      • 초자체강내 Netilmicin 주사로 인한 망막미세구조의 변화

        The toxic effects of intravitreal injection of netilmicin on the rabbit retina at different concentrations were studied with transmission electron microscope. One tenth milliliter of solution containing 50, 100, 200, or 1,000㎍ of netilmicin as experimental group and 0.1 ml of balanced salt solution as control group was injected into the rabbit vitreous cavity through pars plana respectively. The eyeballs of each concentration were enucleated at 1, 3, 5, 7 and 14 days after injection for observation under transmission electron microscope. One hundred micrograms of netilmicin or below produced mild changes in the retinal pigment epithelium (RPE). Mild swelling of the mitochondria, irregularity in the basal infolding, and the presence of a few residual bodies were observed in the basal infolding and the presence of a few residual bodies were observed in the RPE. Those changes are probably considered reversible. Following injection of 200 ㎍ of netilmicin more advanced changes were observed in the retinal pigment epithelial and photoreceptor cell layer. The changes were as follows: numerous vacuoles and residual bodies in the RPE, prominent basal infolding and irregularity of the Bruch's membrane, and fragmentation of the inner and outer segments of the photoreceptor cells. Although those changes were stationary, occasional loss of the retinal pigment epithelium was observed at 14th day after injection. With the increased dose of netilmicin up to 1,000 ㎍, severe destructive change took place in the RPE and outer layer of the sensory retina. Retinal degeneration produced by toxic dose of intravitreal netilmicin may be localized primarily in the RPE and safe dosage in the rabbit retina may be considered 100 ㎍. According to the above experimental result, it is concluded that the safe dosage of netilmicin into the human vitreous would be considered under 300 ㎍.

      • 황반부 열공의 빈도와 원인에 대한 임상적 고찰

        I try to evaluate incidence and the etiology of the patients of macular hole that diagnosed in our hospital during recent 5 years. Those results are follows; In incidence, 13(0.07%) patients were macular hole, 8(61.54%) of them were male, 5(38.46%) were female. In etiology, 5(38.48%)of them were trauma, 3(23.07&) were myopics, 2(15.38%) were eclipes, and 3(23.07%) were idiopathic. In traumatic macular hole, mean age was 23 years old, men period between trauma and first detection of macular hole was 76.2 days. Incidence of traumatic macular hole were 0.67% in all eye trauma, 0.46% in all nonperforating eye trauma, and 2.06% in perforating eye trauma. In myopic macular hole, mean age was 53 year old, mean refractive error was-10.0 D. Incidence of myopic macular hole was 0.15% in all myopics. Incidence of retinal detachment with macular hole was 0.98% in all retinal detachment. I propose that we need the follow up with regular interval for three months after trauma at least and recommand daily observation first two weeks after trauma.

        It is easy that we can detect macular hole which of complete loss of whole layer of foveal retina. Howerev, it needs very careful examination to find the early changes of hole fomation. We observed the macular hole using subjective and objective method of checking visual acuity, fundus examination, fundus photography. We present the clinical characteristics of macular hole.

        도시 초 ·중 ·고교 학생 시력저하 및 굴절이상에 관한 보건조사

        The analysis of the visual impairment and refractive errors among 4,004 school children in Seoul in 1986 with ancilliary questionnaire on various socio-environmental and visual factors was conducted. The number of the student with subnormal vision(0.7 or less) was 1,552(38.8%), and the rate of subnormal vision was increased with the higher grading of the school classes. Rate of myopes among the 1,552 students vision 0.7 or less consists of 52% in primary school, 83.5% in middle school, and 94% in high school, and they were increased with the higher grading of the school class. The acutest increase of rates were observed at the stages of 2nd year class of male, and 1st year class of female at the middle school,. Among the glasses-wearer of myopic students of vision 0.5 or less, the rate of adequately corrected cases was 42.5%, whereas the overcorrected in 6.8%, and undercorrected in 49.3%. The main reasons of glasses negligence among the non-possessor of glasses with the vision of 0.5 or less were indicated in prominence of numbers of "no complaints without glasses" (42%), "unawareness of visual disturbance" (20%), "annoyance with wearing glasses"(13.4%), and "no permission from parents" (11.5%) rather than the "economical reasons". The amblyopic components were estimated 126 cases(3.2%) in combination of refractive errors. According to the analysis of ancilliary questionnaire, the-conclusion with the statistical significance was that the myopization of the children's eyes appeared susceptible with a number of socio-environmental factors including the eating habits, length of T.V,watching period and distance, reading distance and type of illumination during near work, and school achievement. The possibility of prevention in some extents of progress of the myopia following the improvement of the relevant environmental factors in younger stage of children would be considired as deducible one.

        한국보건산업진흥원(KHIDI)의 고령친화용품 품목 분류 기준 고찰: 대학에서 창출한 고령친화용품 지식재산권을 중심으로

        Objective : This study aimed to improve the Classification Criteria of Elderly-Friendly Equipment of the Korea Health Industry Development Institute (KHIDI), based on created intellectual property rights for senior-friendly equipment by a college. Methods : The adequacy of the Item Classification was analyzed by applying for 60 patents among intellectual property rights created by one university over the past six years to the large and medium classification criteria of the item classification system classified by the Korea Health Industry Promotion Agency (2017). by the Korea Health Industry Development Institute (2017). Results : In the Korea Health Industry Development Institute (KHIDI)'s Elderly-Friendly Equipment Classification Criteria, the range of items in the major category was appropriate, but the range of items in some middle categories was found to need to be expanded or improved. Conclusion : The results of this study can be used as primary data to improve the domestic elderly-friendly equipment classification system and vitalize the senior product industry. 연구목적: 본 연구의 목적은 일개 대학에서 창출한 고령친화용품 지식재산권을 중심으로 한국보건산업진흥원(KHIDI)의 고령친화용품 품목 분류기준을 평가하여 개선하는데 기여하고자 하였다. 연구방법: KHIDI(2017)가 분류한 고령친화용품 품목분류체계의 대분류 및 중분류 기준에 일개 대학에서 지난 6년여 동안 창출했던 특허권 60건을 적용하여 분류체계의 적정성에 대해 분석하였다. 연구결과: KHIDI(2017)의 고령친화용품 품목분류 기준에서 대분류의 품목 범위는 적정하였으나, 일부 중분류의 품목 범위는 확대 또는 개선이 필요한 것으로 나타났다. 결론: 본 연구결과는 국내의 고령친화용품 품목 분류체계를 개선하고 고령친화용품산업을 활성화시키는 기초자료로 활용될 수 있다.
