    본 연구의 목적은 동사할 지경이 되어도 스스로 상황을 극복할 방법을 찾을 수 없어 홀로 죽어가는 장애인, 장애가족의 보호·양육이 힘겨워 죽음의 길을 선택하고 마는 장애인과 장애인 가족들을 위해 장애인복지는 무엇을 어떻게 하여야 할까를 고민하면서 재가서비스가 그런 문제들을 해결하고 장애인의 삶의 질을 향상시킬 수 있는 대안적 복지서비스가 될 수 있는지를 경험적 연구를 통해 검증해 보고 이를 토대로 향후 장애인을 위한 재가서비스 제도의 확대를 제언하고자 하는 것이다.
    본 연구는 상기한 연구목적을 달성하기 위하여 첫째, 경상남도 장애인도우미뱅크의 운영과 관련된 기관 자료의 분석을 통해 경상남도 장애인도우미뱅크서비스의 현황을 파악하였으며, 둘째, 장애인과 그 가족을 대상으로 설문조사와 전화면접 결과의 분석을 통해 조사대상자들의 재가서비스 이용실태와 재가서비스 만족도, 그리고 재가서비스가 장애인의 삶의 질의 변화에 미치는 영향을 분석하였고, 셋째, 사례연구를 통하여 재가서비스가 장애인의 삶의 질에 미치는 영향을 심층적으로 분석하였다.
    연구결과는 다음과 같다.
    첫째, 경상남도 장애인도우미뱅크 서비스는 저소득층만을 대상으로 하는 선별성을 탈피하여 장애인1~2급과 정신지체·발달장애 3급이면 누구나가 사용할 수 있는 보편성을 확보하고 있으며, 2006년 12월말 현재 서비스 대상자 수는 41,488명이었다.
    재가 도우미파견 서비스는 1인이 월 총 32시간의 서비스를 이용할 수 있다. 서비스 이용료는 제공되는 도움 영역에 따라 1시간당 5,000원~10,000원이 소요되는데 총비용 중 30%는 이용자가 부담하고 70%는 지방자치단체가 지원하며, 국민기초생활보장 대상자에게는 무료로 제공되고 있다.
    가사·간병, 활동보조, 교육지원 및 장애아동의 일시적 위탁보호 등으로 구분되어 제공되는 재가서비스는 2006년 한 해 동안 총 2,511명에게 153,112건의 서비스가 제공 되었고 이 서비스에 투입된 재가 도우미 인력은 1.044명이었다.
    재가서비스를 이용하는 사람들은 남자가 1,432명, 여자가 1,079명으로 남성 장애인이 더 많았다. 연령대별로는 19세 이하의 학령기가 48.1%, 20~30세가 5.4%, 31~50세 19.1%, 50세 이상이 27.4%이고, 19세 미만의 학령기 장애인이 가장 많은 서비스를 이용하고 있는 것으로 나타났다. 장애영역별 재가 서비스 이용자는 정신지체장애인이 758명(30.2%)으로 가장 많고, 다음이 지체장애인 606명(24.1%), 뇌병변장애인 512명(20.3%), 발달장애 269명(10.7%), 시각장애 214명(8.5%)이었다. 그리고 등급별로는 1급이 1,237명, 2급이 1,001명, 3급이 273명이었는데 3급 장애인이 상대적으로 적은 것은 서비스 대상이 1~2급은 모든 영역의 장애인이지만 3급의 경우에는 정신지체·발달장애에 국한되어 있기 때문이다.
    둘째, 경상남도장애인도우미뱅크의 재가서비스를 이용한 경험이 있는 208명에 대한 설문조사와 전화면접조사의 분석결과는 다음과 같다.
    도우미뱅크의 재가서비스를 이용하기 전에 일상생활에 도움을 준 사람은 55.7%가 배우자등 가족이었고, 재가서비스 이용하게 된 후 가장 먼저 도움을 청하는 곳은 도우미뱅크가 61.5%를 차지하기는 하였으나 22.1%의 응답자가 여전히 가족들에게 가장 먼저 도움을 청하는 것으로 나타나 서비스의 양적 증대 요구와 관련이 있을 것으로 해석된다. 재가서비스의 여러 영역 가운데 가장 많이 이용되고 있는 서비스 영역은 청소·빨래, 병원·치료 센터 등 통원, 식사·반찬 만들기, 시장보기 등의 순으로 나타났는데 가장 기본적인 일상생활영역이라고 할 수 있는 의식주·건강생활과 관련된 서비스를 집중적으로 이용하고 있음을 알 수 있었다.
    한편 응답자의 86.5%가 자립생활에 많은 도움이 된다고 하였으며, 90.4%가 삶의 질 향상에 많은 도움이 된다고 응답하였고, ‘개인·가정·사회적 문제해결’에의 기여정도 역시 89%가 만족하고 있는 것으로 나타나 재가서비스로 장애인의 자립생활을 촉진하고 장애인과 가족의 삶의 질을 향상시킬 수 있음을 알 수 있었다.
    이러한 높은 만족도에도 불구하고 ‘제도 개선에 대한 요구’와 관련성을 가지고 있는 서비스의 양적 측면인 ‘서비스 이용 시간’에 대해 응답자의 63.0%가 이용시간의 확대를 요구했는데 그중 36.5%가 이용자의 환경조건에 따라 사례별로 월 90시간 이상의 서비스를 지원해 줄 것을 희망하고 있는 것으로 나타났다.
