    염증성 피부질환은 지속적 또는 재발성 염증을 특징으로 하며, 병변 조직과 전신 면역 반응이 모두 작용하는 다요인성 면역병리기전을 가진다. 아토피피부염, 전신 농포성 건선, 판상형 건선, 화농성 한선염, 백반증을 포함한 염증성 피부질환은 임상 양상에서 중복된 부분도 있으며 병태생리적 이질성과 복잡성도 있으나 일부 분자 기전이 중첩된다. 현재의 진단 및 치료 전략은 임상 표현형에 주로 의존하고 있어, 각 질환 내 또는 질환 간 존재하는 면역학적 이질성을 충분히 반영하지 못한다. 그 결과 많은 염증성 피부질환에서 병태생리는 부분적으로만 이해되고 있으며 적지 않은 환자들이 불완전한 반응이나 일시적인 치료 효과만을 경험하고 있다.

    아토피피부염은 가장 유병률이 높은 염증성 피부질환 중 하나로, 발병 연령, 임상 양상 및 질병 경과에 있어 이질성이 크다. 본 연구에서는 질량세포분석(mass cytometry)과 단일세포 전사체 분석(scRNAseq analysis)을 활용하여 아토피피부염의 전신 면역 환경을 분석하였으며, 성인과 소아 아토피피부염 간 차이를 탐색하였다. 혈액 내 cDC1의 빈도가 성인 아토피피부염에서 유의하게 감소하였으며, 질병 중증도와 반비례 상관관계를 보였다. cDC1에 피부 림프구 항원 유전자(CLA)와 케모카인 수용체(CCR)가 증가되어 있었으며 병변 내 cDC1의 분포는 증가하여 아토피피부염 cDC1의 피부로의 이동을 사사한다. 성인 아토피부염에서는 Th2 유전자 발현 이외에 Th1, Th17, Th22 신호 또한 증가하였으며 JAK/STAT 경로 유전자가 다양한 세포에서 증가하였다. 소아 아토피피부염에서는 Th2 유전자 발현이 증가하였고, CD6-ALCAM 경로가 특이적으로 활성화되어 CD4 T 세포에는 CD6가, 수지상세포 중 cDC2에서 주로 ALCAM이 크게 증가하였다. 두번째 부분에서는 IL-12/23 또는 IL-17A blockade 치료 전후 GPP 병변을 분석하여 IL-36G/IL-17/IL-12/23가 관여하는 병인성 염증 루프 (pathogenic loop)를 규명하였다. 치료 후 IL36G, CCL20, IL17C, CXCL8, CSF2 등 염증 및 호중구 관련 유전자 발현이 감소하고, Th17/Th1 signature이 약화되면서 임상적 호전과 연관됨을 확인하였다.

    세번째 부분에서는 아토피피부염, 농포성 건선, 판상형 건선, 화농선 한선염, 백반증을 아우르는 통합 다중오믹스 단일세포 아틀라스를 구축하였다. 이 아틀라스는 피부조직의 단일세포 전사체, 공간 전사체 분석과 말초혈액 단핵구의 단일세포 전사체 및 질량 세포 분석을 포함하며 100만개 이상의 세포를 포괄한다. 이를 통해 질환 간 층화를 수행한 결과, 조직 수준에서는 AD는 PSO와 인접하지만 독립된 클러스터를 형성하고, GPP와 PSO는 부분적 중첩이 있었으며 HS는 독립적으로 클러스터링 되는 것을 확인하였다. 이러한 현상은 질환 특이적 사이토카인 환경에 의해 형성되는 각질형성세포의 이질성이 주요 결정 인자일 가능성을 확인하였다. 또한 아틀라스를 통해 질환 특이적 면역 지형을 분석하여 잠재적 약제 표적을 탐색하였다. 본 연구는 단일세포 전사체, 단일세포 공간 전사체 기법을 활용하여 염증성 피부질환의 세포 아틀라스를 구축하였으며, 질환 특이적 및 공통 면역병리기전을 제시하였다. 해당 아틀라스는 염증성 피부질환의 분자적 층화와 면역학적 공통적 및 특이적 병태생리기전을 이해하는 데 기여하며, 향후 치료 타겟 발굴, 환자 계층화 전략 및 치료 반응 예측에 기여하는 중개 연구 플랫폼을 제공한다.

