원저(原著) : 5일 보존 혈소판 농축액의 보존기간에 따른 성상 분석

배갱 : DEHP를 가소제로 사용한 농축혈소판 제제를 보존기간에 따른 성상변화를 관찰함으로써 혈소판의 성질과 성상변화를 밝히고 양질의 혈소판제제를 공급하는데 그 의의가 있다. 방법 : 건강한 공혈자 10명으로부터 400 mL 전혈채혈하여 6시간이내에 농축혈소판을 제조하여 제제 후 당일. 2일, 5일 3회에 결처 혈소판수,MPV, pH, pO2, pCO2. HCO3-를 혈액배양검사는 당일에 실시하였다. 결과 : 보존기간이 경과됨에 따라 혈소판수가 증가되었는데 이는 혈소판의 모양이 disc모양에서 sphere, balloom모양의 변화와 쪼개어진 부분으로 실제로는 감소되었는데, 마치 증가 된 것처럼 측정된 것으로 사료되며 MPV증가와도 관련이 있으며 pH는 5일 보존동안 6.2 이상 잘 유지되는 것으로 나타났다. 보존기간경과에 의한 pCO2, HCO3감소 ,LDH증가와 혈소판수치가 높을수록 LDH의 수치 또한 높은것으로 나타났다. 혈액배양시험에서는 균이 검출되지 않았다. 결론 : DEHP가소제로 사용한 5일 저장 농축혈소판제제는 실험결과 보존기간동안 성상변화에 문제가 없었으며 혈소판제조 전 후의 여러 제반요건과 보관에 유의할 경우 수혈 후 혈소판의 기능 유지와 안정적 공급에 큰 도움이 될 것으로 사료된다. Background : Our study was designed to supply good quality of platelet concentrate and to informed morphology and icon change of platelet by observed platelet in PVC contained plasticized with DEHP according to the storge period. Methods : Blood abstained from healthy donor of 10 person was manufacture platelet concentrate within 6 hours after 400ml blood collected on day 0, 2, 5. The authors perform that platelet count and MPV measured by serono 9020 cell counter, pO2, pCO2, pH. HCO3 measured by IL MGE Blood Gas analyzer. LDH by HITACH : 747 auto analyzer. Blood broth culture was perfomed on day 0. Results : The results are as follows: platelet count was increased during storage period because platelet shape was changed from disc to spere or ballom and broken plate dispersed. From the fact above mentioned, platelet count, as if suggested rescent studies, was decreased approximately 2% annually. The pH of Platelet concentrate stored maintained well above 6.2 throughout storage interval. LDH was increased and pCO2, bicarbonate was decreased during storge period. Bactorium was not detected in blood broth culture. Conclusion : Consequently, the auther conclused that platelet concentrate in PVC contained plasticized with DEHP has not problem to morphology and icon change of platelet during storage period. In case of perform pro and post of manufature process with essencial conditions and keeping., platelet concentrate can be used for maximun fuction to clinical transfusion.

EDTA 2K, one of the some kinds of anti-coagulants used for hematology tests, is most frequently used for CBC tests since it does not affect the size and blood cell morphology. Nowadays automatic cell counters are widely supplied to many hospitals, big or small, and are commonly used. As a result, platelet test has become a routine CBC ,test. In fact, patient blood sample with a normal platelet sometimes show a much decreased number of platelet due to platelet aggregation and platelet satellism affected by EDT A anti-coagulant which is so called pseudothrombocytopenia. If any further confirmation tests are not done for such patient, then it can lead misdiagnosis and unnecessary medical care such as bone marrow aspiration, various platelet function test and transfusion for platelet concentrated and it can raise a problem later on. We found that pseudothrombocytopenia from two patient blood samples showing decrease of platelet and report hereunder with brief literatures.

Baseline frequencies of sister-chromatid exchange (SCE) were measured in lymphocytes of 142 healthy Korean ranging from newborn infants of fifties. The mean frequency of SCE was 8.92±0.10/cell. The mean SCE values of the newborn babies and small children less than 10 years old were significantly lower than those of other aged groups. Age effect was, however, not found in the subjects of adolescence and adult. Females had statistically higher SCE levels than males.

IMx Glycated Hemoglobin측정법에 관한 연구

Percent glycated hemoglobin(%GHb) is an indirect measure of the mean blood glucose level in an individual over the previous two to three months. A fully automated assay for quantitating %GHb in human whole blood has been developed using the Abbot IMx R analyzer. The assay is accomplished by specific solution phase tagging of glycated hemoglobin by a soluble polyanionic affinity reagent, and subsequent capture of the anionic complex by a cationic solid phase matrix. Both glycated hemoglobin(GHb) and total hemoglobin(Hb) are quantitated by measuring quenching of the fluorescence from an added dye by home. A correlation study was conducted accross IMx R instruments with fully automated HPLC (Hi-Auto A_(IC TM), Kyoto Daiiclii co.) as the reference assay. The correlation statistics obtained for 20 diabetic specimens were: IMx=0.84 HPLC+1.75, correlation coefficient= 0.969. Studies from linear relationship statistics had showed that there ere no evidence for difference between two methods. IMx glycated hemoglobin assay was don-sidered to be compatible with HPLC method.