    재가서비스 이용이후 사회적 변화에 대하여는 응답자의 59.1%가 생활전반에 대해 자신감이 생겼다고 응답하였음에도 불구하고 ‘문화생활욕구’에 대해서는 응답자의 27.0%, ‘행사·신앙생활 등의 참여욕구’에 대해서는 25.0%만이 재가서비스를 받기 이전보다 욕구가 커졌다고 응답하여 아직은 재가서비스가 장애인으로 하여금 사회활동에 적극적으로 참여할 수 있는 정도의 여건을 조성해 주지 못하고 있음을 알 수 있었는데 재가 서비스의 양적 확대와 다양성 확보의 필요성을 드러내는 것으로 해석된다.

    The purpose of this study is to suggest expanding alternative plans for a welfare service system by home visiting caregivers for the handicapped, who come close to dying alone because they cannot overcome their difficulties by themselves; and for the families who are beyond their ability to guard and nurse their handicapped family member.
    Also, the purpose of this study is to solve the problem and improve the quality of the handicapped family life using the home visiting service system and by suggesting practical study for the future.
    This study analyzed to accomplish that pre described purpose as below. First this study showed the status of the management processes and material of Gyeongsangnamdo Handicapped Caregiver Bank Service. Second, this study analyzed the effect on change of quality of life for those using home visiting caregivers service. Percentage of satisfaction was derived from phone interviews and surveys of handicapped individuals and their families. Third, this study made an in depth analysis of the effect on quality of life of handicapped individuals who use home visiting service thru study cases.
    The study result is as follows.
    First Gyeongsangnamdo Handicapped Caregiver Bank avoided sorting by only low income groups. They ensure the universal concept which anyone who is classified as handicapped (class I and II) or having a mental or development disorder (class III) is able to use the service. There were 41,488 people in service in December of 2006.
    One person is allowed to use the home visiting caregiver service for a total of 32 hours in a month. The service fee is between 5,000 won and 10, 000 won per hour depending on the offering field; but 30% of total service fee is charged to user and the rest (70%) is supported by local provinces.
    The service is offered free to those on welfare. The home visiting caregiver service is offered and classified by household affairs, nursing, activity assist, educational support, and temporary entrust care for handicapped children. This service offered 153,112 cases to total of 2,511 people for the year of 2006. There were 1,044 caregivers invested for those services.
    There were more male handicapped users (1,432) than females (1,079). The breakdown in ages is: 48.1% under 19 years old, 5.4% from 20 to 30 years old, 19.1% from 31 to 50 years old, and 27.4% over 50 years old. The type of handicapped field assisted was: 758 people (30.2%) had a mental disorder which shown most percentage, 606 people had a physical disorder (24.1%), 512 people were paralyzed (20.3%), 269 people had a development disorder (10.7%), and 214 people had a visual disorder (8.5%). Separated by class of disabilities there were 1,237 people in class I, 1,001 people in class II, and 273 people in class III. The reason for the relatively small number of class III is because class 3 is limited to those people with a mental or development disorder, but class I and II covered a larger field of handicapped individuals.
    The second analysis was made through the survey and phone interviews of 208 people who used Gyeongsangnamdo Handicapped Caregiver Bank Home Visiting Service. It showed 55.7 % of their family members or spouses helped the handicapped individual in their daily life before they used the home visiting caregiver service. Home visiting caregiver service bank took 61.5% help requests after they started using home visiting caregivers service; however, 22.1% of the handicapped individuals using the service preferred to ask for the help of family members first.
    So the figure of 22.1% above is related to the requirement for increasing the quantity of service. It showed that most used service field was in order: house cleaning, laundry, transfer to clinic or treatment center, making meals and grocery; however, the service has been used most for the basic necessities which are food, clothing and shelter.
    In addition, 86.5% of the respondents stated the service helped them live independently. 90.4% of respondents stated the service helped improve their quality of life. 89% of respondents were satisfied that the home visiting caregiver service contributed to solve their individual, family and social problems. The respondents agreed the service greatly improved their feeling of independence and their overall quality of family life.
    Regardless of the high percentage of satisfaction, 63% of respondents questioned the “quantity of service time period” which related with ‘the requirement of system improvement’ and requested to enlarge using hours. 36.5% out of these respondents hope to get 90 hours of monthly service support based on their special cases.
    After they use home visiting caregiver service, 59.1% of respondents stated they have more of a self confident life now that they have had in the past and see more of a social change. In spite of this, 27.0 % of these respondents desire a greater cultural life and 25% desire a greater participation in events and religious life than before they received the home visiting caregiver service. This shows the home visiting caregiver service does not build up enough of a good environment yet to allow handicapped individuals to participate in a more positive social life. So it explains that there is a necessity for variety to ensure and enlarge that quantity of home visiting caregiver service.
    The main factor of home visiting service which related to quality of handicapped life was the service using hours and time period. It showed a high degree of life change and satisfaction of service from the handicapped individuals that were given service continuously 4 times a week on a regular schedule. This proved the efficiency of the social welfare service by repeatability and durability of social welfare service.
    Third, it confirmed the following changes for quality of handicapped life by taking home visiting service from qualitative analysis about individual cases collected from service users.
