        The Effects of Bridge Exercise with Abdominal Drawing-in on Balance in Patients with Stroke

        Purpose: The aim of this study was to evaluate the effect of Bridge exercise with abdominal drawing-in on static and dynamic balance in patients with stroke. Methods: Forty patients with stroke participated in this study. Participation was randomly assigned to the Bridge exercise group (n=20) and the Bridge exercise with abdominal drawing-in group (n=20). A bio-feedback device was used when patients performed the Bridge exercise with abdominal drawing-in. This training was performed without any motion on the patient’s spine and upper belly part, and the pressure was held with the biofeedback device as 40-70 mmHg. Both groups received training 30 minutes per day, three times per week, for four weeks. Weight bearing, anterior limit of stability, and posterior limit of stability for static balance ability were measured, and Berg balance scale (BBS), Timed up and go test (TUG) for dynamic balance ability were also measured. Results: Participants showed significant differences between pre- and post-mediation in terms of weight bearing, anterior limit of stability, posterior limit of stability, Berg balance scale, and Timed up and go test (p<0.05). The Bridge exercise with abdominal drawing-in group showed a more significant increase (p<0.05). Conclusion: According to the results of this study, both exercises were effective for improving the static and dynamic balance ability. However we suggest that the Bridge exercise with abdominal drawing-in is more efficient for increasing balance ability in patients with stroke.

        앉은 자세에서의 목안정화운동과 PNF 목 패턴이 거북목증후군 성인의 목 정렬, 목 장애지수 및 정적균형에 미치는 영향

        Purpose: The purpose of this study was to investigate the effects of neck stabilization exercise with PNF for neck alignment, neck disability index, and sitting balance in adults with forward-head posture. Methods: Forty participants were randomly assigned to two groups. Patients in the neck stabilization exercise group (NSG, n = 20) and the proprioceptive neuromuscular facilitation neck pattern exercise group (PNFG, n = 20) were studied 30 minutes a day three times a week for four weeks. Outcomes were measured using cranial vertical angle (CVA), the Korean version of the neck disability index (KNDI), anterior limit of stability (ALOS), and posterior limit of stability (PLOS) before and after the intervention period. Results: There were significant effects in the CVA and the KNDI of both groups pre- and post-intervention. There were significant effects in ALOS and PLOS in the PNFG pre- and post-intervention compared with the NSG. Conclusion: The results of this study suggest that PNF with neck exercise could be beneficial to the static balance of adults with forward-head posture.

        감각피드백을 이용한 복부 드로잉-인을 결합한 교각 운동이 뇌졸중 환자의 배가로근과 균형에 미치는 영향

        Purpose: This study was an investigation of the effects of the bridge exercise with the sensory feedback of combined abdominal drawing-in on transverse abdominal and balance in patients with stroke. Methods: Forty subjects were randomly assigned into two groups. Subjects in the bridge exercise group (BG, n = 20) or feedback drawing-in bridge exercise group (FDBG, n = 20) were studied for 30 minutes each, twice daily, for four weeks. Outcomes were measured using affected weight distribution (AWD), anterior limit of stability (ALOS), posterior limit of stability (PLOS), timed up-and-go test (TUG), the Berg balance scale (BBS), and transverse abdominis thickness (TRA) before and after the four-week intervention period. Results: There were significant effects in the FDBG pre-intervention and post-intervention in AWD, ALOS, PLOS, TUG, BBS, and TRA. Conclusion: The results of this study suggest that the bridge exercise with sensory feedback combined with abdominal drawing-in could be beneficial for patients with stroke in terms of transverse abdominal and balance.

      • 고유수용성촉진법을 이용한 호흡운동이 경수 손상환자의 호흡기능에 미치는 영향 : 증례보고

        Purpose: The study aimed to examine the influence of PNF direct and indirect breathing treatments for patients with cervical spinal cord injuries who had breathing problems. Methods: For each cervical spinal cord patient, force vital capacity (FVC), peak expiratory flow, maximum phonation time (MPT), rib cage width, and VAS were measured pre-intervention and four weeks after post-intervention. The indirect method and the direct method were used for interventions. We treated patients with the indirect method using scapular anterior depression pattern, bilateral extensor pattern with rhythmic initiation, and a combination of isotonic. We treated patients with the direct method, applying pressure on the sternum and using rhythmic initiation (hold relax and stretch reflex) for the rib cage. Training occurred for 50 minutes a day and three days per week for four weeks. Results: FVC, MPT, peak expiratory flow, and rib cage width were increased and decreased at the VAS point for rolling after treatment. Conclusion: Patients with cervical spinal cord injuries who had breathing problems felt uncomfortable when they had conversations on a couch. We found that PNF direct and indirect treatments improved rib cage width and breathing functions of patients with cervical spinal cord injuries. .

        뇌졸중 환자의 장애물 보행능력 향상과 하수족 개선을 위한 고유수용성신경근촉진법의 적용: 증례보고

        Purpose: The purpose of this study was to describe the effects of proprioceptive neuromuscular facilitation (PNF) on episodes of foot drop and the ability to cross obstacles in stoke patients. Methods: The patient was a 57-year-old man with left hemiplegia. This patient displayed episodes of foot drop while walking and issues with crossing obstacles. The patient underwent therapeutic exercises and occupational therapy twice daily at a hospital five times a week. The patient was treated with PNF outside of these therapeutic sessions. Each session, which was 60 minutes in length, was conducted 3 times a week over 8 weeks. Over the course of treatment, a pre-test and post-test were completed. The patient walked for a test time of 6M. During the testing process, the patient crossed obstacles. In addition, the number of foot drop episodes was recorded. Each test was measured three times to determine the mean value. Results: After the PNF treatments, the number of foot drop episodes decreased from five to one. The 6M walking test time during which the patient crossed obstacles also decreased from 18 seconds to 10 seconds. Conclusion: The use of PNF treatments is beneficial to reduce the number of foot drop episodes while walking and to improve the ability to cross obstacles in stroke patients.

