양극성장애를 겪고 있는 딸에 대한 가족치료 사례연구 : 가족상호작용방식을중심으로 = Family Therapy for Bipolar Disorder: A Case Study Examination of Family Interaction Methods

Objectives: This study investigated the effectiveness of family therapy interventions for enhancing the interactions of a family with a daughter diagnosed with bipolar disorder. The therapist used an MRI interactional model and Bowen Family Systems theory as methods of treatment. Methods: The researchers employed open coding and Miles and Huberman’s matrix and network display to analyze the data from the family therapy case. Results: Three failed solution attempts by the family were identified in the study: an aversive method, a detractive method, and an offensive method. The factors influencing these dysfunctional family interactions were marital conflicts, the husband and wife’s characteristics related to their families of origin, the husband’s affair, and the wife’s jealousy. The therapist intervened to enhance their interactions by probing attempted solutions, suggesting new solutions, and inducing change by strengthening family members’ insights. Three functional interactive methods evolved from the family therapy intervention: a reacting method, a favorable method, and an inoffensive method. Conclusions: The findings of this study showed that the daughter’s bipolar symptoms were associated with her family members’ interaction methods. Furthermore, the researchers found that the symptoms of bipolar disorder may be alleviated through functional interaction methods implemented with the entire family.

국문 초록 (Abstract)

본 연구는 양극성장애를 겪고 있는 딸(IP)에 대한 가족치료를 통하여 가족상호작용을 향상시키는 개입의 효과를 밝히고자 하였다. 본 사례에 적용된 이론적 준거 틀은 MRI의 상호작용적 가족치료모델과 Murray Bowen의 가족체계이론이다. 본 연구는 질적 데이터를 분석하기 위하여 개방코딩을 사용하였고 Miles와 Huberman이 제시한 매트릭스와 네트워크 방법을 활용하였다. 본 연구의 결과는 다음과 같다. 첫째, 내담자 가족이 시도했던 상호작용의 방식은 역기능적 교류방식이었으며, 회피하는 방식, 비난하는 방식, 공격적인 방식이 포함되었다. 둘째, 내담자 가족의 상호작용을 어렵게 만든 요인은 부부갈등이었는데, 남편의 원가족 특성, 남편의 외도, 남편의 의처증, 부인의 원가족 특성이 포함되었다. 셋째, 내담자 가족의 상호작용을 개선시키기 위한 치료자의 개입기술은 시도된 해결책 탐색과 새로운 해결책 제시, 통찰력 강화를 통한 인식변화 유도로 나타났다. 넷째, 가족치료를 통하여 변화된 가족상호작용방식은 기능적 교류방식이었는데, 반응하는 방식, 호의적인 방식, 비자극적인 방식으로 나타났다. 다섯째, 상담회기가 진행되면서 딸은 기능적인 상호작용방식을 사용하게 되었다. 본 연구의 결과는 가족구성원의 양극성장애가 가족구성원들의 상호작용방식과 연관이 있었으며, 기능적인 상호작용 방식에 의해서 내담자의 양극성장애 증상을 호전시킬 수 있다는 것을 보여준다.
