This study aims to explore practicability in implementation of holistic care management and provide the foundation for the cvaluation system by investigating the care managers’ framework on successful concept of it and corrclations between its factors to success and implementability in field
It inductively comes to formulate and frame practice wisdom in the successful implementation of holistic care management by utilizing the Concept Mapping method on care managers who led a pilot project for Households in
2009.
In order to structuralize the success factors, 342 statements were collected
after brainstorming among the 31 participants. And then 12 managers of welfare offices, whose in-field working experiences are longer than three years, in regions concerned narrow down those statement to society-wise
statements by performing the sorting and rating through the idographic approach and detect the success factors by analysing the collected data for each statement through nomothetie approach. That is‘ data for specific success
factors to the holistic care management were drawn from the care managers' first hand experience, and were statistically analysed and classified.
The resulted success factors include ’ the degree of care managers' capability and support(4.20) ’, ’construction of care management system(4.14) ,'the dynamic of network and budget(4.08)', ’training and treatment of care
managers(4.08)’, ’promotion and sponsor( 4.00)‘, ’client card(3.99)’, and ’job-related assignment for care managers’ in the order of significance Noticeably, the direct and indirect factors, such as the assignment, support, treatment given to them as well as on their qualifications, serve as the important variables and, consequently, the care manager playes the biggest role
in determining the success or failure Meanwhile, the implementability rates at relatively low, which implies that
the care managers are not content with their own capabilities for implementation and faced with the limitation to it. It also suggests that it is urgent to provide the care managers with professional and various education
programmes and to build institutionalized measures such as the introduction of managerial system, for example, Super Vision, consulting and advisory agency, the improvement of working condition inc1uding an employment stability,and the organization of task-force. The empowerment of the care managers and well-organized system can foster favorable physical environments for the rest of factors, such as the dynamic of network budget, promotion, sponsor and so
on, to function well to attain successful performance
In spite of the increasing importance of roles of both public and private institutions in the client management, the client management has not been effectively and efficiently conducted. To overcome this problem‘ both public
and private institutions should understand cooperate each other and, if possible and favorably, come up with a concrete guideline to their roles respectively
In sum, at the time of complication and chaos in care management and implementation, it is worth examining the success factors to implementation in holistic care management based on practice wisdom and discussing the way ahead.
