The aim of the present was to investigate the effects of stroke on Basic IQ and Executive IQ, to examine the effects of stroke-related lesion locations on Basic IQ and Executive IQ, and to examine the effects of Basic IQ and Executive IQ on ADL of stroke patients. The study's subjects were 108 patients that were hospitalized in a general hospital and a rehabilitation hospital, in Daejeon, with the diagnosis of stroke. The assessment of cognitive function was done using the Cognitive Scale for Older Adults(CSOA) and the assessment of and ADL was done using Assessment of Motor and Process Skills(AMPS). We compared effects of stroke on Basic IQ and Executive IQ, which are the summary scores of CSOA, and also compared the effects of presence/absence of prefrontal/basal ganglia lesions on the Basic IQ and Executive IQ. Furthermore, we examined whether Basic IQ and Executive IQ had differential influences on motor skills and process skills, which are the summary indices of AMPS. The main results were as follows.
First, strokes had more severe harmful influences on Basic IQ relative to Executive IQ.
Second, presence of the frontal lobe/basal ganglia had more severe harmful influences on Executive IQ than Basic IQ.
Third, Executive IQ had more severe harmful influences on ADL than Basic IQ
These results indicate that Executive IQ deficit is the core of cognitive impairment associated with strokes and the impairment of Executive IQ was related to the lesion on the frontal lobe and the basal ganglia in a stroke patient. Furthermore, while both basic and executive intelligence affected the motor skills of the ADL, only executive intelligence affected the process skills. The most significant results was that Executive IQ is a significant influence factor in performing ADL, suggesting the cognitive aspect of ADL. Therefore, the importance of executive IQ must be stressed in a stroke patient's rehabilitation assessment and intervention plan.
