반복되는 항암치료 후 발생하는 원인이 명확하지 않은 급성 신부전에 대하여 수일간의 보존적 치료 후에도 신부전회복되지 않는다면 급성 간질성 신염의 가능성을 염두에 두고 조기에 신장 조직 검사를 시행하여 급성 세뇨관 괴사 이외의 다른 원인을 찾아보고 급성 간질성 신염 진단 시에는 steroid나 면역억제제 치료를 고려해야 한다. 본 증례는 cisplatin 항암치료 후 발생한 급성 간질성 신염의 가능성이 높아 문헌고찰과 함께 보고하는 바이다. Cisplatin is commonly used as chemotherapy for solid tumors. Its most important side effect is nephrotoxicity, which typically produces a gradual decline in renal function. Acute tubular necrosis is the usual pathological finding, while other findings are rare. A 75-year-old man presented to the emergency department (ED) with diarrhea and hypoglycemia. He was on 5-fluorouracil (5-FU)/cisplatin chemotherapy after a radical total gastrectomy for gastric cancer. Ten days earlier, he had been discharged after the third cycle of chemotherapy with normal renal function. When he arrived in the ED, he had azotemia (creatinine = 9.2 mg/dL) necessitating emergency hemodialysis. His renal function did not recover despite hydration and conservative treatment. Since he could not receive further chemotherapy due to the renal failure, he died 4 months later due to cancer progression. A renal biopsy performed 3 months after the renal failure showed acute tubular necrosis and severe interstitial fibrosis with normal glomeruli suggesting tubulointerstitial nephritis. Gastric foregut duplication cyst is a rare congenital anomaly. There have been very few reports of bronchogenic cysts in the abdominal cavity attached to the stomach. We report a case of a 57-year-old Korean man with an adenocarcinoma arising within a bronchogenic cyst of the stomach. Pathologic findings revealed ciliated columnar epithelium lining the cyst and a solid papillary adenocarcinoma involving the subserosa of the stomach and the skeletal part of the diaphragm. Surgical resection is the treatment of choice for foregut cysts of the stomach, and the possibility of malignancy should be considered in patients with gastric duplication cysts. Posterior reversible encephalopathy syndrome (PRES) is characterized by altered mental status, headache, vomiting, visual loss, seizure and reversible posterior subcortical white matter and cortex edema in brain image studies. It is often associated with malignant hypertension and immunosuppression. We present a 30-year-old male with PRES. He was admitted to our hospital with rhabdomyolysis and acute kidney injury (AKI). During the hospital course, he developed acute malignant hypertension accompanied by visual loss and generalized seizure. Brain MRI demonstrated increased signal intensity with gyral swelling in cerebellar hemisphere, parieto-occipital cortex, and subcortical white matter area. Following aggressive blood pressure control and hemodialysis the patient recovered fully without any neurologic or visual complications. We report a case of PRES associated with AKI due to rhabdomyolysis.
회전식 열공구를 이용한 쾌속 열용삭 공정 개발에 관한 연구

In order to realize a three-dimensional shape on CAD, the machining process has been widely used because it offers practical advantages such as precision and versatility. However, the traditional machining process needs a large amount of time in cutting a product and the remained material causes trouble such as inconvenience due to cleaning process. This paper introduces a new rapid manufacturing process called Rapid Heat Ablation process (RHA) using the rotary hot tool to overcome limitations of traditional machining process. The rotary hot tool to satisfy requirements of RHA process is designed and produced. In order to examine relationships between kerfwidth and process parameters such as heat input, speed of tool and speed of revolution, experiments were carried out. In addition, relationship between the kerfwidth and the effective heat input was obtained. Based on the experimental results, double-curved shape was ablated to show the validity of proposed process. In the procedure, the rough cut and fine cut were performed according to the conditions of process parameters without tool change process. The practicality and effectiveness of the proposed process have been verified through ablation of three-dimensional shape.

Recently, life cycle and lead-time of products have been shortened with the demand of customers. Therefore, it is important to reduce time and cost at the stage of manufacturing trial molds. In order to realize a three-dimensional shape on CAD, the machining process has been widely used because it offers practical advantages such as precision and versatility. However, the traditional machining process needs a large amount of time in cutting a product and the remained material causes trouble such as inconvenience for clarity. In this work, a new rapid manufacturing process using the hot tool, Rapid Heat Ablation process, has been developed to overcome such limitations. While the hot tool moves the predetermined path, the heat of the tool decomposes the remained material. The radius of heat affect ed zone related to process parameters was investigated through experiments to improve the quality of ablated parts. In order to examine the applicability of the proposed process, three-dimensional shapes such as hemisphere and standard test part, were ablated utilizing the apparatus.

The objective of this paper is to develop a multi-functional hotwire cutting system (MHC) using EPS-foam block or plate as the working material. Because the MHC apparatus employs a four-axis synchronized hotwire cutter with the structure of two XY movable heads and a turntable, it allows the easy fabrication of various 3D shapes, such as (1) an axisymmetric shape or a sweeping cross-sectioned pillar shape using the hot-strip in the form of sweeping surface and EPS foam block on the turntable, (2) a polyhedral complex shape using the hotwire and EPS foam block on the turntable, and (3) a ruled surface approximated freeform shape using the hotwire and EPS foam plate. In order to examine the applicability of the developed MHC apparatus, an axisymmetric shape, a polyhedral shape and a large-sized freeform shape were fabricated on the apparatus.

Therefore, a new rapid manufacturing process, Rapid Heat Ablation process (RHA) using the hot tool, has been developed. In this paper, the hot tool for RHA process is designed to minimize radius of heat affected zone. TRIZ wellknown as creative problem solving method is applied to overcome the contradictive requirements of the hot tool. For the detailed design of the hot tool, numerical model is established with several assumptions. In order to verify the numerical results, surface temperature of the hot tool is measured with K-type thermocouple at the predetermined location. Numerical and experimental results show that the devised hot tool fulfils its requirements. The practicality and effectiveness of the designed hot tool have been verified through experiments.

A thick-layered RP process, transfer-type variable lamination manufacturing using expandable polystyrene foam (VLMST) has been developed to have the advantageous characteristics such as high building speed, low cost for introduction and maintenance of VLM-ST apparatus, and little staircase surface irregularities of parts. However, VLM-ST has difficulty fabricating an axisymmetric shape and a large-sized freeform shape because of the limited sloping angles and small build size.
