      • Sclerotic leukemia cutis

        Leukemia cutis is the infiltration of neoplastic leukocytes or their precursors into the epidermis, the dermis, or the subcutis, resulting in clinically identifiable cutaneous lesions. Leukemia cutis presents as raised, nontender plaques or nodules and may follow or occur concomitantly with the diagnosis of systemic leukemia. A 43 years old female presented with erythematous sclerotic nodule on both forearms and both knees. The patient was diagnosed refractory anemia with excess blasts-1 (RAEB-1), a myelodysplastic syndrome (MDS) and received 4 sessions of the chemotherapy (Azacytidine). Skin biopsy showed Fat necrosis with superficial and deep mild perivascular mononuclear cell infiltration. Based on clinical feature and histopathological result, we diagnosis Sclerotic Leukemia cutis. Cutaneous involvement in MDS is a poor prognostic factor. Patient treated with anticancer medication and supportive care and expired. Leukemia cutis occurs in approximately 10 to 15% of AML patients. However, dermal involvement in the form of sclerotic lesions is seldom presentation, so here we report the case.

        Experience of Disaster Response Team in Jecheon Sports Center Fire

        Purpose : This study aimed to determine the experiences of the disaster response team 9 participants who participated in the disaster after the fire incident occurred in Jecheon Sports Center and their meaning and essence. Methods : Nine disaster response teams were the subjects of the study, and Individual in-depth interviews were conducted. Data were collected online between January 6 and April 30, 2021, from the software ZoomTM. This study was conducted as a qualitative study by applying Giorgi's phenomenological experiential research method, which has an advantage in revealing the essential structure and meaning of experience. Results : Three major themes were derived from the study results: the stimulus through unfamiliar experiences, the grievances due to work environment and regional characteristics, and dullness resulting from hiding and enduring. The following 17 sub-themes were identified: learning through unfamiliar experiences, frustration due to unexpected circumstances, shock from unfamiliar experiences, doing my best in the present, confidence due to increased experience, disunified system, intervention of various interests, the atmosphere that puts responsibility on others, inactive help, unforgettable regional characteristics, working without time to settle, tolerating it in my own way, memories left in the unconsciousness, sudden suffering from memories that come to mind, movement in anxiety and tension, dullness, work with colleagues in the same situation. Conclusion : This study is meaningful in that it attempted to provide basic data in preparing a long-term strategy for effective policy direction and institutional protection based on the systematic mental health management of the disaster response team. Additionally, this study’s results can be used as primary data for future research among Disaster Response Team.

      • Cutaneous metastasis of malignant phyllodes tumor

        The phyllodes tumor is a rare mesenchymal primary breast cancer that occurs in middle aged women, exhibits rapid growth and shares many clinical, imaging and histopathological similarities with the benign fibroadenoma. Breast cancer is the most likely tumor to present as a cutaneous metastasis. A 44-years-old female presented with an irregular round, 15-cm sized protruding mass on her scalp area since one month ago. Scalp mass lesions were confirmed by histopathologic analysis as malignant mesenchymal tumor. Considering overall medical conditions detected mass on both upper lung of chest PA, Rt. mastectomy on two years ago history and abnormal CBC lab, we consulted with hemato-oncology. Tumor metastasises on Scalp, lung, vulva area and Lt. humerus bone metastasis are confirmed by PET-CT. Plastic surgeon excised scalp mass and following histopathologic analysis confirmed it as malignant phyllodes tumor. Patient was treated with medication(Doxorubicin, Ifosfamide) and radiation treatment on bone metastasis. Until now, she is under supportive care. There is no case report of malignant phyllodes tumor cutaneous metastasis. We present this case as a reminder that physicians must consider cutaneous tumor as metastasis the when a patient has breast cancer history.

        Perioperative Risk Factors associated with Immediate Postoperative Extracorporeal Membrane Oxygenation in Lung Transplants