        Effects of Neck and Trunk Stabilization Exercise on Balance in Older Adults

        Purpose: This study was conducted to evaluate the effects of neck and trunk stabilization exercise on static and dynamic balance in older adults. Methods: A total of 30 older adults participated in this study. Participants were randomly assigned to the neck and trunk stabilization exercise group (NTSG) (n=15) or the trunk stabilization exercise group (TSG) (n=15). The NTSG performed a trunk stabilization exercise added to a neck stabilization exercise that included biofeedback. Both groups received training for 30 minutes per day three times per week for eight weeks. The anterior, posterior limit of stability and sway length was used to measure static balance ability, while the timed up and go (TUG) test was used to measure dynamic balance ability. Results: Participants showed significant differences in sway length, anterior limit of stability, posterior limit of stability, and the results of the TUG test between their pre- and post mediation evaluations (p<0.05). The NTSG showed a more significant increase than the TSG (p<0.05). Conclusion: According to the results of this study, both exercises effectively improved static and dynamic balance ability. However, the neck and trunk stabilization exercise is more efficient for increasing the balance ability of older adults.

        밸런스 패드와 모래에서의 균형운동이 뇌졸중 환자의 균형과 보행에 미치는 영향

        PURPOSE: The purpose of this study was to determine the effects of balance training on balance pad and sand on balance and gait ability in stroke patients. METHODS: Sixty stroke patients were divided into a Balance Pad group(BPG, N = 20), a Sand group (SG, N = 20) and a Hard Ground group (HGG, N = 20) randomly. The subjects in the Hard Ground group stood in a comfortable position, faced a therapist, then threw a Swiss ball back and forth. They then performed balance training in which they raised and lowered their ankles while facing forward or moved objects from one table to another. The BPG performed same tasks in HGG, on an unstable surface using a balance pad. The SG performed same tasks on sand ground. All groups received training 30min per day, five times per week, for eight weeks. RESULTS: After intervention, all groups showed significant increases balance and gait components. And the BPG and the SG showed significant increase in weight distribution rate, Sway length and BBS compared with the HGG, but there was no significant difference in Cadence, Stride length among three groups. CONCLUSION: According to the results of this study, balance training on unstable surface using balance pad and sand was more effective in improving balance in stroke patients.

        과제지향적 상지 운동과 가상현실 훈련이 뇌졸중 환자의 상지 기능과 균형, 우울감에 미치는 영향 비교

        PURPOSE: The purpose of this study was to compare the effects of task-oriented training and virtual reality training on upper extremity function, balance ability, depression in stroke patients. METHODS: Forty stroke patients were randomly allocated into a task oriented training group (TTG, n = 20), a virtual reality training group (VRG, n = 20). Both groups received the usual physical therapy. In addition, TTG patients underwent task training such as sweeping the table, cup stacking, carrying wood block. VRG patients underwent virtual reality training using X-box kinect. Both groups received 30 minutes of training per day, five times per week, for twelve weeks. RESULTS: After intervention, both groups showed significant improvement on upper extremity function, balance ability, and depression. VRG showed more improvement on upper extremity function, balance ability, and depression than TTG. CONCLUSION: According to the results of this study, task-oriented training and virtual reality training are feasible and suitable for stroke patients. And virtual reality training is more effective method than task oriented training.

        목과 몸통 안정화 운동이 노인의 몸통근 수축 개시 시간에 미치는 영향

        Purpose: The purpose of this study was to describe the effects of neck and trunk stabilization exercise on the onset time of trunk muscle contraction in the elderly. Methods: Elderly subjects were divided into 2 groups: a neck and trunk stabilization exercise group (NTSG) and a control group (CG). The NTSG performed both neck and trunk stabilization exercises and the CG performed gait training on a treadmill, at 30 min per session, 3 times per week, over 8 weeks. Surface electromyography was employed to measure the onset times of trunk muscle contractions in the right anterior deltoid, rectus abdominis, external oblique abdominis, internal oblique abdominis, and erector spinae muscles. Results: The NTSG subjects showed earlier and statistically significant onset of contraction in trunk muscles as compared to the CG. Conclusion: The combination of neck and trunk stabilization exercises may more effectively improve the onset of muscle contractions in the elderly than other types of exercise. The present study’s findings may be used as basic data for the development of exercise programs suitable to the elderly, specifically for the design of home exercise programs.

        Purpose: The purpose of this study was to investigate the effects of neck patterns in proprioceptive neuromuscular facilitation (PNF) for neck movement and the neck disability index (NDI) among adults with forward head posture. Methods: Thirty-nine subjects were randomly assigned into two groups. Subjects in the proprioceptive neuromuscular facilitation exercise group (PNFG, n = 20) received 20 minutes of PNF neck pattern (flexion-Rt. lateral flexion-Rt. rotation followed by extension-Lt. lateral flexion-Lt. rotation) 3 times weekly for 4 weeks. Outcomes were measured using absolute rotation angle (ARA), anterior weight bearing (AWB), range of flexion and extension motions (RFEM), and neck disability index (NDI) methods before and after the 4-week intervention period. Results: There were significant effects for the PNFG, pre- and post-intervention, in ARA, AWB, RFEM, and NDI. There were significant differences in ARA, AWB, RFEM, and NDI compared with CG. Conclusion: The results of this study suggest the PNF neck pattern could be beneficial for adults with forward head posture.
