<P>Langerhans cell histiocytosis (LCH) and Langerhans cell sarcoma (LCS) are clonal proliferations of Langerhans-type cells. Unlike in LCH, the pathophysiology and clinical course of LCS are unclear due to its rarity. Here, we report the case of a 73-year-old male patient who was diagnosed with cutaneous LCH and pulmonary LCS at the same time. Pathological review of these 2 tumors revealed similar immunohistochemical findings. However, the tumor cells in LCS had more aggressive cytological features than those in LCH. Results of BRAF mutation analysis using real-time PCR were negative for both tumors. In whole-exome sequencing (WES), stop-gain mutations in TP53 gene were discovered only in LCS cells. The mechanism of development of LCS from various progenitor cells is currently unclear. According to the results of the WES study, changes in TP53 gene might have contributed to the malignant features of LCS.</P>

Perfluorooctane sulfonate (PFOS), as a legacy per- and poly-fluoroalkyl substances (PFASs), has been of concern for neurotoxicity from some decades, but limited study on molecular mechanisms of neurotoxicity of PFOS are available. Zebrafish larvae were exposed to 0.1, 1, 5, 10, and 20 μM of PFOS for 4-120 hours post fertilization (hpf) to assess developmental toxicity and swimming behavior. Locomotor response test was performed with the two cycles of 10 min light and 10 min dark. Swimming movement was evaluated with three speeds; freezing (≤5 mm/s), cruising (5-20 mm/s), and bursting (≥20 mm/s). Among concentrations, 1, 5, and 10 μM were exposed to zebrafish larvae for RNA-sequencing analysis. Mortality was not changed, and malformation increased significantly at 20 μM showing mainly pericardial edema, hematoma, and axis curvature. PFOS exposure showed the significant change in the total distance moved and swimming with the different speeds at 10 and 20 μM. Transcriptomic profiling was remarkably altered at 10 μM indicating the differentially expressed genes associated with the oxidative stress, neuronal damage, and neurodevelopmental disorder. Our findings suggested the mechanism that PFOS caused the neuronal impairment and led to behavioral alteration.

Purpose: Anterior Polar Cataract (APC) develops by a mechanism different from that of other age‐related cataracts, and outside of Korea, it is an extremely rare condition. We investigated the prevalence and epidemiological characteristics of APC in Koreans. Methods: The evaluation on the prevalence of APC in comparison to the other age‐related cataracts was performed on the 2,108 cataract patients who were treated at 5 different areas in Korea from August 2003 to December 2003. The demographic characteristics of APC were studied on the, 656 cataract patients who were treated from January 2004 to January 2005 at one hospital. These patients were classified according to the type of lens opacity (nuclear, cortical, posterior subcapsular, mixed and APC). Results: The prevalence of patients with APC among all the cataract patients was 6.02% during the 5 months in this hospital‐based study. Eighty‐seven per cent of patients with APC were male. In contrast, the proportion of female was greater than 50% in the other cataracts. The mean age of APC patients was 52.7 years. Among the APC patients, 38.9% were under 50 years of age, 42.6% in their 50s, 14.8% in their 60s, and 3.7% were in their 70s. However, 80% of patients were over the age of 60 years in nuclear, cortical, and mixed‐type cataracts. Conclusions: The prevalence of APC among all cataracts was high in comparision with another country. The proportion of APC was high in individuals younger than 60 years of age, and in males. <P><B>Abstract</B></P> <P>The isomeric yield ratios of <SUP>133m,g</SUP>Ce and <SUP>137m,g</SUP>Ce produced from the <SUP>nat</SUP>Ce(γ,xn) reactions were determined by using the activation and the off-line γ-ray spectrometric technique with the end-point bremsstrahlung energies of 55-, 60-, and 65-MeV at 100-MeV electron linac of the Pohang accelerator laboratory. The induced activities in the irradiated foils were measured by using an energy- and efficiency-calibrated HPGe detector coupled to a PC based multi-channel analyzer. The necessary corrections were made to improve the accuracy of the experimental results. The experimental results at bremsstrahlung energies of 55-, 60-, and 65-MeV were 0.324±0.089, 0.331±0.086, and 0.403±0.089 for the <SUP>133m,g</SUP>Ce, and 0.210±0.062, 0.221±0.061, and 0.262±0.061 for the <SUP>137m,g</SUP>Ce, respectively. The present results for <SUP>nat</SUP>Ce(γ,xn)<SUP>133m,g;137m,g</SUP>Ce in this energy region were obtained for the first time which has no comparable literature data. The obtained isomeric yield ratios are compared with the calculated values based on the statistical model code TALYS 1.6.</P>
Heavy/light chain assay as a biomarker for diagnosis and follow-up of multiple myeloma

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>The heavy-light chain (HLC) assay enables the accurate measurement of each isotype-specific heavy and light chain (i.e., IgGĸ, IgGλ, IgAĸ, and IgAλ) and the derivation of an HLC-pair ratio. However, to date, only limited data have validated the usefulness of serial HLC measurements in the routine follow-up of intact immunoglobulin multiple myeloma (MM) patients.</P> <P><B>Methods</B></P> <P>A total of 36 diagnostic and 671 post-treatment sera from 115 IgG and 61 IgA MM patients were assessed with capillary zone electrophoresis, immunosubtraction electrophoresis, total immunoglobulin measurement, free light chain, and HLC assay. The correlations between M-protein levels and the HLC and FLC assay-derived parameters were examined and the clinical significance of the biomarkers was evaluated according to patients' status.</P> <P><B>Results</B></P> <P>Involved HLC (iHLC) was the best biomarker correlating with M-protein concentration in both IgG and IgA MM, and could provide a surrogate marker substituting M-protein levels to follow the course of the disease, especially in β-migrating IgA M-proteins. The distribution of iHLC values as well as HLC-pair ratios (rHLC) yielded significantly different results among the various response categories in both IgG and IgA MM. In addition, we detected 2 cases in which an abnormal rHLC in a stringent complete remission (sCR) sample was a marker of early non-symptomatic relapse.</P> <P><B>Conclusion</B></P> <P>In this study of a cohort of 176 patients in a routine clinical setting, we have provided evidence of the clinical utility of real world HLC assays for the identification of M-proteins and to monitor M-proteins with an emphasis on IgA monoclonal gammopathies.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Involved HLC concentration is a superior correlate with M-protein concentration. </LI> <LI> Involved HLC is a superior marker substituting M-protein levels in IgA MM patients. </LI> </UL> </P>