    Inflammatory skin diseases, such as atopic dermatitis (AD), generalized pustular psoriasis (GPP), plaque psoriasis (PSO), hidradenitis suppurativa (HS) and vitiligo, are a heterogeneous group of chronic disorders characterized by persistent or recurrent inflammation, and often have complex, multifactorial immunopathomechanisms involving complex interplay between local tissue and systemic immune responses. These diseases share overlapping clinical features and some overlapping molecular mechanisms, despite heterogeneity and complex pathophysiology. Despite significant advances in clinical management and the development of targeted biologics, current diagnostic and therapeutic strategies remain largely phenotype-driven and do not fully capture the underlying heterogeneity within and across the diseases. As a result, the pathophysiology of many inflammatory skin diseases remains only partially understood, and a considerable proportion of patients experience incomplete or transient therapeutic responses.

    In the first part of this thesis, I aimed to characterize the systemic immune landscape of AD, which is clinically heterogeneous and exhibits variability in the age of onset, clinical presentation and disease course, at the single-cell level using mass cytometry and single-cell RNA sequencing (scRNAseq). In adult AD, the frequency of circulating conventional dendritic cell (cDC) 1 was significantly decreased in AD. cDC1 further decreased in the severe group compared to mild and moderate groups, and its frequency was negatively correlated with disease severity scores and serum immunoglobulin E levels. The reduced circulating cDC1 frequency might reflect migration to the skin, as supported by increased expression of skin homing molecules, including SELPLG (gene encoding cutaneous lymphocyte antigen) and chemokine receptors on circulating cDC1. In addition, cDC1 was increased in AD lesional skin compared to control skin. Profiling both adult and pediatric AD revealed elevated T helper type (Th) 2 gene expression in both adult and pediatric AD as expected. However, Th1, Th17 and Th22 signatures were elevated prominently only in adult AD. In addition, the decreased circulating cDC1 was a unique characteristic of adult AD, as its frequency did not change in pediatric AD. I further explored the differences and unique pathogenic axis of adult AD or pediatric AD to elucidate within disease heterogeneity and identify unique molecular drivers in adult or pediatric AD. In the second part of the thesis, I characterized the pathogenic axis of GPP by profiling GPP lesions before and after treatment with IL-12/23 or IL-17A blockade. After treatment, both patients responded clinically and had clear skin. There was attenuation of genes including IL36G and CCL20 leading to the reduction of inflammatory signals including Th17 and Th1 signatures, and attenuation of IL17C, CXCL1, CXCL8 and CSF2 leading to downregulation of neutrophil response. This suggests that there is a pathogenic loop involving IL-36G/IL-17/IL-12/23 in GPP, and that disruption of this loop leads to successful clinical improvement in GPP patients.

    The underlying pathomechanism of each inflammatory skin disease is not fully understood. As inflammatory skin diseases have overlapping but differing phenotypes, to unravel this, in the third part of the thesis, I constructed a single-cell atlas encompassing AD, GPP, PSO, HS and vitiligo. This atlas includes scRNAseq and spatial transcriptomics of skin tissue as well as scRNAseq and mass cytometry analyses of peripheral blood mononuclear cells, covering over one million cells. Then I suggest two strategies that the atlas could be applied: cross-disease stratification, and identification of druggable targets by investigating heterogeneity in immune signatures. Tissue analysis revealed distinct molecular stratification, with AD forming a unique but PSO-adjacent group, PSO and GPP showing overlap, and HS forming a distinct cluster. As keratinocytes showed distinct features, I uncovered heterogeneity in keratinocytes shaped by disease-specific cytokine milieus to be a major determining factor in disease stratification using the tissue dataset. Disease-specific and shared immune circuits were delineated, and potential drug targets were explored. This study provides insight into the immune profiles of individual diseases, and also provides a comprehensive atlas of inflammatory skin diseases, uncovering disease stratifying immune landscapes. This resource not only furthers mechanistic understanding of disease pathophysiology but also establishes a translational platform that can advance therapeutic targeting and molecularly guided patient stratification.
