임플란트주위염은 임플란트의 장기적 예후를 위협하는 주요 합병증으로, 다양한 전신질환이 위험인자로 보고되고 있다. 특히, 골다공증은 골 대사에 직접적인 영 향을 미쳐 임플란트 주위 골소실을 가속화할 수 있으나, 이에 대한 임상적 근거 는 제한적이다. 이번 연구는 50세 이상 여성 환자를 대상으로 골다공증 유무에 따른 임플란트주위염 심화 영향과 변연골 소실 차이를 분석하고자 하였다. 2010년 1월부터 2020년 1월까지 전자의무기록과 방사선 영상을 후향적으로 검토하여 최소 5년 이상 추적 관찰이 가능한 환자를 선별하였으며, 골다공증군 은 79명(나이 72.86 ± 8.82세), 258개의 임플란트, 대조군에서는 69명(나이 66.25 ± 7.48세), 239개의 임플란트가 포함되었다. 임플란트주위염은 변연골 소 실 3 mm 이상과 임상적 증상(bleeding on probing [BOP], suppuration [SUP]) 중 하나 이상이 동반되거나, 임플란트 제거가 이루어진 경우로 정의하였 다. 변연골 소실은 식립 직후와 최종 추적 방사선 영상에서 fixture shoulder와 근원심 변연골정 사이 거리를 측정하여 산출하였다. 통계분석은 chi-square test, independent t-test, 단변량 로지스틱 회귀분석을 시행하였다. 임플란트주위염 발생률은 골다공증군 8.9%, 대조군 5.0%였으나, 통계적으로 유 의하지 않았다. 변연골 소실량은 골다공증군에서 0.85 ± 1.80 mm, 대조군에서 0.55 ± 1.32 mm로 나타났으며, 골다공증군에서 유의하게 더 컸다(p = 0.034). 평균 추적 관찰 기간은 골다공증군은 8.26년, 대조군 8.40년이며 연간 변연골 소실률은 골다공증군 0.11 mm/yr, 대조군 0.07 mm/yr로 나타났다. 민감도 분 석에서, 골소실 4 mm 이상 기준으로 한 로지스틱 회귀 결과, 골다공증군의 임플 란트주위염 위험은 정상군 보다 약 2.3배 높았으며, 통계적으로 유의하였다(OR = 2.26; 95% CI, 1.02–5.05; p = 0.046). 추가 분석에서 Monje 등의 severity classification을 적용했을 때도 골다공증군에서 중등도 이상 임플란트주위염 비 율이 더 높았으나, 통계적 유의성은 일관되게 확인되지 않았다. Kaplan–Meier 분석 결과, 골다공증군의 누적 생존율은 94.6%, 대조군은 96.2%였으며, 두 군 간 차이는 통계적으로 유의하지 않았다. 그러나, 2007년 ICOI 기준에 따른 임플 란트 성공률은 골다공증군에서 84.9%, 정상군에서 92.1%로 나타나, 생존율과 달리 정상군에서 유의하게 높은 성공률이 관찰되었다(χ² = 5.51, p = 0.02). 한 편, 임플란트 직경과 길이는 임플란트주위염 발생과 유의한 관련성을 보이지 않 았으나, 악궁 위치는 주요한 요인으로 확인되었다. 하악에 식립된 임플란트는 상 악에 비해 임플란트주위염 발생 위험이 유의하게 낮았다(OR = 0.40, 95% CI: 0.17–0.91, p = 0.030). 골다공증 환자에서 임플란트주위염 발생 위험과 변연골 소실 경향이 높게 나타 났으며, 특히 엄격한 진단 기준(변연골 소실 ≥ 4 mm)에서는 통계적으로 유의한 위험 증가가 확인되었다. 이번 연구 결과는 임플란트 치료 전 골대사 상태 평가 의 필요성을 시사하며, 특히 골다공증 환자와 같은 위험군에서는 정기적인 supportive peri-implant therapy(SPT)를 통한 주기적인 점검과 유지관리가 임 플란트의 장기적 성공에 핵심적임을 강조한다.

Peri-implantitis is a major complication that compromises the long-term prognosis of dental implants, and various systemic conditions have been identified as potential risk factors. Osteoporosis, in particular, affects bone metabolism and may accelerate peri-implant bone loss; however, clinical evidence on this association remains limited. This study aimed to evaluate the impact of osteoporosis on peri-implantitis severity and marginal bone loss (MBL) in women aged 50 years and older.
Electronic medical records and radiographs from January 2010 to January 2020 were retrospectively reviewed, and patients with at least 5 years of follow-up were included. The osteoporosis group consisted of 79 patients with 258 implants, and the control group included 69 patients with 239 implants. Peri-implantitis was defined as marginal bone loss ≥ 3 mm accompanied by at least one clinical symptom (bleeding on probing [BOP] or suppuration [SUP]) or implant removal. Marginal bone loss (MBL) was measured as the distance between the implant fixture shoulder and the mesial/distal alveolar bone crest at baseline and final follow-up. Statistical analyses included chi-square test, independent t-test, and univariate logistic regression.
The incidence of peri-implantitis was higher in the osteoporosis group (8.9%) than in the control group (5.0%), although the difference was not statistically significant (χ² test, p > 0.05). Mean MBL was significantly greater in the osteoporosis group (0.85 ± 1.80 mm) compared with the control group (0.55 ± 1.32 mm; p = 0.034). No significant differences were found in mean follow-up duration (osteoporosis group: 8.38 years; control group: 8.40 years; p > 0.05) or annual bone loss rate (0.11 mm/yr vs. 0.07 mm/yr; p = 0.071). In sensitivity analysis, using a stricter diagnostic threshold of ≥ 4 mm bone loss, logistic regression revealed that the risk of peri-implantitis was significantly higher in the osteoporosis group (OR = 2.26; 95% CI, 1.02–5.05; p = 0.046). Additional analysis applying Monje’s severity classification showed a higher proportion of moderate-to-severe peri-implantitis in the osteoporosis group, although statistical significance was not consistently demonstrated. Kaplan–Meier analysis indicated cumulative survival rates of 94.6% in the osteoporosis group and 96.2% in the control group, with no statistically significant difference between the groups (Fisher’s exact test, p = 0.402).
According to the 2007 ICOI criteria, the implant success rate was significantly lower in the osteoporosis group (84.9%) than in the control group (92.1%) (χ² = 5.51, p = 0.02). Moreover, although implant diameter and length were not associated with peri-implantitis occurrence, jaw location was identified as a significant factor; implants placed in the mandible exhibited a significantly lower risk of peri-implantitis compared with those placed in the maxilla (OR = 0.40; 95% CI, 0.17–0.91; p = 0.030).
Osteoporosis was associated with greater peri-implant bone loss and a higher likelihood of peri-implantitis. Notably, under stricter diagnostic criteria (≥ 4 mm bone loss), osteoporosis significantly increased the risk of peri-implantitis. These findings underscore the importance of evaluating bone metabolism before implant placement and highlight the need for regular supportive peri-implant therapy, particularly for high-risk populations such as patients with osteoporosis.