        Background: Extracorporeal membrane oxygenation (ECMO) is administered for a few days after lung transplantation (LTx) in recipients who are expected to have early graft dysfunction. Despite its life-saving potential, immediate postoperative ECMO has lifethreatening complications such as postoperative bleeding. We investigated the risk factors related to the use of immediate postoperative ECMO. Methods: We retrospectively reviewed the records of 60 LTx patients who were at our institution from October 2012 to May 2015. Perioperative variables associated with postoperative ECMO were compared between the two groups. Results: There were 26 patients who received postoperative ECMO (ECMO group) and 34 patients who did not (control group). Multivariate regression analysis revealed preoperative ECMO (odds ratio [OR] 12.55, 95% confidence intervals [CI] 1.34 – 117.24, p = 0.027) and lower peripheral pulse oxymetry saturation (SpO2) at the end of surgery (OR 0.71, 95% CI 0.54 – 0.95, p = 0.019) were independent risk factors for postoperative ECMO in LTx patients. The incidences of complications, such as re-operation, tracheostomy, renal failure and postoperative atrial fibrillation, were higher in the ECMO group. There was no difference in the duration of postoperative intensive care unit stay or postoperative 30-day mortality between the two groups. Conclusions: The preoperative ECMO and lower SpO2 at the end of surgery were associated with postoperative ECMO. Further, postoperative adverse events were higher in the ECMO group compared with the control group. This study suggests that determination of postoperative ECMO requires careful consideration because of the risks of postoperative ECMO in LTx patients.

The mooring winch brake hold the ship in harbor. But sometimes it appeared the excessive stress and brake lining would be broken. So it is necessary to change the shape of brake system with improve the braking power. This study carried out research on optimum design like as moved the position of link pin or modified the thickness of brake band and lining.

        Bacterial profile of suction drains and the relationship thereof to surgical-site infections in prosthetic breast reconstruction

        Background Despite the increasing popularity of prosthetic breast reconstruction, scant data exist on the microbiological profile of drainage fluid from closed-suction drains and the relationship thereof to surgical-site infections (SSIs) in breast reconstruction surgery. This study aimed to determine whether bacteria isolated from drainage fluid were associated with the development of SSIs, and whether the bacterial profile of drainage fluid could be a clinically useful predictor of SSIs. Methods We performed a retrospective chart review of 61 women who underwent tissue expander/implant or direct-to-implant reconstructions. Patient demographics and culture studies of drainage fluid from suction drains collected on postoperative day 7 were evaluated. Results Sixteen patients (26.23%) were culture-positive, and 45 patients (73.77%) were culture- negative. The most frequently isolated bacteria were coagulase-negative staphylococci, followed by Staphylococcus aureus. SSIs were diagnosed in seven patients and were mostly resolved by systemic antibiotics; however, the tissue expander or implant was explanted in two patients. Positive culture of drainage fluid from closed-suction drains was significantly associated with the development of SSIs (P<0.05). The positive predictive value was 37.50%, and the negative predictive value was 97.78%. Conclusions To our knowledge, this study is the first to demonstrate a significant association between the microbiological profile of drainage fluid from closed-suction drains and the development of SSIs in patients with prosthetic breast reconstructions. The high negative predictive value suggests that microbial testing of drainage fluid from closed-suction drains may have clinical utility. Further prospective studies with larger sample sizes are required to confirm our findings.

      • Designing Hierarchical Assembly of Carbon-Coated TiO<sub>2</sub> Nanocrystals and Unraveling the Role of TiO<sub>2</sub>/Carbon Interface in Lithium-Ion Storage in TiO<sub>2</sub>

        <P>Despite the many benefits of hierarchical nanostructures of oxide-based electrode materials for lithium-ion batteries, it remains a challenging task to fully exploit the advantages of such materials partly because of their intrinsically poor electrical conductivities. The resulting limited electron supply to primary particles inside secondary microparticles gives rise to significant variation in the lithium-ion (Li<SUP>+</SUP>) storage capability within the nanostructured particles. To address this, facile annealing, where in situ generated carbon-coated primary particles were assembled into porous microagglomerates, is demonstrated to prepare nanostructured titanium dioxide (TiO<SUB>2</SUB>). A systematic study on the effect of the carbon coating reveals that it is exclusively governed by the characteristics of the TiO<SUB>2</SUB>/carbon interface rather than by the nature of the carbon coating. Depending on their number, oxygen vacancies created by carbothermal reduction on the TiO<SUB>2</SUB> surface are detrimental to Li<SUP>+</SUP> diffusion in the TiO<SUB>2</SUB> lattice, and structural distortion at the interface profoundly influences the Li<SUP>+</SUP> (de)intercalation mechanism. This new insight serves as a stepping stone toward understanding an important yet often overlooked effect of the oxide/carbon interface on Li<SUP>+</SUP> storage kinetics, thereby demanding more investigations to establish a new design principle for carbon-coated oxide electrode materials.</P> [FIG OMISSION]</BR>
