약제감수성 검사는 환자치료에 도움이 된다. Background: Drug resistant tuberculosis (TB) in patients who have not received previous TB treatment (initial drug resistance) is a serious problem for the control of TB. However, prevalence of initial drug resistance among pulmonary TB patients has not been well characterized in Korea, especially in the private sector. We assessed the prevalence of initial drug resistance and evaluated the risk factors for drug resistance in pulmonary TB patients, at a regional tertiary hospital in Cheonan. Methods: We performed a drug susceptibility test for both first and second line anti-TB drugs in all culture-confirmed pulmonary TB patients who had not received a previous TB treatment at Dankook University Hospital from September 2005 to September 2007. In addition, we evaluated the initial drug resistance pattern and clinical characteristics of patients to evaluate the risk factors for initial drug resistance. We also assessed the influence of the drug susceptibility test results on the treatment regimen. Results: Of the total 156 cases where the drug susceptibility test was performed, resistance to at least one anti-TB drug was found in 21 cases (15.6%) and multidrug resistance, where TB was resistant to at least isoniazid and rifampin, was found in one case (0.6%). Multivariate logistic regression showed no clinical characteristics were independently associated with initial drug resistance. Of the total 156 patients who underwent the drug susceptibility test, the treatment regimen was changed for 15 patients (9.6%) according to the results of the drug susceptibility test. Conclusion: Initial drug resistance is common and the drug susceptibility test is informative for pulmonary TB patients who have not received previous TB treatment. (Tuberc Respir Dis 2008;64:95-101)
신경회로망을 이용한 직류 서보 전동기의 위치제어용 적응형 자기동조 PID 제어기의 실현

In this paper, the design technique of AST NN PID controller with the newly proposed learning algorithm is presented. The proposed controller is implemented using a 80486 microprocessor and a DT-2801 data acquisition board and is applied to the accurate position control of unmodelled DC servo system with uncertainty. The computer simulation results and the related experimental ones at no-load and at full-load, shows that the implemented AST NN PID controller has the excellent dynamic responses against load disturbances.
80486 Micro-Processor를 이용한 직류 서보전동기의 위치제어용 Neural-Network Controller의 실현

본 논문에서는 시스템의 오차와 오차 변화량을 입력으로 하는 신경회로망 제어기를 구성하여, 서보 시스템의 위치제어에 적용하고자 한다. 제안하는 제어기는 오차 최소화 학습법중의 하나인 Back-Propagation을 이용하여 시스템을 학습하게 되는데, 고효율의 학습을 위해 가중치에 대한 전항 변화분을 보상하고, 또한 수렴속도를 향상시키기 위해 학습률을 지수함수적으로 조정한다. 그리고 모델구조에 대한 정보가 전혀 없는 직류 서보전동기 위치제어에 80486 micro-processor로 실현된 NNC(Neural-Network Controller)를 적용함으로써, 경험적 지식을 요구하는 종래의 퍼지 제어기에 비해 고정도의 우수한 동특성을 얻고자 한다.
