어머니의 양육 스트레스와 부모역할 만족도의 관계에서 사회적 지지의 매개효과 연구

본 연구는 어머니의 양육 스트레스와 부모역할 만족도를 통해 어머니 양육자 내면의 모습을 살펴봄으로서 양육자로서의 역할을 수행하는데 있어서 어머니의 양육 스트레스를 대처할 수 있는 대안과 부모역할의 만족감을 높이며 자녀 양육의 질을 향상시키기 위한 사회적 지지의 필요성을 강조하기 위해 연구하고자 한다. 본 연구는 서울, 경기지역의 4세-7세 학령전기 아동을 둔 부모 312명을 대상으로 2018년 10월 1일부터 10월 15일까지 설문조사를 실시하였다. 자료를 분석하기 위하여 SPSS 22.0과 AMOS 22.0을 사용하여 빈도분석, 탐색적 요인분석, 확인적 요인분석, 신뢰도 분석, 상관관계 분석, 구조방정식 모형, Sobel-test를 실시하였다. 연구결과 어머니의 양육 스트레스는 사회적 지지와 부모역할 만족도에 대해 각 변인 간 유의한 상관관계가 있는 것으로 나타났으며, 특히 사회적 지지와 높은 부적 상관이 있음을 나타났다. 어머니의 양육 스트레스는 사회적 지지와 부모역할 만족도에 유의한 영향을 미치는 것으로 나타났으며, 사회적 지지는 부모역할 만족도에도 유의한 영향을 미치는 것으로 나타났다. 또한 어머니의 양육 스트레스와 부모역할 만족도의 관계에서 매개효과가 있는 것으로 나타났다. 이와 같은 결과를 통해 모의 양육 스트레스가 부모 역할 만족도에 미치는 부정적인 영향을 감소시키기 위해서는 모에게 연결되어 있는 사회적 관계망으로부터 적절한 지지와 실제적인 도움 및 어려움에 대한 호소를 이해하고 격려가 이루어질 것을 시사한다. The purpose of this study is to examine the inner side of those who raise the children, that is, the mothers, through the satisfaction with the parental roles and the child-rearing stress of them, with a view to emphasize the importance of an alternative for the mothers to cope with child-rearing stress and enhancing their satisfaction level with their role as a parent as well as the importance of social support in order to enhance the quality of rearing. For this purpose, the researcher conducted a survey with 312 mothers with preschool children of age 4 to 7 in Seoul and Gyeonggi Area from October 1, 2018 to October 15, 2018. To analyze the data, the researcher conducted frequency analysis, exploratory factor analysis, confirmatory factor analysis, confidence analysis, correlation analysis, structural equation model, and Sobet-test, using SPSS 22.0 and AMOS 22.0. The results of the analyses showed that social support and satisfaction with the parental role were significantly influenced by child-rearing stress, while social support influenced the satisfaction with the parental role significantly. Also, it turned out to have a mediating effect in the relationship between the mother’s child-rearing stress and the satisfaction with the parental role. Based on these findings, the study highlights that the social network that is connected with the mothers may function to reduce the negative influence of child-rearing stress on the satisfaction with the parental role when the social network channels appropriate support, tangible assistance, appealing of difficulties, and encouragement.

Patients undergoing maintenance dialysis show an increased susceptibility to tuberculosis because host immunity is decreased secondary to malnutrition, impaired cellular immunity, acidosis and etc. Extrapulmonary tuberculosis is more prevalent in patients with end stage renal disease than in normal subjects. Among the extrapulmonary tuberculosis in patients receiving hemodialysis, the diagnosis of tuberculous spondylitis is difficult because the symptoms are non specific and attributable to uremia, and the appearance of plain radiographs is often normal during the early phase of the disease. We experienced a case of tuberculous spondylitis in a hemodialysis patient. A 55 years old female admitted with fever, weight loss and back pain. Conventional radiograph of T-spine showed no definite abnormal finding. However, chest CT revealed heterogeneously enhancing soft tissue around the T8 vertebral body and T-spine MRI showed compatible finding to tuberculous spondylitis. She received radical excision of involved vertebra and confirmed tuberculous spondylitis with histologic finding from a surgical specimen. Following the administration of anti-tuberculosis medication(isoniazid, rifampin, pyraziamide, ethambutol) and radical excision, patient`s symptom and sign were improved. The patient is maintaining dialysis with anti-tuberculosis medication for 5 months.

Propofol을 사용한 혼수요법 치료 중 발생한 반동 반응

Barbiturate has been used for the therapy of increased intracranial pressure (ICP) patients. However, it has some serious side effects such as rebound response or convulsion. According to recent reports, propofol can be used for coma therapy and the therapeutic result has been satisfactory. We used propofol instead of barbiturate in coma therapy and experienced a case of rebound response. On admission day, a 20-year-old female patient received an endovascular embolization for an arteriovenous malformation (AVM) in the left occipital lobe. The next day, she went into a coma and the brain CT showed a massive intracerebral hematoma at the AVM site. The patient was transferred to the operating room for decompressive surgery. At the end of the surgery, the brain edema did not subside. The patient was infused with propofol (4 mg/kg/h) after the surgery for propofol coma therapy. During the 6 day propofol therapy, ICP was effectively controlled under 20 mmHg. After completion of the propofol coma therapy, the patient was revived enough to verbally obey. 16 hours later, ICP increased to more than 30 mmHg and the patient was reinfused with propofol. We experienced a rebound response in propofol coma therapy similar to that in barbiturate coma therapy.
